Fulminant Powassan Virus Encephalitis Presenting as Acute Ischemic Stroke: A Case Report.

Powassan virus (POWV) is a rare tick-borne flavivirus that can cause severe neuroinvasive disease with high morbidity and mortality. Early clinical and radiographic features are often nonspecific and may mimic acute ischemic stroke, leading to diagnostic delay. We report the case of a 77-year-old woman with recent tick exposure who presented with acute-onset focal neurologic deficits and fever. Initial neuroimaging demonstrated focal diffusion restriction, raising concern for acute ischemic stroke, and dual antiplatelet therapy was initiated. Over subsequent days, the patient developed progres

Drug-Induced Pemphigus Vulgaris Following Ceftriaxone Use.

Pemphigus vulgaris (PV) is a rare, potentially life-threatening autoimmune blistering disorder characterized by intraepidermal acantholysis caused by IgG autoantibodies directed against desmoglein 1 and 3. Although most cases are idiopathic, certain medications, including antibiotics, have been implicated as potential triggers in susceptible individuals. We report the case of a 61-year-old man with a history of nonischemic cardiomyopathy, chronic kidney disease, and substance use disorder who developed widespread flaccid bullae six days after initiation of intravenous ceftriaxone for a suspect

Interobserver variability in synovial tissue biopsy markers in Rheumatoid Arthritis: Results from OMERACT special interest group on synovial tissue biopsy.

OBJECTIVES: to evaluate interobserver agreement in synovial tissue (ST) biopsy histology and immunohistochemistry (IHC) analysis across expert centers, and the influence of observers' experience on interobserver agreement. METHODS: ST slides, collected from 38 biologic-naïve rheumatoid arthritis patients, stained for H&E, FVIII, CD68, CD3 and CD19/20, were uploaded to an online platform. Using semiquantitative analysis, independent observers scored ST slides for immune and vascularity markers (0 to 4), and Krenn's synovitis score (0 to 9). Presence of lymphoid aggregates and synovial pathotyp

Prone positioning in ARDS.

Over the past five decades, prone positioning has evolved from single case reports to an evidence-based intervention. Initially used as a rescue therapy, it is now recognized as an integral component of lung-protective mechanical ventilation strategies, applied early in intubated patients with acute respiratory distress syndrome (ARDS) with a PaO2/FIO2 ratio < 150 mmHg. The COVID-19 pandemic further expanded its use to non-intubated patients, the so-called awake prone position (APP), with promising results. APP requires confirmation in non-COVID patients and in a more routine ICU practice. Imp

Hematopoietic Antigen Presenting Cells Fine-Tune the Effector Phenotype of Intrathymic Unconventional αβ T Cells.

The development of unconventional αβ T cells, including invariant natural killer T (iNKT) and mucosal-associated invariant T (MAIT) cells, in the thymus is distinct from conventional T cells. Unconventional αβ T cells adopt a memory phenotype, acquire effector functions, and can reside in the thymus long-term. It is well-established that positive selection of these unconventional T cells from CD4+CD8+ double-positive (DP) precursors depends on interaction with other DP cells. However, postselection, the regulation of their maturation and effector differentiation is less well understood. Profes

The Tumor Multi-Omic Landscape of Endometrial Cancers Developed on a Background of Adiposity.

Background: High body mass index (BMI) is a causal risk factor for endometrial cancer, but the tumor molecular mechanisms affected by adiposity remain poorly understood. Here, we characterize the tumor multi-omic landscape of endometrial cancers that have developed on a background of lifelong germline genetic liability to elevated BMI. Methods: We built a polygenic score (PGS) for BMI in women using data on independent, genome-wide significant variants associated with adult BMI in 434,794 women. We performed germline (blood) genotype quality control and imputation on data from 354 endometrial

Social and economic marginalization in the context of a public health emergency: HIV care disruptions experienced by black sexual minority men (BSMM).

BACKGROUND: HIV in the United States disproportionately affects Black sexual minority men (BSMM), including disparities in HIV care engagement and outcomes. During public health emergencies, such as the COVID-19 pandemic, marginalized populations such as BSMM may face additional barriers to HIV care engagement. METHODS: We examined associations between HIV care engagement (current antiretroviral therapy use, healthcare visits for HIV, viral load testing, viral load suppression) and experiencing COVID-19-related disruptions to HIV care in a sample of BSMM living with HIV in 2022 (n = 172). Systematic characterization of four novel Twort-group bacteriophages infecting Staphylococcus hyicus.

Exudative epidermitis (EE) in piglets, caused by the bacterium Staphylococcus hyicus, remains a major concern for the swine industry, particularly due to antimicrobial resistance. Bacteriophages represent a promising alternative treatment. This study aimed to isolate and characterize phages infecting S. hyicus from EE-affected swine farm environments in Quebec. Four novel phages (BQE1, BQE4, BQE7, BQL4) were isolated from water and manure samples. All produced clear plaques (<1 mm) and reached titers of 108-1010 PFU/mL. Electron microscopy revealed myovirus morphology. Their genomes (115.4-134
