The authors have examined the ultrastructure of the rectum of infective-stage Wuchereria bancrofti by transmission electron microscopy. Our observations show that the rectum is divided into anterior and posterior segments. The cells of the anterior rectum appear to be derived from the microfilarial R (rectal) cells described by other authors. In both stages, these cells show voluminous nuclei, abundant mitochondria, and small cytoplasmic processes which contain fibrillar components. During the period 1965 to 1978, 62 cases of severe pneumococcal infection were admitted in Saint-Vincent de Paul pediatric intensive care unit. Recovery without sequelae was observed in only 17 of 49 meningitis. 5 fulminant pneumococcal infections after splenectomy or other disorders were observed. This late condition may be preventable by vaccination.

An epidemic of infection associated with Serratia marcescens and other Gram-negative organisms resistant to aminoglycosides and other chemotherapeutic agents occurred in the intensive care unit of St Vincent's Hospital, Melbourne, and spread to other areas of the hospital. This paper describes the problems of sepsis in the critically ill patient, outlines the occurrence of organisms in the patients concerned in this epidemic, and discusses the policies adopted to control the incidence of life-threatening infection caused by bacteria resistant to all other agents.

Tobramycin has been shown to be safe and effective when given intravenously in combination antibiotic therapy for infected patients with acute leukemia or other profound pancytopenic states. Tobramycin in combination with one or more antibiotics (usually cephalothin) produced satisfactory clinical results in 21 of 30 episodes of fever. Toxic effects were minimal and were limited to occasional mild thrombophlebitis.

Of 222 patients with pneumonia in St Vincent's Hospital, Sydney, in 1972, more were affected by bronchopneumonia (53%) than lobar pneumonia (46%). Two-thirds of the patients were males and 86% were aged 40 years or more. Only 59% had any bacteriological studies performed. Subphrenic abscess is still a significant hazard which complicates surgical procedures as well as certain abdominal catastrophes. This is a report of 88 patients with subphrenic abscess at St. Vincent's Hospital and Medical Center of New York from 1954 through 1971. There were 46 males and 42 females, ranging from 2 to 88 years. Operations on the stomach, duodenum and biliary tract were the major causes.
