Otoscleroma of the Middle Ear and Mastoid Cavity with Facial Palsy: A Case Report.

INTRODUCTION: Scleroma is a chronic, specific granulomatous disease that affects the head and neck mucosa. Its common sites are the nose and larynx; however, it might affect other areas. One of the rare sites to be affected is the middle ear and mastoid cavity, for which the term otoscleroma was coined. We present such a rare case in this report. CASE REPORT: A 47-year-old patient with a history of both old laryngoscleroma and recent Rhinoscleroma. He presented with symptoms of ear discharge and facial palsy. Examination revealed complete facial nerve palsy and an external auditory canal poly

Rare Presentation of Middle Ear Neuroendocrine Tumor: A Case Report.

BACKGROUND Neuroendocrine tumors (NETs) of the middle ear are exceptionally rare, representing less than 2% of primary ear tumors in adults. These neoplasms originate from neuroendocrine cells within the middle ear cavity, posing significant diagnostic and therapeutic challenges. CASE REPORT We present a 27-year-old man with a 2-month history of left-sided hearing loss, tinnitus, and ear fullness. Comprehensive assessments, including tuning fork tests, pure tone audiometry, computed tomography (CT) scan of the temporal bone, and magnetic resonance imaging (MRI), were conducted. Differential di

Chronic suppurative otitis media: disrupted host-microbial interactions and immune dysregulation.

Recent research has uncovered new mechanisms that disrupt the balance between the host and microbes in the middle ear, potentially leading to dysbiosis and chronic suppurative otitis media (CSOM). Dysbiotic microbial communities, including core pathogens such as persister cells, are recognized for displaying cooperative virulence. These microbial communities not only evade the host's immune defenses but also promote inflammation that leads to tissue damage. This leads to uncontrolled disorder and pathogen proliferation, potentially causing hearing loss and systemic complications. In this discu

Chronic otitis media.

Chronic otitis media, defined as persistent or recurrent inflammation of the middle ear mucosa, is a frequent entity in primary and secondary care. This disease causes otorrhea and hearing loss that significantly affect the quality of life of patients. In recent years, there have been new developments in its diagnosis, prevention and treatment. In this last aspect, pharmacological treatment, surgical techniques and hearing rehabilitation through implants have shown significant advances. On the other hand, chronic otitis media complications continue to occur in emergency services and have to be

Gelatin versus chitosan as packing materials in endoscopic myringoplasty for chronic suppurative otitis media.

PURPOSE: There is a lack of literature studying the impact of various middle ear packing materials on the outcomes of myringoplasty. Absorbable gelatin sponge is commonly used, though it has been reported to induce middle ear fibrosis or adhesions. Chitosan-based sponge has useful properties including hemostasis, anti-adhesion and antimicrobial activity, potentially making it useful for supporting the graft in myringoplasty. This study compares graft success when chitosan and gelatin are used as packing materials for endoscopic myringoplasty. METHODS: An audit was performed of all patients wh

Correlation of bacterial biofilm profile based on optical density cut-off with clinical severity in patients with chronic suppurative otitis media tubotympanic type.

INTRODUCTION: Chronic suppurative otitis media (CSOM) is a middle ear infection with a high incidence in ear cases, is often recurrent, and causes hearing impairment. Bacteria in the CSOM frequently form biofilms, which enhance antibiotic resistance and contribute to disease progression. The aim of this study was to determine the correlation of bacterial biofilm profiles based on optical density cut-off with the clinical picture of patients with tubotympanic type CSOM. MATERIALS AND METHODS: This was a cross-sectional study using a descriptive analytical design. The study was conducted at the

Modern aspects of diagnosing purulent pathology of the ear (Review of cases of middle ear cholesteatoma using non-EPI DWI MRI).

Chronic suppurative otitis media (CSOM) is a prevalent condition in otolaryngology with significant medical and social implications, including hearing loss and severe intracranial complications. This article discusses the challenges in diagnosing cholesteatoma, a common complication of CSOM, particularly when using computed tomography (CT) and magnetic resonance imaging (MRI). We present three clinical cases where MRI, particularly in the non-EPI diffusion-weighted imaging (DWI) and apparent diffusion coefficient (ADC) modes, effectively identified the presence and extent of cholesteatoma that

Antimicrobial Resistance Trends Among Obligate Anaerobic Bacteria Isolated From Chronic Suppurative Otitis Media Patients in a Tertiary Care Teaching Hospital in Central India.

INTRODUCTION: Chronic suppurative otitis media (CSOM) remains a major public health concern, especially in low- and middle-income countries, contributing significantly to preventable hearing loss. While aerobic bacteria are well-recognized pathogens in CSOM, anaerobic bacteria's role is increasingly acknowledged due to improved sampling and culture techniques. Anaerobes may synergize with aerobes, worsening disease severity and complicating management. This study aimed to identify the anaerobic bacterial profile in CSOM patients and assess antimicrobial susceptibility patterns of these isolate

Deep learning for otitis media classification using otoscopic image.

Otitis media (OM) is a globally prevalent health issue that poses significant threats to hearing and overall health. However, traditional diagnostic methods rely heavily on subjective interpretation of otoscopic images, which often leads to inconsistent outcomes and high misdiagnosis rates. This study evaluated 5 deep learning models (ResNet-18, GoogLeNet, AlexNet, MobileNet-V3, and VGGNet-19) for classifying 819 otoscopic images into 4 categories: normal, acute otitis media, otitis media with effusion, and chronic suppurative otitis media. Specifically, the dataset was split into training (60

Repurposing Povidone-Iodine for the Treatment of Chronic Suppurative Otitis Media in Nigerian Children.

BACKGROUND: Chronic suppurative otitis media (CSOM) is inflammation of the lining of the middle ear cleft. It is a leading cause of pediatric hearing loss. Antibiotic eardrops are CSOM's mainstay of treatment, but with emerging resistance and increasing treatment failure, the need to explore other treatment options has become pertinent. STUDY DESIGN: This was an 18-month randomized clinical trial of children aged 6 months to 14 years with uncomplicated CSOM, randomized into 3 intervention groups: (a) 0.5% povidone-iodine aural irrigation and oral amoxicillin-clavulanic acid, (b) 0.3% ciproflo

Antibiotics versus topical antiseptics for chronic suppurative otitis media.

BACKGROUND: Chronic suppurative otitis media (CSOM), sometimes referred to as chronic otitis media, is a chronic inflammation and often polymicrobial infection of the middle ear and mastoid cavity. It is characterised by ear discharge (otorrhoea) through a perforated tympanic membrane. The predominant symptoms of CSOM are ear discharge and hearing loss. Antibiotics and antiseptics kill or inhibit the micro-organisms that may be responsible for the infection. Antibiotics can be applied topically or administered systemically via the oral or injection route. Antiseptics are always directly applie
