An Unusual Presentation of Nectrotizing Otitis Externa in an Elderly Patient.

Necrotizing otitis externa (NOE) is a rare but life-threatening condition that can often be challenging to diagnose. In this case report, we describe a rare presentation of NOE in an 83-year-old man with subtle signs of multiple unilateral cranial nerve palsy (VII, VIII, IX, X, XII). Prior to this, he had a three-month history of bilateral otitis externa, managed with topical and oral antibiotics. NOE was confirmed on CT head, which revealed left-sided skull base osteomyelitis (SBO). A repeat CT scan revealed left sigmoid sinus thrombosis and intracerebral abscess, indicative of disease progre

Otitis Externa and Malignant Otitis Externa-for the Hospitalist/Internist.

Otitis externa has varying degrees of chronicity and severity. Acute otitis externa refers to a process of less than 6 weeks' duration, while chronic otitis externa lasts more than 3 months. Treatment of both includes ototopical agents and appropriate aural hygiene. Chronic otitis externa often requires routine debridement by an otolaryngologist. Malignant otitis externa is now referred to as osteomyelitis of the temporal bone. Patients will often have excruciating pain on examination. Granulation tissue at the cartilaginous-bony junction is a hallmark of the disease, but not always encountere

Cutaneous Infections Caused by Pseudomonas aeruginosa. Clinical Presentation, Diagnosis and Treatment.

Pseudomonas aeruginosa is a gram-negative opportunistic bacillus of great relevance in dermatology due to its ability to cause a wide spectrum of skin infections. These infections can be characterized by a greenish discoloration and a distinctive odor and can range from mild signs, such as folliculitis, green nail syndrome, hot hand-foot syndrome, and uncomplicated acute otitis externa, which generally respond to conservative therapy and topical antibiotics, to deeper and potentially severe infections, such as subcutaneous nodules, malignant otitis externa, ecthyma gangrenosum, and necrotizing

Malignant Otitis Externa Complicated by Skull Base Osteomyelitis Presenting as Vernet's Syndrome with Central Venous Thrombosis.

Skull base osteomyelitis (SBO) is a rare disease that often has delayed presentation in clinical settings. SBO is often misinterpreted as skull base malignancy, thus posing a challenge in diagnosis. We submit a case that presented with chronic otitis media with multiple cranial nerve palsies. His diagnosis was challenged in view of nonspecific imaging findings and odd presentation, which caused a delay in diagnosis. Methicillin-resistance coagulase-negative Staphylococcus (MRCONS) was isolated from the drained pus. MRCONS usually presents with an indwelling medical device or in severe burn cas

Patient cases with malignant otitis externa at the university clinic of Ludwig Maximilians University Munich from 2009 until 2020.

Malignant otitis externa (MOE) is a rare, potentially life-threatening disease. It involves inflammation of the external auditory canal with concomitant osteomyelitis of the temporal bone, usually with evidence of Pseudomonas aeruginosa. The patient population is often immunocompromised, mostly due to inadequately treated type 2 diabetes mellitus or the use of immunosuppressants. Erosion of the temporal bone is common and can lead to complications such as peripheral facial nerve paralysis, meningitis, and/or cochlear and vestibular damage. The treatment of choice is usually several weeks of an

Extensive Base of Skull Osteomyelitis: Why Does It Still Occur?

Skull base osteomyelitis (SBO) is a rare yet serious intratemporal infection that often masquerades as a skull base malignancy. It is most common in diabetic and immunocompromised patients. We present a case of an elderly diabetic patient with end-stage renal disease with progressive malignant otitis externa. The disease progressed to involve the base of the skull, causing multiple cranial neuropathies. Early initiation of intravenous (IV) antibiotics, along with supportive treatment, may improve the long-term prognosis of the disease. This case highlights the importance of keeping a high inde

Reassessing the Utility of Surgical Intervention for Skull Base Osteomyelitis: A 16-Year Experience.

OBJECTIVE: The role of surgery in lateral skull base osteomyelitis (SBO) is controversial. Surgical intervention is often requested by consulting services in the interest of additional culture data to inform medical management. However, whether surgery alters subsequent antibiotic treatment or modifies disease outcome remains unknown. The aim of this study was to investigate the role of surgical intervention in the treatment of SBO by (1) comparing nonsurgical and surgical culture data and (2) assessing clinical outcomes and treatment course following surgical intervention. STUDY DESIGN: Retr

Role of follow-up gallium scintigraphy in the evaluation of malignant external otitis (skull base osteomyelitis): A case report.

Malignant otitis externa (skull base osteomyelitis) can be fatal and long-term antibiotic therapy is recommended. Despite being potentially fatal, this infection causes minor changes in inflammatory biomarkers (white blood cell count and C-reactive protein levels) upon blood testing. Computed tomography and magnetic resonance imaging changes persist over a long period. Therefore, it is difficult to determine the optimal time for the discontinuation of antibiotics. We present a 77-year-old male whose medical history included type 2 diabetes mellitus who suffered from chronic otitis media with P

Hyperbaric oxygen therapy in malignant otitis externa: a retrospective analysis.

PURPOSE: Malignant otitis externa (MOE) is a rare form of invasive osteomyelitis of the external ear canal. It is typically caused by Pseudomonas aeruginosa in immunocompromised patients. The diagnosis is clinical, and the initial treatment involves systemic antibiotics or antifungal therapy. Surgery is usually only considered when medical treatment has failed. Although hyperbaric oxygen therapy (HBOT) is recommended for refractory osteomyelitis, there are no specific guidelines for MOE. METHODS: This is a retrospective study that evaluates clinical data, treatment, and results obtained in pa
