The pressure changes developing in the nasopharynx and in the external auditory canal during breathing at rest were examined by means of manometry in patients with patulous eustachian tubes and in healthy control subjects who were able to keep their eustachian tubes opened for prolonged periods of time. Findings demonstrated that pressure changes in the nasopharynx in both groups were practically the same, but pressure changes in the external auditory meatuses of the patients with patulous tubes were about one order greater than in the control group. As gas flows in and out of the nasopharynx, the pressure in that region fluctuates. It drops below or rises above atmospheric pressure, which is itself not constant but is subject to changes in altitude and weather. The disorder of hyperpatent eustachian tube is often unrecognized or misdiagnosed because of the ambiguous symptom of a "plugged ear." But when the patient also complains of autophony and/or hearing amphoric sounds, a patulous tube should be suspected. Persistent symptoms are distressing to most patients. This preliminary report evaluates a new nasal medication that produces closure of the tube and controls or eliminates these symptoms. It is demonstrated to be safe, predictable, and effective with little or no side effects.

The patulous eustachian tube is a benign, yet annoying condition that may be over-looked or misdiagnosed. It is frequently associated with peritubal atrophy, and usually responds to conservative measures such as weight gain or mucus-producing agents. In patients not responding to these measures, a variety of surgical options are available. We have been using a technique that avoids elevating a tympanomeatal flap, is reversible, and is effective. This technique, as well as other surgical options, are reviewed. The advantages and disadvantages inherent in them are discussed.

The literature relating to the effects of otological surgery on tinnitus is reviewed. The results of such surgery are often unpredictable with respect to postoperative tinnitus, and ablative surgery may well make the tinnitus worse. The concept of somatosounds (tinnitus arising from outside the auditory pathway) is presented, and the management of some conditions, including patulous Eustachian tube and palatal myoclonus, is discussed.

The diagnosis of a patulous Eustachian tube relies mainly upon the history, examination and a clinical awareness of the condition. More recently, objective tests such as tympanometry and sonotubometry have been shown to be useful in confirming the diagnosis. Reports on the effectiveness of myringotomy and tube for treatment of symptoms of the patulous eustachian tube are discouraging, even though there are no large series substantiating its ineffectiveness. The patulous Eustachian tube syndrome has previously been regarded as a troublesome but benign condition. Nine out of 13 patients reported here had evidence of cochlear damage similar to that caused by acoustic trauma. In elderly patients an unilateral sensorineural hearing loss is frequently associated with a relatively more patent eustachian tube on the involved side. Patulous eustachian tube is more common than is usually presumed. The symptoms of this condition can paradoxically simulate an obstruction of the eustachian tube. The characteristic eardrum movements synchronized with breathing can be overlooked or be absent altogether and, therefore, the diagnosis in many cases is missed. Eustachian tube function (ETF) was evaluated in 22 ears of 18 chinchillas using the inflation-deflation and forced-response protocols. The function of the Eustachian tube was tested preoperatively by sonotubometry in 100 ears subjected to surgery for chronic ear disease. 36 ears showed no tubal passage for sound during swallowing and the surgical intervention revealed cholesteatoma in 17 of these cases. In 64 cases sonotubometry gave positive results preoperatively and among these cholesteatoma was found in 26 ears. Thirteen patients (16 ears) with patulous eustachian tubes were treated with pterygoid hamulotomy combined with transposition (eight ears) or transection (eight ears) of the tendon of the tensor veli palatini muscle. Autophony and the patulous Eustachian tube is a real but rare clinical entity often misdiagnosed because the symptoms so mimic those of middle ear effusion. The diagnosis is made by the history of fullness or blockage and hearing ones own voice and breath sounds in the ear. Ten years follow-up of 48 patients (33 boys, 15 girls) with middle ear effusion treated by adeno-tonsillectomy, antral lavage and paracentesis is presented. Using the modified but stricter classification of Tos and Poulsen, the results compare well (Table V). Agreement between otoscopy and impedance measurement was remarkably high.
