Predictive factors for facial nerve palsy in malignant otitis externa: a TriNetX data analysis.

BACKGROUND: Malignant otitis externa is a life-threatening infection in which facial nerve palsy is a morbid complication. This TriNetX study identified predictors of facial nerve palsy in malignant otitis externa. METHODS: Retrospective analysis compared malignant otitis externa-only (n = 12 032) versus malignant otitis externa plus facial nerve palsy (n = 719) cohorts for demographics, biochemical markers and co-morbidities using t-tests and odds ratios (p < 0.05). RESULTS: Male gender (p = 4.65 × 10-15) and Hispanic and/or Latino ethnicity (17.66 vs 13.02 per cent, p = 0.0014) predicted f

Malignant Otitis Externa Presenting With Nasopharyngeal Hematoma: A Case Report of an Unusual Complication.

OBJECTIVE: Malignant otitis externa (MOE) is a potentially life-threatening infection of the external auditory canal (EAC), often with skull base extension. Vascular complications are exceptionally rare, with only six reports of petrous internal carotid artery (ICA) pseudoaneurysm following MOE. METHODS: We present the first reported case of a nasopharyngeal hematoma as a complication of MOE. RESULTS: An 82-year-old male with poorly controlled diabetes presented with 2 months of recurrent left ear otalgia, purulent otorrhea, and facial nerve palsy. He was commenced on 6 weeks of antimicrobia

Malignant Otitis Externa in Central Australia: A 15-Year Retrospective Review Between 2009 and 2024.

We performed a 15-year retrospective audit of all cases of malignant otitis externa (MOE) referred to Alice Springs Hospital in Central Australia from 2009 to 2024, the largest Australian series of MOE. Data on demographics, risk factors, microbiological culture results, management and outcomes were collected, identifying 64 cases of MOE, with the incidence increasing over time. The incidence of MOE was associated with Indigenous status and the presence of diabetes mellitus and chronic kidney disease. Pseudomonas, Staphylococcus aureus and fungal pathogens predominate as causal agents. Complic

Diagnostic Test Accuracy and Semi-Quantitative Metrics of 18F-FDG PET in Assessing Treatment Response in Skull Base Osteomyelitis and Necrotising Otitis Externa: A Systematic Review and Meta-Analysis.

BACKGROUND/OBJECTIVES: Skull base osteomyelitis and necrotising otitis externa require prolonged antibiotic therapy, yet determining optimal treatment cessation timing remains challenging. Conventional imaging modalities demonstrate persistent abnormalities beyond infection resolution, confounding treatment decisions. This systematic review evaluated the diagnostic test accuracy of 18F-fluorodeoxyglucose positron emission tomography (18F-FDG PET) for treatment response monitoring in skull base osteomyelitis and necrotising otitis externa. METHODS: We conducted a systematic review following PR

Antibiotic-Impregnated Calcium Sulfate Beads for Local Therapy of Severe Skull Base Osteomyelitis.

We report on a patient with severe progressive osteomyelitis of the lateral skull base, despite targeted long-term antibiotic therapy. Successful treatment and recovery of the facial nerve palsy were achieved through additional surgical debridement and insertion of synthetic calcium sulfate beads as a local antibiotic carrier to the skull base. We describe the characteristics of this novel biodegradable long-term release matrix and the surgical technique in detail. The patient is free from recurrence 2 years postoperatively.

Intrapetrous Internal Carotid Artery Pseudoaneurysm Secondary to Malignant Otitis Externa: A Case Report.

Malignant otitis externa (MOE) can rarely progress to skull base osteomyelitis (SBO) with catastrophic vascular complications. We describe a 68-year-old man with uncontrolled diabetes (HbA1c 9%) who presented with left otalgia, purulent otorrhea, and hearing loss. Initial workup showed CRP 23 mg/L, leukocytes 8,000/µL, and ear pus culture positive for Pseudomonas aeruginosa. Computed tomography (CT) and magnetic resonance imaging (MRI) demonstrated MOE with temporal bone osteomyelitis. Despite prolonged intravenous antibiotics (imipenem, colistin) and topical ofloxacin, symptoms persisted. Thr

Microbial biodegradation of polyethylene terephthalate microplastics by an indigenous Candida tropicalis strain and biocompatibility evaluation of microplastics-degraded metabolites in GIFT Tilapia.

Microplastic pollution is known to impact various abiotic and biotic components of ecosystems. Among the various microplastic remediation methods, biological approaches using microbes have gained significant attention due to their high efficacy and eco-friendly nature. In this study, the degradation of PET-based microplastics was carried out by the indigenous yeast strain Candida tropicalis in in vitro batch degradation experiments. The degradation was measured by monitoring the optical density of the yeast grown on a medium containing microplastic pieces, as well as assessing weight reduction

Defining the Appropriate Length of Antimicrobial Therapy for Skull Base Osteomyelitis.

OBJECTIVES: Skull base osteomyelitis (SBO) is a rare infection comprising either the lateral (LSBO), central (CSBO), or both lateral and central (BSBO) skull base. Due to its rarity, the optimal length of antimicrobial therapy (AMT) has not been well defined. Therefore, the purpose of this study was to identify factors that may influence outcomes of SBO and formulate clearer recommendations on AMT duration. METHODS: A multi-institutional retrospective cohort study of SBO patients was conducted. Patient demographics, medical comorbidities, presenting symptoms, laboratory results, radiographic

Hyperbaric oxygen therapy for wounds of the face, head and neck.

PURPOSE OF REVIEW: Wounds within the face, head, and neck region can have devastating aesthetic, functional, and potentially fatal outcomes for patients and often require multimodality treatment. Here we discuss the role of hyperbaric oxygen for a variety of wounds within the head and neck region and the clinically associated outcomes. RECENT FINDINGS: Hyperbaric oxygen (HBO) treatment can be useful in the management of wounds and infections within the head and neck region. While there are few randomized controlled clinical trials evaluating the effectiveness of HBO therapy, there is a large

Expanding the diagnostic spectrum of malignant otitis externa: a case report of proteus infection in a non-immunosuppressed patient.

INTRODUCTION: Malignant otitis externa (MOE) is a severe and potentially life-threatening infection of the external auditory canal, most frequently caused by Pseudomonas aeruginosa. Although typically seen in immunocompromised individuals-especially those with diabetes mellitus-up to 45% of cases have been reported in non-diabetic patients, highlighting the importance of recognizing atypical presentations. Rare pathogens, such as Proteus mirabilis, have been identified in only a few documented cases. Early diagnosis and prompt intervention are essential to prevent serious complications, includ

An Unusual Presentation of Nectrotizing Otitis Externa in an Elderly Patient.

Necrotizing otitis externa (NOE) is a rare but life-threatening condition that can often be challenging to diagnose. In this case report, we describe a rare presentation of NOE in an 83-year-old man with subtle signs of multiple unilateral cranial nerve palsy (VII, VIII, IX, X, XII). Prior to this, he had a three-month history of bilateral otitis externa, managed with topical and oral antibiotics. NOE was confirmed on CT head, which revealed left-sided skull base osteomyelitis (SBO). A repeat CT scan revealed left sigmoid sinus thrombosis and intracerebral abscess, indicative of disease progre
