Acute bacterial meningitis as a complication of otitis media and related mortality factors.

The aim of this study was to evaluate the characteristics of patients with acute bacterial meningitis (ABM) developed secondary to acute and chronic otitis media (OM). Between 1991 and 2001, among 269 adult patients with ABM, 56 who were secondary to OM were included in the study. We reviewed the charts of patients who were diagnosed as ABM following acute or chronic OM. Risk factors associated with mortality were determined by using a logistic regression model. The mean age of the patients, 38 male and 18 female, was 25.8 +/- 10.8 years (range 14 - 65). Forty-four of these cases (79%) have ha

Intracranial complications of acute and chronic mastoiditis: report of two cases in children.

OBJECTIVE: The clinical picture of mastoiditis, sigmoid sinus thrombosis and brain abscess has changed with the advent of antibiotics. A delay in the recognition of intracranial complications in children and in the institution of appropriate therapy may result in morbidity and mortality. Increased mortality of the children has been correlated with the neurological status of the patient on admission to hospital. METHOD: A retrospective study was made of two children with acute mastoiditis and sigmoid sinus thrombosis and chronic mastoiditis with cerebellar abscess treated in 1997 in the ENT De

Cerebellopontine angle lymphoma presenting as chronic mastoiditis.

Middle-ear tumours are not uncommonly mistaken for mastoiditis on presentation. We report a case of cerebellopontine angle non-Hodgkin's B-cell lymphoma originally presenting as a middle-ear inflammatory process. In mastoiditis not resolving with conventional treatment it is important to look for an underlying cause.

Computed tomography and the diagnosis of coalescent mastoiditis.

OBJECTIVE: To evaluate the sensitivity and accuracy of temporal bone CT findings for the diagnosis of acute coalescent mastoiditis. DESIGN: CT scans were blindly scored for mastoid bone integrity (air cell septae, sigmoid cortical plate, and lateral cortical wall) by an otologist and 2 neuroradiologists. Scores were analyzed to determine their sensitivity and specificity for acute coalescent mastoiditis. SUBJECTS: Twenty-one patients with acute coalescent mastoiditis or acute noncoalescent mastoiditis and 12 patients with chronic mastoiditis. SETTING: Academic tertiary care facility. RESUL

Management of abducens palsy in patients with petrositis.

Our objective was to discuss the management and outcome of abducens nerve palsy in patients with Gradenigo's syndrome. In a retrospective analysis of patients with Gradenigo's syndrome at a tertiary-care center in Houston, Texas, from 1987 to 1995, we identified 2 patients with Gradenigo's syndrome, both female. One had bilateral involvement, so that the total was 3 ears. Both patients had complete recovery of their abducens nerve palsy. In 2 ears with chronic mastoiditis, sixth nerve palsies failed to respond to medical therapy alone, but resolved after mastoidectomy with drainage of the petr

Extensive cranial vault and skull base osteomyelitis complicating chronic mastoiditis : a case report.

The authors report an unusual case of extensive osteomyelitis of cranial vault and skull base as a complication of chronic mastoiditis in a 19 year old girl. The diagnosis was confirmed by bone biopsy which revealed osteomyelitic changes and large bacterial colonies. The unusual features of this case is discussed along with review of relevant literature.

Combined mastoid/middle cranial fossa repair of temporal bone encephalocele.

Temporal bone encephalocele (TBE) has become less common as the incidence of chronic mastoid infection and surgery for this condition has decreased. Due to its declining incidence, the diagnosis of TBE may be delayed and result in the development of serious complications such as cerebrospinal fluid leak, meningitis, epidural or subdural abscess. Six cases of large (>1 cm) TBE of diverse etiology are described. Two patients had suffered previous temporal bone fractures, two had had prior mastoidectomy, and two patients had long-standing chronic mastoiditis. Two patients had undergone prior unsu

Recurrent bacterial meningitis: a 6-year experience in adult patients.

Ten adult patients with recurrent bacterial meningitis (RBM) of 22 episodes were diagnosed and treated at the Dicle University Hospital from January 1990 to December 1995. Apart from 22 episodes of RBM these patients had an additional 25 episodes treated at other hospitals. The RBM attacks developed after closed head trauma in four patients, asplenia and chronic otitis media in one patient, chronic otitis media and oto-mastoiditis in one patient, chronic maxillary sinusitis in one patient, chronic mastoiditis in one patients, and suppurative foci of facial bones caused by shrapnel pieces and n

T cell lymphoma of the ear presenting as mastoiditis.

Mastoiditis is a complication of otitis media characterized by suppuration and destruction of air cell septa in the mastoid and petrous pyramid. Diagnosis is made by clinical findings and computerized tomography (CT) of the temporal bone. We present a patient initially diagnosed by CT as having chronic mastoiditis who was subsequently shown to have an unusual large-cell malignant lymphoma of T cell type.

Mastoiditis in children.

Fifty-seven children were seen over a 10-year period, 1984-1994, at two large pediatric referral centers with a diagnosis of mastoiditis. Twelve had acute infection and 45 had chronic manifestations. Clinical presentations and recovered bacterial pathogens were identical to those reported in earlier literature although the incidence of both acute and chronic mastoiditis has decreased markedly since 1950. The availability of computed tomographic (CT) scans during this decade has improved the management of chronic disease by defining the location of cholesteatomas and the extent of disease as we

Mastoiditis in Kelantan.

Chronic mastoiditis and subperiosteal abscess are rarely seen nowadays in most countries. Thirty-four cases of mastoiditis were reviewed to find out the prevalence of chronic mastoiditis in the east coast of Malaysia. Twenty (58.82%) of these cases were a sequelae of chronic suppurative otitis media with cholesteatoma. All the patients with chronic mastoiditis were more than six years old. Forty-five percent of cases with chronic mastoiditis had a well pneumatized mastoid air cell on the unaffected side. The occurrence of chronic mastoiditis or cholesteatoma in a well pneumatized mastoid is no

Success and pitfalls in myringoplasty: follow-up study of 404 cases.

Results of 404 primary myringoplasty operations were critically analyzed. Patients who underwent tympanic membrane repair as the only procedure were included. In all cases, temporal muscle fascia was used as graft material. Mean follow-up period was 5.5 years. Overall success rate was found to be 88 percent. Welding injuries did not heal as well as other traumatic perforations or perforations caused by otitis media. Age of the patient, size or site of perforation, or surgical technique (underlay or overlay) did not affect success rate. Twenty-five percent of the failures were considered to be

Chronic mastoiditis mimicking recurrence of nasopharyngeal carcinoma: report of a case.

A nasopharyngeal mass with cranial neuropathies usually indicates an advanced neoplastic process. We present a case of nasopharyngeal carcinoma diagnosed and treated 38 years ago. A nasopharyngeal mass recurred with cranial neuropathies and concurrent otologic infection in June, 1990. Repeated nasopharyngeal biopsies were negative with respect to tumors. Suspecting direct extension of the otologic infection, modified radical mastoidectomy was performed. The nasopharyngeal mass resolved after the surgical intervention. Spread of otologic infection from the temporal bone to the nasopharynx may b

Diagnosis and management of anaerobic infections of the head and neck.

Anaerobic bacteria are important pathogens in head and neck infections such as chronic otitis media, chronic sinusitis, chronic mastoiditis, head and neck abscesses, cervical adenitis, parotitis, and postoperative infection. Bacteroides sp (Bacteroides melaninogenicus group, Bacteroides oralis, and Bacteroides fragilis group), Peptostreptococcus sp, and Fusobacterium sp predominate. The observed recent increase in the number of beta-lactamase-producing strains of Bacteroides sp isolated in head and neck infections has been associated with increased failure rates of the penicillins in the manag

Evaluation and management of temporal bone arachnoid granulations.

The diagnosis and evaluation of temporal bone arachnoid granulations (AGs) is based on characteristic bone defects in the posterior or middle fossa surfaces of the temporal bone in adult patients with persistent serous otitis media or septic meningitis following acute otitis media. The cerebrospinal communication caused by AGs should be repaired by a middle fossa craniotomy (middle fossa AGs) or an intact canal wall mastoidectomy (posterior fossa AGs) to preserve normal sound transmission. In adult patients with chronic mastoiditis, the dural defect of an AG may permit intracranial extension o

Tuberculous mastoiditis.

The prevalence of Mycobacterium tuberculosis is increasing due in part to the rising incidence of immunocompromised hosts. Although Mycobacterium tuberculosis is a well-documented pathogen in otitis media and mastoiditis, its extremely low incidence often precludes consideration when treating these infections. This is especially true when concomitant pulmonary symptoms are absent. We describe two patients who initially presented after prolonged unsuccessful medical and surgical therapy for chronic mastoiditis. In both, the diagnosis of Mycobacterium tuberculosis mastoiditis was made after the

Treatment of chronic mastoiditis by grafting of mastoid cavities with autologous epithelial layers generated by in vitro culture of buccal epithelium.

Autologous cultured epithelial layers were established from biopsies from the mucosa of the cheek, a non-keratinizing region of the oral cavity. These were grafted to the unepithelialized mastoid cavities of nine patients with chronic mastoiditis and severe otorrhoea varying from two to 30 years' duration. All procedures were performed on an out-patient basis, with no anaesthesia except for topical anaesthesia for the mucosal biopsy. In seven of the patients the grafts took well, with complete resolution of the otorrhoea for a minimum follow-up period of eight months. In one patient there was
