BACKGROUND: There has been a recent increase in the incidence of paediatric acute mastoiditis (AM) with a global trend towards more conservative management strategies. Our aim was to evaluate the contemporary management of paediatric AM in Aotearoa New Zealand.
METHODOLOGY: A retrospective observational study including AM patients aged <18 years treated in five tertiary hospitals across Aotearoa New Zealand between 2014 and 2022.
RESULTS: There were 212 cases of AM with 104 cases (49%) of uncomplicated AM and 108 cases (51%) of complicated AM. Complications included 75 cases (35%) of sub-per

Complicated infections of the middle ear and paranasal sinuses remain a relevant clinical challenge. Although acute otitis media and acute sinusitis are generally self-limiting, a small but clinically significant proportion of cases progress to severe local, orbital, or intracranial complications. These include acute mastoiditis, labyrinthitis, facial nerve palsy, orbital complications, frontal bone osteomyelitis, intracranial abscess, meningitis, and cerebral venous sinus thrombosis, all of which are associated with considerable morbidity and potential mortality. The current review provides a

Acute mastoiditis is a recognized intratemporal complication of acute otitis media in children, but progression to frank temporal bone osteomyelitis with extensive cortical necrosis is exceptionally rare in infancy. This report presents a 1-year-old infant with refractory acute mastoiditis complicated by destructive temporal bone osteomyelitis. High-resolution computed tomography with 3D reconstruction demonstrated extensive cortical loss. Emergency subtotal mastoidectomy and radical debridement were performed. Deep intraoperative culture yielded multidrug-resistant Staphylococcus hominis, and

Background/Objectives: Recurrent acute otitis media (AOM) in children is known to cause cumulative cochlear and vestibular injury. Whether a single fulminant episode severe enough to require surgical intervention produces an analogous long-term audiovestibular signature, and whether infection severity contributes to outcome independently of cumulative episode count, is unclear. The present study addressed this gap. Methods: In this single-centre retrospective cohort study, 65 paediatric patients who had undergone surgical treatment for acute mastoiditis-the fulminant form of AOM-between July 2

BACKGROUND: Granulomatosis with Polyangiitis (GPA) is an antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis that predominantly affects the upper or lower respiratory tracts and the kidney. While otologic involvement is relatively common in GPA, isolated destructive mastoiditis as the initial manifestation is exceedingly rare, often leading to misdiagnosis.
CASE PRESENTATION: A 69-year-old female presented with a 3-months history of progressive bilateral hearing loss and otorrhea. Based on clinical and radiological findings, she was initially diagnosed with acute mastoiditis but

Subdural empyema, mastoiditis, and sinus venous thrombosis are rare infections that may lead to serious and potentially life-threatening complications. A 2-year-old male patient with no known underlying disease presented with a 2-day history of ear pain, sore throat, and fever. Physical examination revealed no significant pathology other than hyperemia of the left tympanic membrane. Intravenous ceftriaxone was initiated, and the patient was hospitalized because of persistent fever and poor oral intake. However, despite 3 days of treatment, the fever persisted, and an increase in acute phase re

BACKGROUND: While the main aim of cholesteatoma surgery is to eradicate the disease and minimize the risk of recurrence, achieving the best possible hearing after surgery remains a key secondary objective, one that matters greatly to patients. Predicting an individual's postoperative hearing outcome is difficult because it depends not only on factors such as the condition of the ossicular chain, the type of ossicular reconstruction, and the extent of the cholesteatoma, but also potentially on other influences, including middle-ear aeration, which has been suggested to affect hearing results.

Acute mastoiditis represents the most common complication of acute otitis media in childhood and, despite a declining incidence in the era of vaccine prevention, remains a clinically significant entity. Particularly in infants and immunocompromised patients, symptoms may be nonspecific, thereby complicating early diagnosis. The present review provides a structured overview of the etiology, pathophysiology, diagnostic workup, and therapeutic management of acute and chronic mastoiditis, with special emphasis on interdisciplinary aspects. The pathogen spectrum is age-dependent and subject to epide

BACKGROUND: Bilateral acute mastoiditis is an uncommon complication of acute otitis media and is often associated with systemic infections or underlying predisposing conditions. Viral infections, particularly Epstein-Barr virus (EBV), may facilitate secondary bacterial complications in susceptible pediatric patients. Alagille syndrome, a multisystem genetic disorder, may further increase vulnerability to severe infections due to immune and anatomical factors.
METHODS: We report a case of bilateral acute mastoiditis occurring in a child with Alagille syndrome during acute EBV infection: clinic

Acute mastoiditis is an infectious complication of acute otitis media, potentially leading to life-threatening extracranial and intracranial sequelae. In recent years, there has been a noticeable shift in the epidemiology of causative pathogens, with an increasing incidence of less common bacteria. This article presents the case of a 1.5-year-old child diagnosed with acute mastoiditis and additional complications, who failed to respond to empirical antibiotic therapy and had persistently negative cultures. Only through the use of polymerase chain reaction (PCR) testing was the causative organi

Acute mastoiditis is a well-recognized complication of acute otitis media. Despite the absence of a universally accepted standard of care, recent data indicate a global shift toward more conservative management, with escalation to invasive interventions primarily in the presence of complications or failure of initial therapy. The emergence of Fusobacterium necrophorum as an uncommon, yet clinically significant pathogen, challenges this paradigm and underscores the need for heightened vigilance among clinicians.

INTRODUCTION: Acute mastoiditis is one of the most common complication of acute otitis media, and its clinical course may vary significantly among patients.
OBJECTIVE: To investigate the association between adenoid size and the severity of acute mastoiditis in children.
METHODS: This retrospective cohort study included children (<18 years) hospitalized with acute mastoiditis. The primary outcome was the association between disease severity-defined by the presence of intracranial complications or PICU admission - and the adenoid-to-nasopharyngeal ratio (ANR), measured on CT scans at three nas

AIMS: Morphine-6-glucuronide (M6G), one of the active metabolites of morphine, has attracted considerable interest as a potent opioid analgesic with an apparently superior therapeutic index. To date studies have used the intravenous route, which is generally unacceptable in the treatment of cancer related pain. The aim of this study was to define the pharmacokinetics, toxicity and cardio-respiratory effects of three alternative routes of administration of M6G.
METHODS: Ten healthy volunteers participated in an open randomized study. Subjects received M6G 2 mg as an intravenous bolus, 20 mg or

BACKGROUND: Acute otitis media is usually a self-limited bacterial infection; however, in rare cases, contiguous spread to the mastoid and adjacent dural venous sinuses may result in life-threatening intracranial complications. Otogenic transverse-sigmoid sinus thrombosis is uncommon in adults and represents a diagnostic and therapeutic challenge because its early presentation may be nonspecific and evidence guiding management remains limited.
CASE PRESENTATION: We report the case of a 32-year-old man with no relevant medical history who presented with a 3-week history of right-sided otorrhea

Orbital cellulitis is a serious vision and potentially life-threatening condition. In this case report, we present a 3-year-old boy who was admitted with orbital cellulitis, complicated by an intraconal orbital abscess, cavernous sinus thrombosis, petrous apicitis, labyrinthitis, and mastoiditis. There was no paranasal sinusitis, which is the most common cause of orbital cellulitis. His condition initially did not improve with intravenous antibiotics, but gradually improved after drainage of the orbital abscess with anticoagulant followed by corticosteroid therapy. We illustrate the importance

Otitis media, a common childhood disease, can lead to serious complications such as acute mastoiditis and, rarely, Luc's abscess, with life-threatening consequences. Luc's abscess, a rare but severe complication, can occur without acute mastoiditis. This case report details a case of Luc's abscess in a 14-year-old girl with acute otitis media, presenting with ear pain, facial swelling, and hearing loss. Treatment involves a multidisciplinary approach, considering factors like age and abscess extent. Though there is no established standard treatment, this case emphasizes the need for prompt rec

Among acute mastoiditis (AM) complications, cerebral venous sinus thrombosis (CVST) is particularly severe, leading to increased intracranial pressure and potential neurological sequelae. Predicting the development of such complications is challenging. The aims of the present study were to evaluate the incidence, clinical characteristics, and risk factors for the development of CVST in AM. A retrospective study was conducted on children hospitalized with AM at the Meyer Children's Hospital between 2016 and 2024. Patients were divided into two groups: those with CVST (group A) and those without

Introduction: Central venous thrombosis (CVT) represents a well-documented complication of acute otitis media (AOM) and acute mastoiditis (AM). Despite widespread antibiotic utilization, which has significantly reduced the incidence of severe AOM/AM complications, recent years have witnessed an increasing frequency of thrombotic complications in pediatric patients, not invariably presenting with classical neurological manifestations.Aim: This study aimed to investigate the potential correlation between COVID-19 infection and increased CVT incidence, while
