Acute mastoiditis is an infection of the air cells in the mastoid and is primarily seen in the pediatric population. This disease usually occurs after patients develop otitis media, which can result in acute mastoiditis as a complication. Most patients usually present with generalized symptoms of an infection; however, in some instances, cerebral venous thrombosis can occur. Such a rare complication has been described in children, although in rare instances, it can happen in adults. In fact, only a few cases of acute mastoiditis resulting in cerebral venous thrombosis have been reported in adu

BACKGROUND: Balance parameters derived from wearable sensor measurements during postural sway have been shown to be sensitive to experimental variables such as test duration, sensor number, and sensor location that influence the magnitude and frequency-related properties of measured center-of-mass (COM) and center-of-pressure (COP) excursions. In this study, we investigated the effects of test duration, the number of sensors, and sensor location on the reliability of standing balance parameters derived using body-mounted accelerometers.
METHODS: Twelve volunteers without any prior history of

IgG4-related disease (IgG4-RD) is an immune-mediated condition affecting nearly any organ. This review focuses on the nuances of diagnosing IgG4-RD affecting the head and neck. Salivary gland involvement, especially of the submandibular glands, often permits a definitive diagnosis on biopsy. However, elevated IgG4+ plasma cells are nonspecific and can be seen in chronic sialadenitis, lymphoma, and other mimics. Careful correlation of clinical and pathological findings is essential. Given the significant overlap with chronic sinusitis, IgG4-RD of the sinonasal region is difficult to diagnose hi

BACKGROUND: COVID-19 pandemic is thought to have changed the epidemiology of some pediatric neurosurgical disease: among them are the intracranial complications of sinusitis and otitis (ICSO). According to some studies on a limited number of cases, both streptococci-related sinusitis and ICSO would have increased immediately after the pandemic, although the reason is not clear yet (seasonal changes versus pandemic-related effects). The goal of the present survey of the European Society for Pediatric Neurosurgery (ESPN) was to collect a large number of cases from different European countries en

Various approaches have been taken over the years to quantify event-related potential (ERP) responses and these approaches may vary in their utility connecting empirical research and scientific claims. In this work we compared different quantification methods as well as the influence of three reference methods (linked mastoids, average reference, and current source density) on the resulting ERP amplitude. We use the experimental effects and effect sizes (Cohen's d) to evaluate the different methodological variants and we calculate intraclass correlation coefficients (ICC). In addition, the boo

BACKGROUND: Eagle syndrome, an uncommon condition, causes symptoms due to neural and/or vascular compression from an elongated styloid process or calcified stylohyoid ligament and can also complicate other planned surgical procedures.
OBSERVATIONS: A 42-year-old female with loss of balance, dizziness, and ataxic gait underwent cranial magnetic resonance imaging (MRI), revealing a right-sided Koos grade IV vestibular schwannoma. Initially, a retrosigmoid craniotomy for tumor resection was planned. However, preoperative MRI and computed tomography (CT) showed a dilated right-sided mastoid emiss

BACKGROUND AND PURPOSE: Fusobacterium necrophorum (F necrophorum) is an anaerobic bacteria that causes invasive head and neck infections in children. Several studies have demonstrated an increasing prevalence of F necrophorum as the causative agent in acute mastoiditis in children, with associated high rates of intracranial complications such as epidural abscess and sinus venous thrombosis, to name a few. F necrophorum requires a treatment protocol that differs from the empiric treatment that is tailored to more common pathogens (eg, group A streptococci, Streptococcus pneumonia), and hence ex

Hypertrophic pachymeningitis (HP) is a rare disorder marked by thickening of the dura mater due to diverse etiologies. MPO-ANCA-positive HP represents a variant of AAV confined to the central nervous system, distinguished by the presence of serum MPO antibodies. Distinguishing HP triggered by MPO-ANCA from other causes can be challenging. In this study, we present two cases of MPO-ANCA-positive HP initially misdiagnosed as intracranial infections. Case 1 underwent surgery for chronic suppurative otitis media, with histopathological findings revealing inflammatory changes without definitive supp

IgG4-related disease (IgG4-RD) is a complex multi-system inflammatory disorder that can affect various organs in the body. This condition is characterized by elevated levels of immunoglobulin G subclass 4 (IgG4) and the presence of specific histopathological features. While neurological involvement is not as common as in other organs, when it occurs, it can lead to hypertrophic pachymeningitis and hypophysitis. Here, we present a case of a 53-year-old male with right-sided hemicranial headache and diplopia. Computed tomography revealed a soft tissue density lesion in the middle ear cavity and

BACKGROUND: Most US children with acute otitis media [AOM] receive prompt antibiotic treatment, though guidelines encourage watchful waiting. Previous systematic reviews of antibiotics versus watchful waiting have focused on symptom resolution and RCTs, limiting the assessment of serious, rare complications. We sought to evaluate these complications by including observational studies.
METHODS: RCTs and observational studies that compared antibiotics to placebo or watchful waiting for pediatric clinician diagnosed AOM were identified [PubMed/MEDLINE, Embase, Cochrane Database of Systematic Rev

PURPOSE: With an incidence between 1-9/100 000 per year, Langerhans cell histiocytosis (LCH) is a rather rare disease from the hemato-oncologic disease spectrum (Hayes et al. 2009). The tumorlike disease with proliferation of histiocytic cells may manifest as localized to one organ or disseminated with infiltration of a wide variety of organs. Approximately 25-30 % of these cases show involvement of the temporal bone (Ni et al. 2017).
CASE DESCRIPTION: With vertigo persisting for three years, chronic mastoiditis, and acute progressive hearing loss bilaterally (r > l) for three weeks, a 41-yea

INTRODUCTION AND IMPORTANCE: Acute internal carotid artery (ICA) thrombosis is a rare complication of mastoiditis, which can spread to intracranial vessels, leading to thrombosis and infarction.
CASE PRESENTATION: An 83-year-old man presented to the neurologist with progressive left-sided ear swelling and peripheral facial palsy lasting several weeks. Intravenous antibiotic was prescribed for chronic mastoiditis. Sudden consciousness disturbance and right hemiplegia occurred during treatment. Brain magnetic resonance imaging revealed acute ICA occlusion with hemispheric cerebral infarction. S

Background: The COVID-19 pandemic has notably affected the epidemiology of various infectious diseases. The imposed public health measures and disruptions in vaccination programs have potentially altered the patterns of these diseases post pandemic. Objective: To investigate the change in epidemiology of otorhinolaryngological infectious diseases in adult and pediatric patients after the COVID-19 pandemic and the relaxation of public health measures. Methods: A retrospective cohort study was conducted at a large tertiary university otolaryngology department in the south of Germany, examining a

PURPOSE: Common respiratory infections were significantly reduced during the COVID-19 pandemic due to general protective and hygiene measures. The gradual withdrawal of these non-pharmaceutical interventions (NPI) was associated with a notable increase in these infections, particularly in pediatric and adult otorhinolaryngology. The aim of this retrospective monocentric study was to evaluate the impact of NPI during the COVID-19 pandemic on the incidence and severity of acute mastoiditis (AM).
METHODS: Pre-pandemic clinical data of AM cases from 2011 to 2019 were compared with infection count

PURPOSE: Axl is a therapeutic target under active clinical investigation for a variety of cancer indications. While pre-clinical mouse studies have been promising, clinical trials targeting Axl in combination with standard-of-care therapies have yielded mixed results. We hypothesize that this clinical translation gap may in part be due to an incomplete understanding of the effects of Axl inhibition specifically on the myeloid compartment of the human tumor microenvironment, as Axl mediates immunoregulatory signaling in myeloid cells. Utilizing human in vitro melanoma tumor microenvironment mod

Petrous apicitis and acute bacterial meningitis are uncommon in the present antibiotic era. The diagnosis of petrous apicitis is seldom considered unless there is cranial nerve palsy. A young child with aplasia cutis congenita presented with acute bacterial meningitis and an incidental opacified left mastoid in brain imaging. During the course, fever persisted, and high-resolution temporal bone imaging showed rapid progression to coalescent mastoiditis, petrous apicitis with erosions of tegmen tympani, and petrous apex. Other findings included bony dehiscences and thinning of left calvaria. Ty

Gradenigo Syndrome (GS) is not a common intracranial complication of acute otitis media infections that needs proper diagnosis and management. This case report presents a 6-year-old child diagnosed with unilateral headache, right eye diplopia, and right abducens nerve palsy. MRI and cranial nerve examination facilitate the confirmation of GS. The patient was treated with proper antibiotics and showed good outcomes. This case report highlights the importance of early detection and understanding of the clinical picture associated with GS, which have a crucial role in achieving desired outcomes a
