Cost-effectiveness of the hepatitis B birth-dose vaccine in South Africa: a mathematical modelling analysis.

BACKGROUND: In 2022, South Africa had an estimated hepatitis B prevalence of 4·7-6·0%. In an effort to eliminate viral hepatitis as a public health threat, a targeted (ie, selective) hepatitis B birth-dose vaccination (HepB-BD) policy was introduced in 2023. Under this policy, only infants born to mothers with confirmed hepatitis B positivity are eligible for HepB-BD, which is given in addition to peripartum antiviral prophylaxis (PAP) for mothers from the second or third trimester of pregnancy. This policy differs from the WHO recommendation that all newborns should receive a HepB-BD vaccine

Substantial improvement in short-term survivorship in patients receiving robotic arm-assisted medial unicompartmental knee arthroplasty : a retrospective cohort study.

AIMS: Robotic arm-assisted unicompartmental knee arthroplasty (R-UKA) has been shown to improve alignment and accuracy of component placement compared with conventional unicompartmental knee arthroplasty (C-UKA). Despite this, there is conflicting evidence for whether this has an impact on revision rates. METHODS: We performed a retrospective analysis of a prospective database, comparing patients who underwent R-UKA using the Stryker Restoris MCK implant with those who underwent C-UKA using the cemented Oxford Partial Knee implant, between January 2002 and December 2024 at The Prince Charles

Management of suspected infectious and autoimmune encephalitis in adults - National Guidelines from the United Kingdom Joint Societies.

Encephalitis, characterised by inflammation of the brain parenchyma, is a neurological emergency with diverse infectious, autoimmune and inflammatory causes. Delays in recognition and treatment carry significant risks of morbidity and mortality. Since the first UK guideline for management of suspected viral encephalitis in adults was published in 2012, there have been substantial developments in this field. Advances in molecular diagnostics, antibody testing, alongside improved access to neuroimaging and electrophysiology have broadened the capacity to identify underlying aetiologies. The reco

Isolated granulomatous pulmonary artery vasculitis associated with Hughes-Stovin syndrome: a case report.

BACKGROUND: Hughes-Stovin syndrome (HSS) is an exceptionally rare vasculitis, characterized by pulmonary artery aneurysms with in situ thrombosis and is increasingly regarded as part of the vascular Behcet spectrum. Diagnosis remains challenging due to its rarity, lack of validated diagnostic criteria, and overlap with other vasculitis. CASE SUMMARY: We describe a 71-year-old male who presented with progressive dyspnoea and chest pain. Serial imaging demonstrated rapid enlargement of a main pulmonary artery aneurysm from 5 mm to 68 × 55 mm over 18 months, prompting surgical repair. Histopatho

Immune-inflammatory profiles, coronary artery disease, and peripheral artery disease in people with HIV.

BACKGROUND AND AIMS: People with HIV (PWH) have high risk of coronary (CAD) and peripheral (PAD) artery disease independent of traditional risk factors. Single immune-inflammatory biomarkers provide limited insight into disease risk and phenotypes, whereas integrative profiling may reveal novel mechanisms. Here we investigated associations between immune-inflammatory profiles, CAD, and PAD in PWH. METHODS: We included PWH from the Copenhagen Comorbidity in HIV Infection (COCOMO) Study with undetectable HIV RNA and measurements of 25 immune-inflammatory biomarkers. Participants underwent coron

Circulating Inflammatory Markers and Mammographic Density Phenotypes: a Mendelian Randomization Analysis.

Observational studies have identified associations between circulating inflammatory markers (CIMs) and mammographic density (MD) phenotypes, but whether these associations reflect causal effects remains unclear. We utilized two-sample Mendelian randomization (MR) to assess relationships between 60 CIMs and MD phenotypes in individuals of European genetic ancestry by examining associations between genetically predicted CIM concentrations and three MD phenotypes (dense area, DA: N = 13,965; non-dense area, NDA: N = 14,036; percent density, PMD: N = 17,841). Primary analyses were conducted using

Three months of weekly rifapentine and isoniazid versus four months of rifampicin for tuberculosis infection: a randomised controlled trial.

BACKGROUND: Treatment of tuberculosis infection (TBI) is a key pillar of the WHO End TB Strategy. Two short-course rifamycin-based regimens-weekly isoniazid plus rifapentine for 12 weeks (3HP) and daily rifampicin for 16 weeks (4RIF)-are widely recommended; however, they have not previously been directly compared in a randomised controlled trial. We compared treatment completion between 3HP and 4RIF among individuals with TBI. METHODS: We conducted a multicentre, open-label, parallel-group randomised controlled trial across seven tuberculosis clinics in Sydney, Australia, between July 2019 an

The Hepatitis C Cascade of Care for Opioid Agonist Therapy Recipients in Australia.

BACKGROUND: People prescribed opioid agonist therapy (OAT) are a key population for hepatitis C virus (HCV) elimination. Health service engagement associated with OAT provision may facilitate hepatitis C testing and treatment. We aimed to quantify the HCV care cascade among people receiving OAT in Australia. METHODS: We extracted linked data from individuals attending any of 58 clinics participating in the ACCESS national sentinel surveillance network of primary care and sexual health clinics from 1 January 2016 to 31 December 2023. Outcomes included evidence of any HCV test (antibody or RNA)

Spatial profiling reveals complex cellular responses of anti-IL5 treatment with mepolizumab in eosinophilic chronic rhinosinusitis with nasal polyposis.

There is limited understanding of the impact of anti-IL5 treatment on nasal polyp tissue biology in chronic rhinosinusitis with nasal polyps (CRSwNP). This study examined nasal polyp tissue cellular proteome and transcriptome responses to anti-IL5 treatment in CRSwNP utilizing spatial profiling. GeoMx Digital Spatial Profiling of 80 proteins and 1,833 messenger RNA targets in the polyp stroma and the whole transcriptome (18,815 messenger RNA targets) in polyp epithelia was undertaken on sinonasal biopsies collected from 20 individuals with eosinophilic CRSwNP before and after 16 and 24 wk of m

Anti-HBs Immune Complex Levels: A Novel Marker of Hepatitis Flare Following Nucleos(t)ide Analog Withdrawal in HBeAg-negative Chronic Hepatitis B.

BACKGROUND AND AIMS: Humoral immunity is thought to play a key role in the immunopathogenesis of chronic hepatitis B (CHB). Peaks in anti-HBs immune complexes (HBsAg-ICs) have been associated with subsequent HBsAg seroclearance. Discontinuation of nucleos(t)ide analog (NA) therapy can promote HBsAg seroclearance but also carries a risk of hepatitis flare. We evaluated the relationship between serum HBsAg-IC levels and hepatitis flare following NA cessation in HBeAg-negative CHB. METHODS: In a multicentre prospective cohort of noncirrhotic HBeAg-negative CHB patients undergoing NA discontinuat

2025 Clinical Practice Guideline Update by the Infectious Diseases Society of America on Group A Streptococcal Pharyngitis: Risk Assessment Using Clinical Scoring Systems in Children and Adults.

This publication represents the first part of an update to the clinical practice guideline on the diagnosis and management of group A streptococcal (Streptococcus pyogenes or GAS) pharyngitis, developed by the Infectious Diseases Society of America (IDSA). Diagnosis of GAS pharyngitis by clinician judgment alone is unreliable, and unselective testing incurs cost and inconvenience for individuals at low risk of having GAS infection. Clinical scoring systems have been used to quantify the probability of a positive GAS throat culture based on standardized criteria such as the presence of fever; t

Skin-derived myeloid precursors and joint-resident fibroblasts spread psoriatic disease from skin to joints.

Psoriatic disease initially affects the skin and later extends to the joints. Here, we show a two-step process that orchestrates the spread of inflammation from the skin to the joints. Induction of psoriatic skin disease in photoconvertible mice, followed by sequencing and computational characterization of skin-derived cells in the joints, was used to identify a population of CD2+MHC-II+CCR2+ myeloid precursors that builds a skin-derived myeloid cell compartment in the joints. Single-cell cross-species reference mapping and mitochondrial variant tracing showed an orthologous human cell populat

Social Determinants of Health and Pediatric Long COVID in the US.

IMPORTANCE: Millions of children worldwide are experiencing prolonged symptoms after SARS-CoV-2 infection, yet social risk factors for developing long COVID are largely unknown. As child health is influenced by the environment in which they live and interact, adverse social determinants of health (SDOH) may contribute to the development of pediatric long COVID. OBJECTIVE: To identify whether adverse SDOH are associated with increased odds of long COVID in school-aged children and adolescents in the US. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional analysis of a multicenter, longitu

De novo and inherited dominant variants in U4 and U6 snRNA genes cause retinitis pigmentosa.

Small nuclear RNAs (snRNAs) combine with specific proteins to generate small nuclear ribonucleoproteins (snRNPs), the building blocks of the spliceosome. U4 snRNA forms a duplex with U6 and, together with U5, contributes to the tri-snRNP spliceosomal complex. Variants in RNU4-2, which encodes U4, have recently been implicated in neurodevelopmental disorders. Here we show that heterozygous inherited and de novo variants in RNU4-2 and in four RNU6 paralogs (RNU6-1, RNU6-2, RNU6-8 and RNU6-9), which encode U6, recur in individuals with nonsyndromic retinitis pigmentosa (RP), a genetic disorder ca
