Systems perspectives on pediatric otitis media: environmental exposures, genetic susceptibility, and biomarker-guided interventions.

BACKGROUND: Pediatric otitis media remains one of the most common childhood diseases worldwide and is a major cause of hearing impairment, recurrent healthcare utilization, and antibiotic exposure. Its pathogenesis is highly heterogeneous and can no longer be adequately explained by a traditional single-pathogen model. OBJECTIVE: This review aims to synthesize current evidence on the roles of environmental exposures, host genetic susceptibility, biomarkers, and microbial factors in pediatric otitis media, with particular emphasis on their interactions and translational relevance. Bacteriological spectrum, antimicrobial susceptibility, and year-to-year variation among referred culture-positive presumed chronic suppurative otitis media specimens in a Syrian reference laboratory (2022-2025).

BACKGROUND: Chronic suppurative otitis media (CSOM) remains an important cause of chronic otorrhea and preventable hearing impairment, particularly in resource-limited settings. Because bacterial profiles and antimicrobial susceptibility patterns vary across settings, local laboratory surveillance may support culture-guided management, especially in persistent or previously treated cases. METHODS: We conducted a retrospective laboratory-based study at a private reference laboratory in Damascus, Syria. The analyzed cohort primarily represents referred laboratory specimens rather than a populat

Eustachian Tube Interventions in the Context of Chronic Otitis Media.

Chronic obstructive Eustachian tube dysfunction results in persistent negative middle ear pressure, effusion retention, and tympanic membrane retraction. This results in chronic otitis media, which encompasses a spectrum of disease from otitis media with effusion and atelectasis to chronic suppurative otitis media and cholesteatoma. Eustachian tube (ET)-directed interventions add therapeutic value beyond traditional tympanoplasty, which emphasizes disease eradication and hearing mechanism reconstruction or tympanostomy tubes, which act by bypassing the ET. These techniques fail to address the

Usefulness of 99mTc-HMPAO leukocyte scintigraphy in necrotizing external otitis/skull base osteomyelitis.

Necrotizing external otitis (NEO) is a rare but serious disease of the skull base (osteomyelitis). Immunosuppressed, and elderly patients are most commonly affected. The most common pathogen is Pseudomonas aeruginosa. Early diagnosis is crucial for the prognosis. To establish the diagnosis, both positive imaging findings, and clinical criteria are required to be met. Imaging is used during follow-up to monitor a potential recurrence due to its high risk, even after successful treatment. The use of radiological or nuclear medicine methods (scintigraphy) in the management of NEO is not currently
