OBJECTIVE: To investigate the clinical classification and treatment of localized attic cholesteatoma through surgical management.
METHOD: Twenty-one patients (21 ears) of localized attic cholesteatoma accepted atticotomy. Then different surgical managements were carried out according to the extent of cholesteatoma and whether the malleo-incudal joint was destroyed or not. If the cholesteatoma has only reached the surface of the malleo-incudal joint, it was regarded as type I (5 patients). To this type, the cholesteatoma was removed and the ossicular chain was preserved. If the cholesteatoma h

OBJECTIVES: Microscopic postauricular tympanomastoidectomy provides a limited exposure to the attic, especially anteriorly. In contrast, the endoscope offers wide transcanal access to the attic, allowing for complete removal of limited attic disease, possibly without interrupting the ossicular chain. This report evaluates 8 years of experience with transcanal endoscopic management of limited attic cholesteatoma.
STUDY DESIGN: Case series.
METHODS: Seventy-three ears with limited attic cholesteatoma underwent endoscopic transcanal tympanotomy and extended atticotomy to access and completely r

INTRODUCTION: Management of chronic perforation and severe posterior and/or attic Retraction Pocket (RP) or atelectasis of the tympanic membrane continues to be one of the most difficult problems for otologists.
AIM OF THE STUDY: To analyse the usefulness of the tympanic membrane reconstruction with tragal cartilage.
MATERIALS AND METHODS: The study included 27 patients with chronic perforation or severe RP operated by the same surgeon, using tragal cartilage tympanoplasty. Seven supplementary procedures were required for recurrent cholesteatoma (second-look surgery).
RESULTS: There has bee

BACKGROUND: Despite a high mortality rate in childhood, there is limited evidence on the causes and outcomes of paediatric bloodstream infections from low- and middle-income countries (LMICs). We conducted a systematic review and meta-analysis to characterize the bacterial causes of paediatric bloodstream infections in LMICs and their resistance profile.
METHODS: We searched Pubmed and Embase databases between January 1st 1990 and October 30th 2019, combining MeSH and free-text terms for "sepsis" and "low-middle-income countries" in children. Two reviewers screened articles and performed data

The clinical features of 58 consecutive patients presenting with a new case of chronic otitis media were prospectively collected over a 15-month period. Twenty-three ears had a keratin filled marginal or attic defect (14 with cholesteatoma), 20 had a self-clearing marginal or attic defect, and 21 had a central tympanic membrane perforation (including one cholesteatoma). Twenty patients (35 per cent) had an abnormal finding in the opposite ear. The patients' ages were dispersed over a wide range of age groups with a mean age of 34 years. Hearing loss was the most common presenting symptom (78 p

The prevalence of chronic otitis media was studied in an urban and a rural community in southern Vietnam in 1995. A total of 3,300 children, aged from 6 months to 10 years, were selected randomly by taking every third child on the vaccination list. In this cross-sectional study, 453 ears were found to have chronic otitis media or sequelae otitidis, giving an overall prevalence of 6.86%. A total of 140 ears had perforations. Various attic changes were found in 73 ears and other chronic tympanic membrane changes, such as atrophy, tympanosclerosis and adhesions, in 219 ears. Cholesteatoma was see

We report our experiences in managing a patient with cholesteatoma complicated by meningitis, labyrinthitis and facial nerve palsy. The antero-inferior half of the tympanum was aerated but the postero-superior portion of the tympanic membrane was tightly adherent to the promontry mucosa. An attic perforation was present at the back of the malleolar head. High-resolution computed tomography also uncovered a fistula in the lateral semicircular canal. Surgical exploration of the middle ear cavity demonstrated that both the vestibule and cochlea were filled with cholesteatoma, and the cholesteatom

INTRODUCTION: Pathohistological studies have shown that in secretory otitis media an increased number of mast cells have been found in mucosa of the middle ear and an increased level of histamine in the cavity of the middle ear. The aim of this study was to analyse the distribution and functional state of mast cells in middle ear mucosa biopsies in patients with chronic otitis media.
MATERIAL AND METHODS: The pathohistological analysis was performed on 118 biopsy specimens of mucosa of the middle ear. The samples were taken intraoperatively from 39 patients during the surgical treatment of ch

INTRODUCTION: Lymphomas appear mostly in lymph nodes and parenchymal organs such as liver and spleen, while other localizations are less frequent. They are classified as Hodgkin and non-Hodgkin types, and these two are divided into subtypes, according to cell morphology and the characteristics of concurrent elements. Parotid salivary gland lymphomas are rare primary tumours of this region, although 80% of all salivary gland lymphomas, appear in parotid. They originate from intraparotid or periparotid lymph nodes, or from lymphoid elements of other pathological states in the gland such as siala

The presence of keratin in the middle ear cavity is usually associated with the diagnosis of cholesteatoma or epidermoid metaplasia. Analysis of a series of 18 cases suggests that it may correspond to a specific entity developing in the course of severe or long-lasting opened chronic otitis. This condition, we called mallear epidermosis, is characterized by: i) a perforation of the tympanic membrane lining the handle of the malleus and the umbo; ii) a proliferation of keratin surrounding the handle of the malleus and diffusing into the mesotympanum on the internal side of the tympanic membrane

Use of costal cartilage in middle ear surgery. Tympanic membrane retraction often occurs in otologic pathology. In an effort to reduce this pathology, the surgical procedure considered incorporates the use of a new material which diminishes retraction and allows freer movement of the eardrum. Since 1991, we have been using costal cartilage allograft obtained from donors and treated to prevent disease transmission. The allografts were used for tympanoplasty, retraction pockets and attic reconstruction. More than 150 patients were treated and the results evaluated in order to determine the indic

The status and hearing function of the contralateral ears in 493 patients undergoing surgery for chronic otitis media were studied at the final follow-up examination of the patients (on average, 6.5 years after the surgical treatment). In only 37% of the patients was the contralateral ear found to be normal, with atrophy of the pars tensa and tympanosclerosis being the most common abnormal findings, followed by pars tensa and attic retractions. In 18% of the patients, the contralateral ear had also undergone surgery. In only 64% of the patients, the contralateral ears had normal (< or = 20 dB)

We present a 76-year-old male patient with adhesive-type cholesteatoma and with metal foreign bodies which were shown to be located in the bony eustachian tube by computed tomography. He sustained a burn injury of the left tympanic membrane when he was struck by a bomb 52 years ago, during World War II. The cannonball fragments that entered the tympanic cavity were apparently transported to and stuck in the eustachian tube isthmus by mucociliary action after spontaneous closure of the tympanic membrane perforation. Persistent tubal obstruction due to the impacted foreign bodies and surrounding

The records of 190 adults who presented over a 10-year period with previously untreated aural cholesteatomas were analyzed with respect to age, mode of presentation at the level of the tympanic membrane, and pneumatization of the mastoid. The findings were compared with those obtained in an earlier study of 109 children (aged 13 years and younger) seen over the same period of time. Two main types of cholesteatomas were found in both adults and children, most of them (82.3%) associated with sclerotic or diploic mastoids (i.e., poorly pneumatized mastoids) and the rest (17.3%) with pneumatized m

Persistent retraction pockets causes erosion by adhering to ossicles and other structures of the middle ear. These are characterized by lysis of fibrous layer. Being in contact the squamous epithelium and the middle ear mucosa, it comprises the precursor mechanism of cholesteatoma. The present study discards the classical theories assuming epithelial migration through marginal or attic perforation; and demonstrates that these secondarily develop as a result of cholesteatoma. Retraction pocket specimens obtained from 10 cases of adhesive otitis media were examined with light and transmission el

The use of the closed tympanoplasty in the treatment of cholesteatoma has been abandoned by many surgeons in the last years. In contrast, as well as other authors we have increasingly limited the use of open techniques. Our attitude towards the closed tympanoplasty is strictly connected with an accurate prevention strategy of recurrent cholesteatoma. The recurrent cholesteatoma takes origin from two following mechanism: 1--the residual cholesteatoma which exteriorizes towards the external auditory canal; 2--the new immigration of the skin into the middle ear through a new perforation or depend
