Cost-effectiveness of the hepatitis B birth-dose vaccine in South Africa: a mathematical modelling analysis.

BACKGROUND: In 2022, South Africa had an estimated hepatitis B prevalence of 4·7-6·0%. In an effort to eliminate viral hepatitis as a public health threat, a targeted (ie, selective) hepatitis B birth-dose vaccination (HepB-BD) policy was introduced in 2023. Under this policy, only infants born to mothers with confirmed hepatitis B positivity are eligible for HepB-BD, which is given in addition to peripartum antiviral prophylaxis (PAP) for mothers from the second or third trimester of pregnancy. This policy differs from the WHO recommendation that all newborns should receive a HepB-BD vaccine

A Case Report of Lyme Carditis Presenting as New-Onset Atrial Flutter in the Emergency Department.

Lyme disease (LD) is the most common vector-borne disease in the United States and has several well-known pathologic sequelae if not treated, including third-degree atrioventricular block. However, an association between Lyme carditis and atrial arrhythmias has been described in some cardiology literature but has not been reported in emergency medicine or wilderness medicine literature. This case report describes an older adult with new-onset atrial flutter with rapid ventricular rate in the setting of early disseminated LD, who ultimately was treated with antibiotics, had resolution of atrial

First records of Borrelia burgdorferi sensu lato in voles and selected Apodemus species in Serbia.

BACKGROUND: Voles and Apodemus mice are recognized as important hosts for numerous parasites and pathogens. Understanding the occurrence of zoonoses, key host characteristics, and the factors influencing their population dynamics and distribution is essential for predicting disease occurrence and assessing the risks of transmission and spread. Determining the role of voles and field mice, in maintaining Borrelia burgdorferi, s.l. in the environment clarifies its transmission during tick activity periods and highlights the need for timely monitoring and control measures. RESULTS: We examined t

PREnatal Syphilis point-of-care ScreenING in an urban low-barrier clinic (PRESSING) study: Protocol for a prospective single arm feasibility trial.

INTRODUCTION: Syphilis has resurged across North America, including syphilis in pregnancy. Congenital syphilis can be prevented by timely treatment of syphilis in pregnancy, but conventional syphilis screening is problematic for people facing barriers to care. Syphilis point-of-care tests (POCT) have improved prenatal syphilis management in low- and middle-income countries. There is minimal literature regarding POCT feasibility and effectiveness for prenatal syphilis screening in high-income countries, where barriers and challenges may differ. METHODS: Academic and community partners co-desig

Identification and validation of stable reference genes for RT-qPCR analysis in blood samples from golden Syrian hamsters under different immunological conditions.

BACKGROUND: The golden Syrian hamster is the main experimental model for leptospirosis. However, there are no validated reference genes for reverse transcription quantitative PCR (RT-qPCR) analysis of blood samples. METHODS AND RESULTS: The expression stability of 11 candidate reference genes was evaluated in hamster blood under distinct immunological conditions, including Leptospira interrogans infection and different immunization strategies. Stability was assessed using BestKeeper, geNorm, NormFinder, and the comparative ΔCt method, with the consensus ranking calculated using the RefFinder

[Neurosyphilis in dementia cases].

This case report describes a middle-aged male who presented with symptoms of dementia. During a routine screening in a lumbar puncture, a total of 209 × 109 leukocytes was found in his cerebrospinal fluid, 208 of which were mononuclear. The patient was admitted to an infectious disease ward, and serum analyses showed a positive Wassermann reaction and a rapid plasma reagin of 128. The man was diagnosed with neurosyphilis and started on penicillin treatment. This lasted 14 days, and the symptoms of dementia decreased substantially.

IL36γ signaling promotes mammalian anti-tick immunity.

Ticks, like Ixodes scapularis, can parasitize a diverse array of vertebrates, favoring natural hosts but feeding opportunistically on others. Natural hosts, as well as permissive hosts such as laboratory mice, tolerate repeated infestations, while many incidental hosts may acquire anti-tick immunity, as first described in guinea pigs. We show that tick-immune guinea pigs display altered dermal cytology and a distinct transcriptome, predominantly upregulating the IL36γ cytokine. Interference with IL36γ signaling, via gene knockdown, chemical inhibition, or its natural antagonist IL36RN, reverse

Borrelia garinii spirochetemia in a previously healthy child presenting with Streptococcus anginosus group brain abscess: a case report.

BACKGROUND: The clinical significance of transient Borrelia burgdorferi sensu lato complex (hereafter referred to as B. burgdorferi) spirochetemia remains poorly understood and has rarely been documented. We report the isolation of viable B. garinii genospecies from the blood of a previously healthy child presenting with a Streptococcus anginosus group (SAG) brain abscess. CASE PRESENTATION: An eight-year-old girl presented with four days of headache, fever, nausea, slightly reduced general condition, and mild facial asymmetry. The Safety of Melasma Treatment in Vitiligo Patients: An International E-Delphi Study.

Vitiligo and melasma frequently coexist, creating a treatment dilemma, since agents effective for one condition can destabilize the other, and no consensus currently exists on safe management. We conducted a scoping literature review (through June 2023) on treatment of patients with concomitant vitiligo and melasma, followed by a two-round international e-Delphi survey (22 experts from 14 countries; consensus defined as ≥ 70% agreement) and a final online consensus meeting. Only one study supported the safe use of oral tranexamic acid for melasma in vitiligo patients. Expert consensus favored

Beyond Fever-The Changing Face of Acute Leptospirosis: A Retrospective Observational Study from Western India.

BACKGROUND: Leptospirosis is a zoonotic infection caused by pathogenic Leptospira species and remains a significant cause of acute febrile illness in tropical regions. Its clinical spectrum ranges from mild, self-limiting illness to severe multi-organ dysfunction. The disease is broadly classified into anicteric and icteric (Weil's disease) forms, the latter associated with jaundice and increased morbidity. In India, diagnosis is often challenging due to overlapping features with other endemic infections such as dengue and scrub typhus. Coinfections and changing epidemiological patterns, inclu

Syphilis Screening in Pregnancy During Emergency Department Gonorrhea and Chlamydia Testing.

INTRODUCTION: Congenital syphilis is rising in the United States, especially among infants born to women with limited prenatal care. Emergency departments (ED) offer opportunities for early detection, particularly among patients evaluated for urogenital sexually transmitted infection (STI) concerns. In October 2019, our health system implemented an ED STI order set that promoted HIV co-testing and included a preselected option for syphilis testing. In this study, we evaluated change in syphilis testing during the post-implementation period among ED encounters with both pregnancy and gonorrhea/

Socioeconomic transitions and the spatial dynamics of syphilis transmission in urbanizing China.

BACKGROUND: Syphilis remains a re-emerging sexually transmitted infection in China, with transmission increasingly shaped by regional socioeconomic disparities. Understanding spatial heterogeneity and inter-provincial inequality is essential for equitable disease control. METHODS: A balanced panel dataset of syphilis incidence was constructed across 31 provinces in mainland China from 2004 to 2020. Temporal trends were stratified by age and disease stage using joinpoint regression analysis. Spatial clustering patterns of syphilis incidence were evaluated through Global Moran's I and Local Mor

Immune mechanisms underlying endothelial and placental dysfunction in leptospirosis during pregnancy.

Leptospirosis is a zoonotic disease caused by pathogenic bacteria of the genus Leptospira and represents a major public health concern worldwide. Although its systemic manifestations are well documented, the consequences of leptospirosis for reproduction and pregnancy, particularly in humans, remain poorly understood. This narrative review summarizes the principal reproductive outcomes associated with leptospirosis and examines the mechanisms that may contribute to pregnancy complications. In livestock, leptospirosis is a well-established cause of reproductive failure, including abortion and i

Not always cancer-Probable Lyme disease mimicking squamous cell carcinoma: A case report.

Lyme disease, caused by Borrelia burgdorferi, can produce head and neck manifestations in up to 75% of cases, most commonly facial palsy and lymphadenopathy. Borrelial lymphoid proliferations are recognized mimics of malignancy; however, tonsillar involvement simulating a malignant neoplasm has not been previously described. We report a 57-year-old male patient who presented with a right cervical lump associated with odynophagia, weight loss, and low-grade fever. PET-CT demonstrated intense FDG uptake in a right palatine tonsillar mass and right cervical lymph nodes, with loss of fat plane def
