The evolving epidemiology of paediatric acute mastoiditis in New Zealand.

BACKGROUND: In 2017, PCV-13 was replaced by PCV-10 in the New Zealand immunisation schedule. The aim of this study was to evaluate the impact of this change on the incidence and microbiology of paediatric acute mastoiditis (AM) in NZ. METHODS: A retrospective observational study was conducted, including patients aged <18 years with an ICD-10 diagnosis of AM documented in the NZ National Minimum Dataset between 2014 and 2022. Clinical and microbiological data were obtained for a sub-group of cases admitted to five tertiary hospitals across NZ. Cases were divided into four periods, 2014-2016 (P

Intratemporal and Extratemporal Complications in a Rare Case of Acute Mastoiditis: A Case Report and Literature Review.

Here, we present a case of a 15-year-old female who presented with an unusual association of intra-temporal (peripheral facial paralysis and petrous apicitis) and extra-temporal (extra-Dural empyema) complications of acute mastoiditis, which was managed with conservative treatment. To our knowledge, this is the first report of such an association. Acute mastoiditis is inflammation and/or infection of the mastoid cavity, which is the most common complication of acute otitis media, although the incidence of complications from otomastoiditis has significantly decreased in the 21st century as a re

Conservative management outcomes in acute pediatric mastoiditis.

BACKGROUND: The aim of this study is to determine whether initial conservative surgical treatment of acute mastoiditis (AM) leads to an increase of morbidity, and to report on recent developments in epidemiology and microbiology. METHODS: At the end of 2016, a new guideline for treatment of pediatric AM was introduced at our tertiary referral center. Mastoidectomy in case of a retroauricular abscess was changed to retroauricular needle aspiration or incision. All patients diagnosed with AM 2017-2021 were identified retrospectively in our patient file records and all relevant clinical data ret

Management of acute mastoiditis in children: a retrospective analysis.

BACKGROUND: Acute mastoiditis (AM) is the most common complication of acute otitis media (AOM) and could lead to serious complications if not diagnosed early and treated appropriately. Nowadays, there is no definitive consensus about the diagnostic algorithm and the optimal therapeutic management for patients with AM. The purpose of this study is to analyze the management of children admitted for AM and complicated AM (CAM) in a referral children's hospital, evaluating differences in clinical presentation and management to outline a diagnostic and therapeutic pathway. Moreover, the incidence o

Acute mastoiditis-associated cerebral sinus venous thrombosis in children: a retrospective review.

UNLABELLED: Cerebral sinus venous thrombosis (CSVT) is a rare but serious complication of acute mastoiditis in children. Early diagnosis is often delayed due to nonspecific symptoms. This study aimed to identify clinical and laboratory predictors of CSVT in children with mastoiditis to support timely imaging and treatment. We performed a retrospective case-control study across two university-affiliated centers from 1999 to 2022 of children with acute mastoiditis who underwent neuroimaging. Clinical, laboratory, imaging, and treatment data from 81 children (40 with CSVT and 41 without) were com

''Clinical Management of Acute Mastoiditis in a Pediatric Patient with Leukocyte Adhesion Deficiency''.

Leukocyte adhesion deficiency type-1 is frequently associated with otorhinolaryngological manifestations, most notably a high prevalence of otitis media. Although multiple case reports have documented an increased prevalence of acute otitis media and mastoiditis in patients with LAD-1, no clinical or experimental studies have fully elucidated the pathophysiological mechanisms underlying this association. Despite guideline recommendations for urgent antibiotic therapy in immunocompromised patients with acute otitis media, a standardized treatment protocol for its complications remains lacking.

Evaluating the Sequelae of Mastoidectomy for Acute Mastoiditis: A Long-Term Follow-Up Study of Mastoid Function.

Background: Despite the widespread use of antibiotics, acute mastoiditis (AM) and related complications resulting from acute purulent otitis media continue to occur, predominantly in children. Although numerous studies have focused on the pathogenesis, aetiological agents, and treatment of AM, comprehensive investigations of the long-term outcomes of AM and the physiological consequences of surgical intervention in the temporal bone are lacking. Methods: Thirty patients who had undergone mastoidectomy for acute mastoiditis at a median age of 2.12 years were invited for evaluation at least five

Ear and nose-related intracranial empyema in children: a 9-year cohort study from a tertiary referral center.

OBJECTIVE: Intracranial empyemas (IEs) are rare but severe complications of ear, nose, and throat (ENT) infections, defined as purulent collections in the normally virtual meningeal spaces. IEs can be classified as sinogenic (S-IE) or otogenic (O-IE), according to the initial infection. These life-threatening complications can entail long-term neurological disabilities. Few studies have described their characteristics, and cohorts are usually small and heterogeneous. In the Greater Paris area, all cases of pediatric IE are referred to the authors' center. The aim of this study was to describe
