Clinical characteristics of otogenic lateral sinus thrombosis in patients under 18 years old compared with that in adult patients.

BACKGROUND: Otogenic lateral sinus thrombosis (OLST) is a rare complication of otitis media. We do not know whether the disease progress between the pediatric and adult OLST patients is consistent. However, pediatric surgical methods always refer to the adults'. AIMS/OBJECTIVES: This study aimed to seek evidence for suitable surgical methods in pediatric patients. MATERIALS AND METHODS: The clinical manifestation, laboratory findings, and findings in operation between children and adult groups were compared. RESULTS: Ten children and 17 adults OLST patients were included. Most pediatric pat

Middle ear congenital cholesteatoma: systematic review, meta-analysis and insights on its pathogenesis.

PURPOSE: Congenital cholesteatoma (CC) presents as a white pearl-like lesion behind a normal tympanic membrane (TM), without a history of otorrhea, infection, perforation or previous otologic surgery. Several recent studies provided new data improving this pathology characterization. The aim of this paper is to expand the knowledge about CC and to provide new insights on its pathogenesis. METHODS: The study consisted of two main research parts: (1) systematic review and meta-analysis; (2) medical literature review englobing anatomy, histology, embryology and congenital pathology of the ear.

Evaluation of Residual Disease Following Transcanal Totally Endoscopic vs Postauricular Surgery Among Children With Middle Ear and Attic Cholesteatoma.

IMPORTANCE: Transcanal totally endoscopic ear surgery (TEES) improves tympanic cavity visibility during cholesteatoma resection but does not readily permit 2-handed surgery, which is possible with a postauricular approach and use of an operating microscope. Improved visibility and 2-handed access may reduce rates of incomplete surgical clearance and residual cholesteatoma. OBJECTIVE: To compare rates of residual cholesteatoma after TEES vs those after open surgery with a postauricular approach for initial disease limited to the middle ear and/or attic. DESIGN, SETTING, AND PARTICIPANTS: This

Chronic Otitis Media Associated with Cholesteatoma in a Case of the Say-Barber-Biesecker-Young-Simpson Variant of Ohdo Syndrome.

BACKGROUND The Say-Barber-Biesecker-Young-Simpson (SBBYS) variant of Ohdo syndrome is characterized by congenital hypothyroidism, facial dysmorphism, postaxial polydactyly, and mental retardation. The SBBYS variant of Ohdo syndrome is extremely rare with only 19 cases previously reported in the literature. A case is presented of chronic otitis media associated with cholesteatoma in a six-year-old boy with the SBBYS variant of Ohdo syndrome. CASE REPORT A 6-year-old boy presented with perforation of the tympanic membrane and a cholesteatoma in the mesotympanic-attic region associated with chron

Automated diagnosis of ear disease using ensemble deep learning with a big otoendoscopy image database.

BACKGROUND: Ear and mastoid disease can easily be treated by early detection and appropriate medical care. However, short of specialists and relatively low diagnostic accuracy calls for a new way of diagnostic strategy, in which deep learning may play a significant role. The current study presents a machine learning model to automatically diagnose ear disease using a large database of otoendoscopic images acquired in the clinical environment. METHODS: Total 10,544 otoendoscopic images were used to train nine public convolution-based deep neural networks to classify eardrum and external audito

Efficacy of Tympanoplasty Without Mastoidectomy for Treating Chronic Otitis Media in Patients With Mastoid Cavity Opacification in Temporal Bone Computed Tomography Findings.

OBJECTIVES: Combined mastoidectomy is generally preferred to tympanoplasty alone when treating patients with chronic otitis media (COM), particularly when temporal bone computed tomography (TBCT) shows that the mastoid cavity contains opacification of soft tissue density. However, in cases with Eustachian tube dysfunction, a mastoid cavity volume may be a burden to its function. We hypothesized that tympanoplasty alone might be better than tympanoplasty combined with mastoidectomy because soft tissue in the mastoid cavity is a sequel to a protective physiological response. Thus, we explored th

[The status of contralateral ear in children with cholesteatoma].

INTRODUCTION: Recently published data indicate that over 60% of patients with middle ear cholesteatoma (MEC) have concomitant changes in the contralateral ear (CE). Studies concerning the condition of the CE are few and rarely present the situation in the pediatric population. THE AIM OF THIS STUDY: was to assess the frequency of pathological changes occurring in the CE in children operated on MEC, as well as to determine their clinical evolution. MATERIAL AND METHOD: Retrospective analysis was performed on basis of medical records of 193 patients treated surgically for MEC in 1998-2008 at t

The value of scutum erosion in the diagnosis of temporal bone cholesteatoma.

CONCLUSIONS: Scutum erosion has an important role in the diagnosis of attic cholesteatoma, but it does not appear in other types of cholesteatoma in temporal bone. Without scutum erosion, the existence of cholesteatoma cannot be excluded. OBJECTIVE: To evaluate the value of scutum erosion in the diagnosis of cholesteatoma in temporal bone and its limitations. SUBJECTS AND METHODS: CT scanning and otoscopy were used in all patients who were suspected of developing, or were probably diagnosed with, cholesteatoma in temporal bone. The condition of the scutum was recorded by otoscopy, CT scannin

Tympanosclerosis and our surgical results.

Tympanosclerosis is a sequel of chronic otitis media characterized by the deposition of calcareous plaques following hyaline degeneration in the fibrous layer of the tympanic membrane and inside middle ear mucosa. It usually results from prior middle ear infections. In this study, results of treatment and clinical findings of the patients with tympanosclerosis are presented. Our objective is to determine the changes in middle ear caused by tympanosclerosis and intervene in the more problematic zone and find out the more useful treatment protocol for patients. Preoperative features, middle ear

Cartilage tympanoplasty: literature review.

There has been renewed interest in the use of cartilage for middle-ear reconstructions. The aim of the present review is to examine the indications, techniques and surgical outcomes of cartilage tympanoplasties reported in the literature. There have been concerns regarding weakening of cartilage struts, from histological studies involving explants; as a result, cartilage struts for ossiculoplasty have not gained popularity. On the other hand, cartilage tympanoplasty is now an established procedure for tympanic membrane and attic reconstruction. The commonest techniques involve cartilage palisa

Anterior atticoantrostomy for cholesteatoma surgery.

OBJECTIVES: We aimed to investigate the long-term results of anterior atticoantrostomy in adult patients with cholesteatoma. METHODS: A total of 83 ears in 78 patients were operated on by the anterior atticoantrostomy technique, supported by a periosteal flap, between 1991 and 2002. RESULTS: Cholesteatoma recurred in only 4 ears (4.8%). In the 79 ears without recurrence, re-perforation was observed in 3 ears (3.8%), and retraction pockets developed in the attic of 5 ears (6.3%), 2 of which needed ventilation tubes. Absorption or migration of cartilage grafts was not seen in any of the patien
