A Case Report of Lyme Carditis Presenting as New-Onset Atrial Flutter in the Emergency Department.

Lyme disease (LD) is the most common vector-borne disease in the United States and has several well-known pathologic sequelae if not treated, including third-degree atrioventricular block. However, an association between Lyme carditis and atrial arrhythmias has been described in some cardiology literature but has not been reported in emergency medicine or wilderness medicine literature. This case report describes an older adult with new-onset atrial flutter with rapid ventricular rate in the setting of early disseminated LD, who ultimately was treated with antibiotics, had resolution of atrial

First records of Borrelia burgdorferi sensu lato in voles and selected Apodemus species in Serbia.

BACKGROUND: Voles and Apodemus mice are recognized as important hosts for numerous parasites and pathogens. Understanding the occurrence of zoonoses, key host characteristics, and the factors influencing their population dynamics and distribution is essential for predicting disease occurrence and assessing the risks of transmission and spread. Determining the role of voles and field mice, in maintaining Borrelia burgdorferi, s.l. in the environment clarifies its transmission during tick activity periods and highlights the need for timely monitoring and control measures. RESULTS: We examined t

IL36γ signaling promotes mammalian anti-tick immunity.

Ticks, like Ixodes scapularis, can parasitize a diverse array of vertebrates, favoring natural hosts but feeding opportunistically on others. Natural hosts, as well as permissive hosts such as laboratory mice, tolerate repeated infestations, while many incidental hosts may acquire anti-tick immunity, as first described in guinea pigs. We show that tick-immune guinea pigs display altered dermal cytology and a distinct transcriptome, predominantly upregulating the IL36γ cytokine. Interference with IL36γ signaling, via gene knockdown, chemical inhibition, or its natural antagonist IL36RN, reverse

Borrelia garinii spirochetemia in a previously healthy child presenting with Streptococcus anginosus group brain abscess: a case report.

BACKGROUND: The clinical significance of transient Borrelia burgdorferi sensu lato complex (hereafter referred to as B. burgdorferi) spirochetemia remains poorly understood and has rarely been documented. We report the isolation of viable B. garinii genospecies from the blood of a previously healthy child presenting with a Streptococcus anginosus group (SAG) brain abscess. CASE PRESENTATION: An eight-year-old girl presented with four days of headache, fever, nausea, slightly reduced general condition, and mild facial asymmetry. Initial evaluation, including lumbar puncture, raised suspicion o

Not always cancer-Probable Lyme disease mimicking squamous cell carcinoma: A case report.

Lyme disease, caused by Borrelia burgdorferi, can produce head and neck manifestations in up to 75% of cases, most commonly facial palsy and lymphadenopathy. Borrelial lymphoid proliferations are recognized mimics of malignancy; however, tonsillar involvement simulating a malignant neoplasm has not been previously described. We report a 57-year-old male patient who presented with a right cervical lump associated with odynophagia, weight loss, and low-grade fever. PET-CT demonstrated intense FDG uptake in a right palatine tonsillar mass and right cervical lymph nodes, with loss of fat plane def

Lyme Disease and Other Tick-Borne Infections.

PURPOSE OF REVIEW: The neurologic manifestations of selected tick-transmitted infections in North America and Eurasia were reviewed. RECENT FINDINGS: Both Powassan virus in North America, and Tick-Borne Encephalitis (TBE) virus in Eurasia can cause encephalitis, often with significant neurologic morbidity. Lyme disease, Ehrlichia chaffeensis (the cause of human monocytic ehrlichiosis [HME]), and Borrelia miyamotoi infections can cause meningitis (the third primarily in immunocompromised individuals). Patients with Lyme disease may also develop cranial neuropathy, and/or radiculoneuropathy. Ti

Mortality dynamics of West Nile virus and Lyme disease in the United States: Temporal, spatial, and socioeconomic dimensions (2003-2021).

BACKGROUND: West Nile virus (WNV) and Lyme disease pose a growing public health threat in the United States. This study investigated spatial and temporal mortality trends of WNV and Lyme disease and evaluated associations with individual and county-level factors. METHODS: We utilized National Center for Health Statistics data to identify WNV- and Lyme-related deaths (2003-2021), including demographics and the decedent's county of residence. County-level insurance status was sourced from the U.S. Census Bureau, and urbanicity was defined using the 2013 National Center For Health Statistics cla

A promoter G-quadruplex recruits Hrb27C to regulate NEMO and immune defense against Lyme disease pathogens in the deer tick.

Genomic DNA can adopt noncanonical secondary structures, such as G-quadruplexes (G4s), which serve as critical regulatory elements beyond the traditional double helix. Whether promoter G4s also participate in controlling innate immune signaling and host-pathogen interactions, however, remains unknown in arthropods. In this study, we identify a G4 structure within the promoter of the NEMO (NF-κB essential modulator) gene in the deer tick Ixodes scapularis. Using a combination of circular dichroism, isothermal difference spectra, and fluorescence-based assays, we demonstrate that this sequence f

Characterization of the Peromyscus leucopus immunoglobulin germline repertoire and anti-OspA antibody response reveals conserved germline IGHV features among cricetid rodents.

Peromyscus leucopus (P. leucopus) is an important reservoir host for several zoonoses, including Borrelia burgdorferi (B. burgdorferi), the causative agent of Lyme disease. However, the immunoglobulin germline repertoire underlying antibody responses in this reservoir host remains poorly defined. In this study, we characterized the immunoglobulin variable genes of P. leucopus and identified 12 IGHV, 12 IGKV, and 3 IGLV gene families comprising 129, 87, and 12 genes, respectively. These genes exhibited similar usage patterns to those of Mus musculus (M. musculus). We also identified a high-dive

Development of bilateral facial nerve palsies while receiving doxycycline for early disseminated Lyme disease.

BACKGROUND: Disease progression is uncommon in patients diagnosed with Lyme disease who receive appropriate therapy. Here we report a case of a patient developing bilateral facial nerve palsies while receiving oral doxycycline therapy for early disseminated Lyme disease and review the literature of similar cases. CASE REPORT: A 58-year-old previously healthy male from the mid-Atlantic area in the U.S. presented with a multifocal erythematous rash consistent with erythema migrans after international travel. His clinical presentation and two-step Lyme-serology testing confirmed a diagnosis of L

Identifying counties at elevated acarological risk for reporting high incidence of Lyme disease in the eastern United States.

Lyme disease, caused primarily by the bite of a nymphal blacklegged tick (Ixodes scapularis) infected with Borrelia burgdorferi sensu stricto (Bbss), is the most common vector-borne disease in the United States. Counties reporting high Lyme disease incidence (LDI) continue to expand beyond historic foci in the Northeast and Upper Midwest. Identifying areas of expansion aids in targeting efforts to improve Lyme disease awareness in affected communities. Tick surveillance is conducted to track changes in acarological risk for tickborne diseases; the density of host-seeking Bbss-infected I. scapu

Lyme Carditis Beyond Atrioventricular Block: Cardiac Arrest and Transient Atrial Fibrillation With Preserved Atrioventricular Conduction.

Lyme disease may involve the heart during early dissemination and most commonly produces atrioventricular (AV) conduction abnormalities. PR interval prolongation, therefore, frequently guides clinical suspicion and management. We describe a previously healthy man who presented with seizure-like activity, aborted cardiac arrest, transient atrial fibrillation, and cardiac magnetic resonance findings consistent with acute myocarditis, although AV conduction remained normal. Serologic testing subsequently confirmed Lyme carditis. The patient recovered completely with oral doxycycline. This case il

Persistent Borrelia burgdorferi protein antigen in CD16 + monocytes is associated with monocyte-skewed immune dysregulation in post-treatment Lyme disease syndrome (PTLDS).

UNLABELLED: Post-treatment Lyme disease syndrome (PTLDS) is characterized by persistent symptoms after antibiotic therapy for Borrelia burgdorferi infection. The mechanisms underlying these symptoms remain incompletely understood and have been attributed to ongoing low-level infection, autoimmunity, or immune dysregulation driven by residual bacterial antigens. We investigated whether Borrelia burgdorferi protein antigen persists in long-lived monocyte subsets in PTLDS patients and is associated with broader immune dysregulation. CD14 ⁺ CD16 ⁺ monocytes from 10 patients with PTLDS were analyze

Capture sequencing enables enhanced genome recovery of Borrelia burgdorferi in mice.

The Lyme disease spirochete, Borrelia burgdorferi, maintains a low burden during vertebrate infection. This limitation has historically proven to be particularly challenging for genome-based studies of B. burgdorferi infection and spirochete presence after antibiotic treatment. Here, we used TBDCapSeq, an oligo capture-based next- generation sequencing assay, to demonstrate its utility for genome recovery of B. burgdorferi directly from C3H- and BALB/c-infected mice. Tissue fragments from the ear, heart, tarsus, and infection-site skin were collected at 3 and 6 months post-tick infection from

Public awareness of alpha-gal syndrome and tick bite prevention practices, United States, Fall 2023.

Alpha-gal syndrome (AGS) is a unique non-infectious tickborne disease that can develop after a tick bite. AGS can cause severe allergic symptoms (when patients are exposed to alpha-gal) and be challenging for patients to manage because alpha-gal is found in mammalian meat, dairy, and some non-food products. We sought to understand public awareness of AGS, especially relative to other tickborne diseases. We used a 2023 nationally representative survey of adults to investigate public awareness of AGS in the United States and found that knowledge was very low, with 70% of respondents having no pr

Tick-borne diseases in French Guiana: A comprehensive review pinpointing differences from mainland France.

French Guiana (FG), an Amazonian French territory, represents a unique ecological and epidemiological setting for tick-borne diseases (TBDs). In contrast to mainland France, where TBDs are well-documented, data from FG remain scarce. We carried out a systematic review using the PRISMA criteria for our search of the PubMed, ScienceDirect, and SciELO data bases. We identified 68 articles, of which 43 met our inclusion criteria. Data from FG were compared with those originating from mainland France. Albeit highly prevalent in Europe, and notwithstanding detection of a distinct Borrelia species in
