Audiological Outcomes in 465 Primary Myringoplasties: Reporting Pitfalls and Lessons Learned.

OBJECTIVES: This study aimed to assess long-term audiological outcomes in 465 primary myringoplasties and evaluate factors influencing surgical success. METHODS: A retrospective analysis was conducted on 442 patients (465 procedures) between 2001 and 2024. Outcomes were evaluated based on perforation location and surgical period. RESULTS: Mean air-bone gap improved from 19.1 dB to 9.8 dB post-operatively. Air-bone gap closure less than 10 dB was achieved in 53 per cent of patients. Posterior perforations yielded significantly better outcomes (8.7 dB vs. 10.2 dB; p = 0.0226). CONCLUSION: A r

Bilateral tympanic membrane perforation during bronchoscopy in an intensive care unit patient: a case report.

BACKGROUND: Bronchoscopy is a fundamental and generally safe interventional procedure in respiratory medicine, with recognized complications being predominantly confined to the airways. Traumatic tympanic membrane perforation, while a common form of barotrauma, is exceptionally rarely associated with bronchoscopy. This report describes a case of bilateral tympanic membrane perforation complicating bronchoscopy, aiming to enhance clinicians'awareness of this uncommon iatrogenic injury. CASE PRESENTATION: A 77-year-old man presented with cough and shortness of breath. Emergency investigations r

Regenerative Nanoscaffolds for Chronic Tympanic Membrane Perforation: From Bench to Clinical Translation.

Chronic tympanic membrane (TM) perforation remains difficult to resolve without surgery because of poor intrinsic healing capacity and limitations of passive, nonsurgical materials. Although patch-based approaches are common, most merely provide passive coverage and lack intrinsic therapeutic capability to drive tissue regeneration. We developed and clinically evaluated an extracellular matrix (ECM)-mimetic, drug-free nanopattern guidance (NG) patch for TM repair, marking a paradigm shift from passive coverage to active, biophysically driven regeneration. The NG patch is a biocompatible, impla

The Efficacy of Acupuncture and Bee Venom Injections in Improving Hearing in a Patient with Idiopathic Conductive Hearing Loss: a case report.

Conductive hearing loss (CHL) is commonly caused by tympanic membrane perforation or otitis media. Congenital ossicular anomalies and otosclerosis are the primary considerations in patients with normal tympanic membranes and external auditory canals, whereas stapedius tendon abnormalities are less frequently observed. Acupuncture and bee venom (BV) injections are frequently used for treating otological disorders; however, their effectiveness in treating CHL remains unexplored. Therefore, this case report describes a 22-year-old man with idiopathic left-sided CHL that persisted for 101 days. Wh

Adjuvant Use of Intranasal Fluticasone Furoate to Improve Graft Uptake in Type 1 Tympanoplasty Cases: A Comparative Study.

BACKGROUND: Chronic otitis media (COM) with tympanic membrane perforation remains a significant cause of hearing impairment worldwide. While tympanoplasty is the standard surgical treatment, graft failure rates of 10%-30% persist despite technical refinements. Topical corticosteroids may improve outcomes by modulating postsurgical inflammation. OBJECTIVE: The objective of the study was to compare graft uptake rates in type 1 tympanoplasty between patients receiving adjuvant topical intranasal fluticasone furoate (FF) versus standard postoperative care alone. METHODOLOGY: This prospective cro

Using leucocyte- and platelet-rich fibrin (L-PRF) concentrates as middle ear packing in type 1 tympanoplasty versus absorbable gelatin sponge: a ten-year comparative study.

PURPOSE: To compare the use of L-PRF versus absorbable gelatin sponge as middle ear packing in type I tympanoplasty, in terms of perforation closure and complication rates. METHODS: We retrospectively analyzed data from patients who underwent type 1 tympanoplasty for tympanic membrane perforation between January 2013 and Decembre 2022, performed by a single surgeon. After reviewing these 10 years, the two cohorts were highly asymmetrical in size, so we extended data collection to earlier cases for the smaller group (back to 1997), to achieve comparable cohort sizes (n = 115). Confounding fact

Combined Basic Fibroblast Growth Factor and Gelatin Sponge Therapy Achieves Near-Normal Tympanic Membrane Regeneration After Blast Injury in Mice.

HYPOTHESIS: We hypothesized that combining basic fibroblast growth factor (bFGF) with a gelatin sponge scaffold (bFGF+gel therapy) would correct the regenerative deficits specific to blast injury and achieve near-normal tympanic membrane (TM) regeneration. BACKGROUND: Blast-induced TM perforation (TMP) is refractory to spontaneous healing. bFGF promotes closure but results in markedly thickened regeneration when used alone. METHODS: CBA/J mice were subjected to shockwave blast exposure (330±11.3 kPa). Their ears were randomized into the normal, blast, bFGF, Gel, or bFGF+gel groups (n=6 each)

Experimental analysis of cartilage graft positioning in tympanoplasty and its impact on middle ear mechanics.

PURPOSE: Cartilage is widely used for tympanic membrane (TM) reconstruction, but the impact of graft position relative to the bony ear canal on middle ear mechanics remains unclear. This study examined how cartilage placement affects the middle ear transfer function (METF) and TM vibration. METHODS: Single‑point Laser Doppler Vibrometry (LDV) was performed on five fresh frozen human temporal bones before and after creating a 2.5 mm perforation in the posterior inferior TM quadrant and after reconstruction with a round-shaped cartilage graft of 4 mm diameter and 0.5 mm thickness placed either

Impact of diabetes mellitus on tympanic membrane healing after fully endoscopic tympanoplasty.

BACKGROUND AND AIMS: Diabetes mellitus (DM) is known to impair wound healing in various tissues. However, its impact on tympanic membrane (TM) healing following tympanoplasty remains controversial. While clinical studies generally suggest that DM does not affect the final TM closure rate, animal experiments indicate it may significantly prolong healing time. This study aims to systematically investigate the correlation between diabetic status and perioperative glycemic control levels with tympanic membrane healing time following tympanoplasty, thereby enhancing the understanding of the impact

Microbiota in Chronic Suppurative Otitis media: association with Postoperative Tympanic membrane outcomes.

Chronic suppurative otitis media (CSOM) is a prevalent condition with global health implications due to its impact on hearing and quality of life. Conventional treatments often fail because of bacterial biofilms and antimicrobial resistance. Effective treatment of CSOM depends on the precise determination of the middle ear microbiota; however, current microbial detection methods do not meet this need. Postoperative reperforation may compromise surgical outcomes. If the risk of perforation can be predicted immediately after surgery, sensitive antibiotics could be administered proactively for ea
