Acute otitis media complicated by peripheral facial nerve palsy in an adult: A case report.

Acute otitis media (AOM) is a common infection, whereas peripheral facial nerve palsy is an uncommon extracranial complication in the post-antibiotic era. We report the case of a 53-year-old immunocompetent man who developed acute suppurative otitis media complicated by ipsilateral peripheral facial nerve palsy following an upper respiratory tract infection. The illness began with sore throat and cough during Hajj and progressed to severe right-sided otalgia, followed by complete right lower motor neuron facial weakness despite sequential empirical oral antibiotics. On return to Pakistan, otos

Are Keynote and Invited Speakers at State Behavior Analytic Conferences Experts on Their Presentation Topics?

UNLABELLED: Prostate cancer poses a significant threat to men's health worldwide. Docetaxel (DTX) remains a cornerstone chemotherapeutic agent for PC, yet the emergence of docetaxel resistance has critically compromised clinical outcomes. Elucidating the molecular mechanisms underlying docetaxel resistance and identifying novel therapeutic targets are therefore imperative. This study integrated transcriptomic profiles and clinical data from the The Cancer Genome Atlas Prostate Adenocarcinoma (TCGA-PRAD) and Gene Expression Omnibus (GEO) databases to identify PCDHGB2 as a pivotal gene associate

Olfactory training using nasal inserts is more effective due to increased adherence.

BACKGROUND: The recommended treatment for hyposmia (a clinically reduced sense of smell) is olfactory training using odor containers that the patients smell twice a day for several weeks. Adherence to the olfactory training regimen is, however, generally low. We aimed to investigate if a new form of odor delivery, using scented nasal inserts, could enhance adherence to olfactory training by allowing participants to be mobile during the training and thereby lower the perceived intrusion on everyday life. METHODS: Using a randomized controlled parallel-group design, individuals (N = 116) with h

Characteristics and antimicrobial susceptibility of bacteria causing acute otitis media in children at Vietnam National Children's Hospital: a cross-sectional study.

BACKGROUND: Acute otitis media (AOM) is a leading cause of healthcare visits, antibiotic prescription and complications in children aged under 5 years. Following the introduction of pneumococcal conjugate vaccines (PCVs), non-typeable Haemophilus influenzae (NTHi) has become the most common causative agent for AOM, followed by Streptococcus pneumoniae and Moraxella catarrhalis. PCVs are not yet included in the National Immunization Program in Vietnam. OBJECTIVES: To determine the frequency and characteristics of the pathogens related to AOM in Vietnam in children ≤5 years old. METHODS: This

Preventing Recurrent Otitis Media in Children Aged 2-7 Years: A Cross-Sectional Evaluation of Serum Vitamin D as a Modifiable Factor.

Background/Objectives: Otitis media (OM) is a common pediatric condition that significantly impacts hearing, language development, and quality of life. Emerging evidence suggests that vitamin D plays a crucial role in immune regulation and that deficiency may predispose children to recurrent OM. This study aimed to evaluate whether low serum vitamin D levels are associated with increased incidence and severity of OM in children aged 2-7 years. Methods: A cross-sectional study was conducted at a pediatric otolaryngology clinic in northwest Romania between November 2023 and April 2024. A total o

Does Topical Steroid Use Increase the Risk for Subsequent Tympanoplasty?

OBJECTIVES: Although the current data support the notion that combination therapy of topical steroids with antibiotics is a safe, and often superior, option for suppurative otitis media when compared to topical antibiotics alone, little is known about the impact of topical steroids on tympanic membrane (TM) healing and risk of residual perforation. This study aims to describe the long-term impact the addition of topical steroids to antibiotics has on TM healing, as measured by rates of subsequent tympanoplasty. METHODS: Electronic health records were queried using TriNetX Research Network to

Exploring Middle Ear Pathologies in Adults with Diabetes Mellitus: A Scoping Review of Available Evidence and Research Gaps.

Diabetes mellitus (DM) is a common chronic health condition, affecting millions of people worldwide, and its incidence is projected to increase by almost 50% in the next two decades. The effects of DM on the auditory system have been reported. However, there is limited evidence on the association between DM and middle ear pathologies. This scoping review aimed to map the available evidence and identify research gaps regarding DM and middle ear pathologies in the adult population. Five electronic databases, namely Scopus, CINAHL, MEDLINE, PubMed, and Web of Science, were searched using a combin

The causal relationship between obstructive sleep apnea and otitis media: a bidirectional Mendelian randomization study.

Obstructive sleep apnea (OSA) is manifested as periodic collapse of the upper airway during sleep. Otitis media is a spectrum of infectious and inflammatory diseases involving the middle ear. In this study, we sought to determine the causal effect of OSA on otitis media using a bidirectional, two-sample Mendelian randomization (MR) analysis. We analyzed the data from two different, extensive genome-wide association studies (GWAS) and selected OSA-related single-nucleotide polymorphisms (SNPs) as instrumental variables (IVs). Bidirectional MR analysis was conducted using the inverse-variance we

Causal association between type 2 diabetes mellitus and acute suppurative otitis media: insights from a univariate and multivariate Mendelian randomization study.

BACKGROUND: Type 2 diabetes mellitus (T2DM) and hearing loss (HL) constitute significant public health challenges worldwide. Recently, the association between T2DM and HL has aroused attention. However, possible residual confounding factors and other biases inherent to observational study designs make this association undetermined. In this study, we performed univariate and multivariable Mendelian Randomization (MR) analysis to elucidate the causal association between T2DM and common hearing disorders that lead to HL. METHODS: Our study employed univariate and multivariable MR analyses, with

Physiotherapy in Bell's Palsy Secondary to Acute Otitis Media: A Case Report.

Acute suppurative otitis media can occasionally result in facial paralysis, which calls for prompt diagnosis and treatment. Facial paralysis, a synonym for Bell's palsy, is a condition that causes rapid weakening of one side of the face muscles, leading to drooping of the face on that side. A major factor in determining the course of the condition is rehabilitation through physiotherapy. Here, we present a case of a 26-year-old female who felt discomfort in her left ear on February 21, 2024, but chose to ignore it then. She observed an abrupt deviation in her mouth and visited a rural hospital

Facial Paralysis in Otitis Media Due to Granulomatosis with Polyangiitis.

Granulomatosis with polyangiitis (GPA), formerly Wegener's granulomatosis, commonly presents primarily with otitis media and hearing loss, as well as upper and lower respiratory symptoms. However, facial nerve paralysis is a rare manifestation of this uncommon necrotizing vasculitis. We report a patient with facial paralysis accompanied by otitis media. In further studies, our patient was diagnosed with GPA, which was neglected before. In such a presentation, acute suppurative otitis media is the most likely cause of the facial paralysis, but GPA must also be considered, especially in cases wi

Causal relationship between allergic rhinitis and otitis media: A Mendelian randomization study.

Otitis media and allergic rhinitis are prevalent conditions. Some data posit a significant association between the 2 ailments, proposing mechanisms such as allergic rhinitis-induced Eustachian tube dysfunction or concurrent allergic pathophysiology affecting both the nasal and aural cavities. Observational studies hint at an association between allergic rhinitis (AR) and nonsuppurative otitis media, yet definitive causality remains elusive. Thus, to elucidate the causal impact of AR on otitis media, a 2-sample Mendelian randomization study was undertaken. Data on AR, acute suppurative otitis m

Otitis Media Progressing to Community-Acquired Meningitis in Diabetic Patients: A Case Report of K2-ST375 hypervirulent Klebsiella pneumoniae and Literature Review.

Community-acquired Klebsiella pneumoniae meningitis (CA-KPM) can rapidly progress to invasive infection in healthy individuals. We present the case of a 54-year-old man with a history of acute suppurative otitis media and uncontrolled type 2 diabetes mellitus (T2DM), who had been treated with oral antibiotics intermittently and irregularly for one month. His symptoms did not improve and continued to worsen, leading to fever and coma. Metagenomic next-generation sequencing (mNGS) of cerebrospinal fluid (CSF) identified Klebsiella pneumoniae (KP) after 24 hours in the intensive care unit (ICU).

Granulomatosis With Polyangiitis (GPA) Mimicking Metastatic Malignancy.

Granulomatosis with polyangiitis is a rare vasculitis characterized by necrotizing granulomatous inflammation affecting small blood vessels. It often presents with pulmonary masses and nodules, mimicking malignancy in imaging studies. Distinguishing granulomatosis with polyangiitis from cancer is crucial due to differing treatment approaches and prognoses. We describe a 54-year-old male with granulomatosis with polyangiitis presenting with headache, bilateral ear discharge, cough, and weight loss. Imaging revealed metabolically active lesions in the lungs and nasopharynx, initially suggestive

Characterization, prognostic factors, and clinical profile of ear infections by Turicella otitidis: Revealing the emerging rise of a controversial pathogen.

INTRODUCTION: Environmental changes and modifications in leisure habits have facilitated the emergence of new bacteria responsible for causing ear infections with different presentations. In this context, Turicella otitidis is a pathogen for which isolated cases of external and middle ear infections have been reported. However, our experience indicates a resurgence in its occurrence in recent years. The main objective of this study is to examine the possible connection between T. otitidis and the most frequent clinical ear manifestations, as well as the associated demographic characteristics.
