Bariatric Surgery, Medical Weight Loss, and the Upper Aerodigestive Tract.

Obesity and intentional weight loss increasingly influence otolaryngologic disease through mechanical, metabolic, inflammatory, gastrointestinal, and regional soft-tissue effects. This review examines how bariatric surgery and contemporary medical weight-loss therapies affect the upper aerodigestive tract. Evidence is strongest for obstructive sleep apnea, in which substantial weight reduction improves disease severity but does not ensure remission. Emerging data suggest potential benefits for chronic rhinosinusitis and selected voice outcomes, alongside risks such as procedure-specific reflux

Application of Patient-operated Otoscope in Diagnosing Patulous Eustachian Tube Dysfunction.

OBJECTIVE: A patulous Eustachian tube (PET) is characterized by persistent Eustachian tube patency, leading to symptoms, such as aural fullness, autophony, and audible breathing. The diagnostic criteria include both characteristic symptoms and objective findings; however, intermittent symptom presentation often prevents confirmation during clinic visits, leading to underdiagnosis. We aimed to evaluate the clinical utility of a patient-operated portable digital otoscope for confirming the diagnosis of PET. STUDY DESIGN: Prospective observational study. SETTING: The study was conducted between

Survival analysis of initially inserted Kobayashi plugs in patulous Eustachian tube: A retrospective study of 297 ears.

OBJECTIVES: Kobayashi Plug Surgery (KPS) is widely performed in Japan for refractory patulous Eustachian tube (PET). However, the duration of retention and long-term stability of the initially inserted plug have not been fully evaluated. Therefore, this study aimed to analyze plug retention (plug survival) and assess the validity of preoperative sonotubometry (ST) probe tone levels as an indicator for plug size selection. METHODS: A retrospective review was conducted of 237 patients (297 ears) who underwent initial KPS between January 2020 and August 2025. Ears were categorized by initial plu

Aural fullness-dominant patulous eustachian tube: Clinical characteristics based on objective findings.

OBJECTIVES: Patulous Eustachian tube (PET) presents with diverse symptoms, including voice autophony, breathing autophony, and aural fullness. Among these, aural fullness is considered non-specific and its clinical significance in PET remains unclear. This study aimed to clarify the clinical characteristics of aural fullness-dominant PET by comparing objective findings according to the dominant symptom. METHODS: Patients who visited a specialized Eustachian tube clinic between June 2019 and December 2025 were screened. Among 394 patients evaluated using a self-administered questionnaire, 109

Diagnostic utility of tubomanometry for patulous eustachian tube: A comparative study.

PURPOSE: The current diagnosis of patulous Eustachian tube (PET) has limitations and cannot accurately assess its functional status. This study aims to validate tubomanometry (TMM) as an objective diagnostic tool for PET by analyzing pressure-specific R-values. METHODS: A retrospective cohort study was conducted involving 95 ears from 64 patients which were stratified into three groups: Definite PET (n = 26), Possible PET (n = 41), and Controls (n = 28). Diagnostic performance was evaluated by receiver operator characteristic (ROC) curve analysis to determine optimal R-value thresholds. RESU

Implications of Glucagon-Like Peptide-1 Receptor Agonists in Otolaryngology - Head and Neck Surgery: A Review.

The number of patients taking glucagon-like peptide-1 receptor agonists (GLP-1RAs) is increasing. Beyond diabetes and weight management, these medications have various effects within the head and neck with both beneficial and potentially adverse clinical implications. Delayed gastric emptying may contribute to reflux, chronic cough, and potential aspiration in the perioperative setting; as such, physicians should be aware of anesthetic guidelines (duration of pre-operative cessation, pre-operative fasting or liquid diet) to improve safety and avoid operative delay. GLP-1RAs have shown benefit

Eustachian Tube Disorders in Children.

Chronic pediatric middle ear disease is commonly ascribed to Eustachian tube dysfunction (ETD). This article reviews the role of obstructive and patulous ETD in otitis media with effusion, tympanic membrane retraction, cholesteatoma, and perforation. It highlights the lack of reliable diagnostic tests to measure ETD. The contribution of different causes of obstructive ETD is discussed, including the influence of mechanical obstruction, inflammation, allergy, mucociliary dysfunction, and craniofacial anomalies. ETD management has traditionally bypassed the Eustachian tube, for example, with tym

Patulous Eustachian Tube Dysfunction.

Eustachian tube dysfunction represents a continuum ranging from obstructive Eustachian tube dysfunction to patulous Eustachian tube dysfunction (pETD), and there is considerable overlap of symptoms that present challenges in the differential diagnosis. Careful history, physical, endoscopic examination, and audiological testing are all important in distinguishing between these conditions and other causes of aural fullness. There are many different treatment modalities, but none are optimal. The most common methods have a good safety record, and they are repeatable. There is a need for more rand

Effects of Body Weight on Middle Ear Disease.

Body weight can have various effects on middle ear health. Pediatric obesity is associated with an increased risk and recurrence of otitis media with effusion. Adult obesity is associated with an increased risk of temporal bone cerebrospinal fluid leak and encephalocele, and weight loss can cause symptoms of patulous Eustachian tube dysfunction. With rising rates of obesity and widespread adoption of glucagon-like peptide 1 receptor agonists, it is increasingly important that patients are appropriately counseled on how weight changes affect the middle ear. In this review, we examine the litera
