Squamous cell carcinoma in vitiligo lesion after long-term PUVA therapy.

This article reviews reports of ACE inhibitor use in pediatric heart failure and summarizes the present implications for clinical practice. Captopril, enalapril, and cilazapril are orally active ACE inhibitors, and widely used in pediatric cardiology, although more than ten other ACE inhibitors have been applied clinically in adults. Effects of ACE inhibitors on the renin-angiotensin-aldosterone system in pediatric patients are similar to those in adults. ACE inhibitors lower aortic pressure and systemic vascular resistance, do not affect pulmonary vascular resistance significantly, and lower

Epidemiology of chronic suppurative otitis media among Saudi children--a comparative study of two decades.

An epidemiological survey to study the prevalence of chronic suppurative otitis media (CSOM) was completed in February 2000. About 9540 children aged up to 12 years were examined clinically by an otolaryngologist. All 125 children (1.3%) suffered from CSOM, nine with cholesteatoma. The prevalence of CSOM in the Central province of the Kingdom was found to be 1.15%. During the first study done in 1982, the prevalence was 5.5% among 293 children aged 6-18 years. In the second study done and completed in 1991 where 6421 children were surveyed, 94 children found to suffer from CSOM i.e. 1.5% with

Treatment of chronic suppurative otitis media with ofloxacin in hydroxypropyl methylcellulose ear drops: a clinical/bacteriological study in a rural area of Malawi.

Chronic suppurative otitis media in young children is a major problem in Africa, with socio-economic consequences at a later age. Common treatment regimens with antibiotics are expensive and often not practically feasible. Therefore, a project was started to develop a low-cost and effective treatment in a rural area of Malawi by studying the clinical efficacy of an inexpensive application regimen of ofloxacin (0.075%) in hydroxypropyl methylcellulose (1.5%) ear drops. In earlier studies with this treatment regimen, it was possible to cure approximately 70% of ears. The aim of this study was to

The methodical collection of ear surgery data as a basis for quality control.

Data relating to daily clinical practice were collected in an otologic database. Over a period of 3 years, information was gathered about 1,000 ear operations. This led to the following conclusions: the collection of data is difficult; the selection of data and the moment it should be fed into the systems are very important; there is a risk of using too many items and therefore reducing surgeon compliance. On the other hand, too few items result in irrelevant overviews. The collection of ear surgery data makes it easier to understand positive and negative outcomes.

Tuberculous otitis media: two case reports and literature review.

Tuberculous otitis media can be difficult to diagnose because it can easily be confused with other acute or chronic middle ear conditions. Compounding this problem is the fact that physicians are generally unfamiliar with the typical features of tuberculous otitis media. Finally, the final diagnosis can be difficult because it requires special culture and pathologic studies. To increase awareness of this condition, we describe two cases of tuberculous otitis media and we review the literature.

[Brain abscesses of otogenic origin].

Chronic inflammation of the middle ear is the most frequent cause of otogenic complications. Meningitis is the most frequent otogenic intracranial complication, followed by otogenic brain abscesses, while other complications are significantly less frequent. The study is aimed at presenting clinical causitry of otogenic brain abscesses consequential to chronic suppurative otitis in order to evaluate modern diagnostic and therapeutic possibilities. The study was retrospective and included the patients treated at the Institute of Otorhinolaryngology and Maxillofacial Surgery of the Clinical Centr

Upregulation of epidermal surface molecule expression in primary and ultraviolet-induced lesions of lupus erythematosus tumidus.

BACKGROUND: Lupus erythematosus tumidus (LET), a photosensitive skin disorder with characteristic clinical and histological features, has not been generally accepted as a subset of cutaneous lupus erythematosus (CLE). OBJECTIVES: To analyse the expression of epidermal surface molecules in skin biopsy specimens from patients with LET and to relate the results to other variants of CLE, such as discoid lupus erythematosus (DLE) and subacute CLE (SCLE). METHODS: In total, 45 patients with different subtypes of CLE were included in the study, and cryostat sections from primary and ultraviolet (UV

Chronic suppurative otitis media.

Restoring precise muscular control in the poststroke wrist/hand (W/H) demands sensorimotor integration to correct compensatory neuroplasticity. However, current rehabilitation robots inadequately modulate ascending somatosensory pathways from specific muscles. This study developed an electromyography (EMG)-driven soft robot with electro-vibro-feedback (EVF-robot) for targeted somatosensory priming in W/H muscles. This system integrates (a) focal vibratory stimulation and neuromuscular electrical stimulation for recruiting the somatosensory pathways of the targeted W/H flexors and extensors; (b

Matrix metalloproteinase-8 (MMP-8) in pulpal and periapical inflammation and periapical root-canal exudates.

Weight gain induced by antipsychotics is the second most frequently given reason for noncompliance with pharmacologic therapy; excessive sedative effects rank first, with extrapyramidal side effects ranking third. Frequently, weight gain leads to inconsistent pharmacologic treatment; this exposes patients to the risk of recurrent symptoms. In fact, one of the key contributors to good clinical outcomes in schizophrenic patients is compliance with pharmacologic treatment. The goals of this study were to evaluate weight gain in a group of patients treated with olanzapine, diet modifications, and

Efficacy of ofloxacin and other ototopical preparations for chronic suppurative otitis media in children.

The safety and efficacy of ototopical preparations, such as ofloxacin, in the treatment of CSOM in infants and children do not currently have approval by the FDA even though ofloxacin is FDA-approved for treatment of CSOM that occurs in adults. However, treatment with ofloxacin is FDA-approved when AOM occurs in children who have a tympanostomy tube in place, and effective treatment of this acute infection should prevent the subsequent development of CSOM.

Efficacy of ofloxacin and other otic preparations for acute otitis media in patients with tympanostomy tubes.

Otorrhea occurs in 21 to 50% of all children with tympanostomy tubes in the United States. More than 1 million children annually undergo tubomyringotomy, constituting placement of more than 2 million tympanostomy tubes each year. The organisms typically responsible for otorrhea are the same as those that cause otitis media in very young children, including Streptococcus pneumonia, Haemophilus influenzae and Moraxella catarrhalis. Drainage from tympanostomy tubes in older children involves organisms that colonize the external auditory canal, the most common being Pseudomonas aeruginosa and Stap

Bacterial agents of discharging ears and antimicrobial sensitivity patterns in children in Ibadan, Nigeria.

Bacteriological studies were carried out on ear swabs from 361 cases of children with discharging ears at the Microbiology Department of the University College Hospital, Ibadan between March 1995 and February 1997. 308 (85.3%) had positive cultures. 78.6% of these yielded one isolate, 19.2% yielded two isolates while 2.2% had three isolates. Pseudomonas spp. Was the predominant agent of CSOM and ASOM. This was followed by Staphylococcus aureus. Other commonly isolated organisms were Proteus and Klebsiella species. Ofloxacin and ciprofloxacin showed the highest activity to all isolates, while t

In vitro and in vivo antimicrobial activity of topical ofloxacin and other ototopical agents.

BACKGROUND: Ototopical agents are extensively used for otitis externa (OE), acute otitis media identified by otorrhea in patients who have tympanostomy tubes (AOM-TT) and chronic suppurative otitis media (CSOM). The quinolones have particular value as ototopical agents because of the broad spectrum of antibacterial activity of importance in otic diseases and the high concentrations of antibacterial activity at the site of infection. METHODS: A survey of literature on in vitro activity and microbiologic efficacy in clinical trials of quinolone otic products for OE, AOM-TT and CSOM. RESULTS: O

Salivary gland choristoma of the middle ear: a case report.

A choristoma is a nonneoplastic proliferation of histologically normal tissue that forms at an abnormal site. It is extremely uncommon in the middle ear space. It appears to be a developmental abnormality and may be associated with abnormalities of adjacent structures. It usually occurs with unilateral conductive hearing loss and requires a differential diagnosis from other mass lesions in the middle ear cavity. This article discusses a case of salivary gland choristoma of the middle ear that we believe to be the 24th case reported on this subject.

Ceftazidime for outpatient parenteral antibiotic therapy (OPAT) of chronic suppurative otitis media due to Pseudomonas aeruginosa.

The number of patients receiving parenteral antibiotic therapy outside the hospital (OPAT) is growing rapidly, not only because of financial considerations, but also to limit the risk of nosocomial infections, hospitalization trauma, and to improve the quality of life. The same benefits of OPAT have recently been extended to pediatric patients. In the present study, the efficacy and safety of OPAT was evaluated in 52 children (age range 6-12 years) affected by chronic suppurative otitis media (CSOM) whose parents were deemed compliant.
