Efficacy and Safety of Balloon Dilatation of the Eustachian Tube in Children With Eustachian Tube Dysfunction: A Systematic Review and Meta-Analysis.

The meta-analysis and systematic review evaluated the efficacy and safety of balloon dilatation of the eustachian tube (BDET) in children with eustachian tube dysfunction. The PubMed, Cochrane Library, and ScienceDirect databases were searched in depth to identify relevant studies, and eight retrospective cohort studies were selected. A total of 652 patients were included in these studies. There were no serious complications, and only minor, self-limiting adverse events were seen, the most common of which were epistaxis and a patulous eustachian tube. The primary result was an improvement in t

Post-traumatic Gradenigo syndrome: an uncommon complication in the antibiotic era?

Gradenigo syndrome (GS) is a rare complication of otitis media, secondary to infection spreading to the petrous apex, with potential involvement of cranial nerves V and VI. We present the unusual case of a previously healthy 5-year-old boy who developed GS after a penetrating trauma to the left ear caused by a schoolmate. The patient presented severe ear pain, purulent otorrhea, and horizontal diplopia with VI cranial nerve palsy and hemifacial hypoesthesia. CT showed petrous bone erosion, and MRI showed meningeal enhancement without abscesses. Antibiotic therapy and transtympanic ventilation

Internal carotid artery arteritis as a rare complication of pediatric otomastoiditis: serial magnetic resonance imaging and magnetic resonance angiography findings.

Acute otomastoiditis is a common pediatric infection that may rarely lead to severe intracranial complications. The frequency of otomastoiditis, and even more so the frequency of intracranial complications, significantly increased after the end of the coronavirus disease 19 pandemic. We report a 14-month-old boy with bilateral otomastoiditis complicated by extensive venous sinus thrombosis, petrous apicitis, and progressive infectious arteritis of the cavernous internal carotid artery. Serial magnetic resonance imaging and magnetic resonance angiography demonstrated interval internal carotid a

Effects of Body Weight on Middle Ear Disease.

Body weight can have various effects on middle ear health. Pediatric obesity is associated with an increased risk and recurrence of otitis media with effusion. Adult obesity is associated with an increased risk of temporal bone cerebrospinal fluid leak and encephalocele, and weight loss can cause symptoms of patulous Eustachian tube dysfunction. With rising rates of obesity and widespread adoption of glucagon-like peptide 1 receptor agonists, it is increasingly important that patients are appropriately counseled on how weight changes affect the middle ear. In this review, we examine the litera

[Gradenigo's syndrome as a complication of acute otitis media in adolescent].

Gradenigo's Syndrome is a rare but potentially serious complication of an otitis media. It associates an otitis media, along with retro-orbital pain and abducens nerve palsy. We report the case of a thirtheen-year-old female adolescent, with no particular medical history, who presented with this syndrome in the context of acute otitis media evolving over 8 days. The infection also caused a facial palsy and a mixed hearing loss. Early medical and surgical management led to a favourable outcome. This case highlights the need to keep in mind the potential neurological complications of acute otiti

Unique Anatomical Imaging Findings in Patients With Patulous Eustachian Tube.

OBJECTIVES: The pathophysiology of patulous Eustachian tube (PET) remains poorly understood. This study characterizes nasopharyngeal anatomy to identify features that distinguish PET patients from controls and determine whether anatomical measurements predict symptom laterality. METHODS: Head CT scans from patients with PET diagnosed by Japanese Otologic Society criteria were compared to scans from demographically matched controls. Anatomical features of the Eustachian tube (ET), turbinates, and nasopharynx were measured. PET patients were stratified into bilateral PET, unilateral affected si

A Novel Decellularized Fibrocartilage Graft Promotes Tympanic Membrane Repair.

Tympanoplasty repairs tympanic membrane (TM) perforations using grafts. In pediatric patients, cartilage grafts are preferred over fascia due to superior mechanical properties that prevent complications from negative middle ear pressure, including graft failure, retraction, and cholesteatoma formation. However, autologous grafts cause donor-site morbidity and increased surgical time. To overcome these limitations, we engineered an allogeneic porcine meniscus decellularized (MEND) fibrocartilage graft with a unique microchannel structure created by selective elastin and vascular digestion to pr

StyleGAN-based synthetic image augmentation for multi-class otoscopy image classification.

Accurate diagnosis of eardrum abnormalities is pivotal for effectively managing various ear conditions. Otoscopy, a non-invasive diagnostic procedure, provides detailed views of the ear canal and eardrum to identify pathologies such as otitis media, tympanic membrane perforations, and other abnormalities. Despite advancements, classifying otoscopy images remains challenging due to lighting variations, motion blur, and diverse abnormalities, exacerbated by limited labeled datasets and subjective assessments. A significant barrier to accurate diagnosis is the lack of an objective evaluation meth
