A specific form of chronic epitympanitis has been identified in children. This pathology includes tympanic membrane perforation above he lateral process of the malleus and is termed cholesteatomic epitympanitis . This is because it is accompanied by epidermis growth across the perforation into Prussak's space. This form of epitympanitis occurred in 40 (75%) children out of 53 cases operated during 10 years. It is underlined that this pathology can hardly be diagnosed since it often develops together with sclerosis of mastoid cells. These children had frequent recurrent otitis media. An immedia

Cholesteatoma in children presents characteristics that differentiate it from the adult condition. Surgery has been performed in 55 ears. The main reason for referral was otorrhoea (83 per cent), either alone (29 per cent) or with hearing loss (54 per cent). Apart from three cases with an intact eardrum, a perforation was always present mostly posterior-superior (50 per cent). Open techniques were performed in 27.3 per cent of the ears and the closed technique in 72.7 per cent. Only in 18 per cent of cases was the ossicular chain normal and mobile. The attic was occupied by cholesteatoma in 79

To obtain the best possible results of treatment of acquired cholesteatoma, we made a subdivision of cholesteatoma types into attic and pars tensa cholesteatomas, and subdivided the latter further into tensa retraction cholesteatoma and sinus cholesteatomas. Tensa retraction cholesteatoma is defined as arising from a retraction or perforation of the whole pars tensa, whereas sinus cholesteatoma is defined as arising from a retraction or perforation of the postero-superior part of the tensa. We present the long-term results obtained in tensa retraction cholesteatomas treated with one stage surg

Of 325 previously untreated cholesteatomas, 109 were found to be in children 13 years old and younger--however, these 109 ears were not found to consist of one clinical entity. Sixty-three of the 109 ears presented a marginal perforation or a retraction pocket, at the level of Shrapnell's membrane or beyond the postero-superior quadrant. In this group the cholesteatoma was distributed mainly in the attic and mastoid and was associated with a non-cellular mastoid. A second group comprising 31 ears presented with cholesteatoma behind an intact drum and were considered to be primary cholesteatoma

We classified cholesteatomas as attic cholesteatoma, developing from Shrapnell's membrane; tensa cholesteatoma, originating in pars tensa, which is subdivided into tensa retraction cholesteatoma involving the entire pars tensa, and sinus cholesteatomas, developing from a posterosuperior retraction (perforation). From 1964 to 1980, one-stage operations were carried out on 271 ears with sinus cholesteatomas. Follow-up included 90% of the patients, and the median observation time was 9.75 years. The recurrence rate was 10%. The recurrence rate was found to be independent of the mastoidectomy type

In order to examine whether a myringotomy made with a CO2-laser might be alternative to a ventilation tube, a perforation was made in the upper rear quadrant bilaterally, of 22 rats. The laser-made perforations showed a delayed healing pattern, about twice the closing time of a similar-sized perforation made with a myringotomy lancet. Serous effusion was constantly observed in the attic space during the first 9 days but only in one ear was there actual discharge through the perforation. The size of the perforation started to diminish when the hyperplastic keratinizing squamous epithelium reach

The object of this study was to establish the morphological changes which occur when a polyethylene grommet is implanted in the normal tympanic membrane of experimental animals. We found a serous effusion around the grommet and in the attic during the first weeks after installation. All the grommets had been displaced away from the tympanic memebrane in a medical direction into the middle ear cavity. The displacement was most likely brought about by a squamous cell epithelial and connective tissue hyperplasia, which reached its maximum after 2--3 weeks. Neither atrophy, atelectasis, nor retrac

An attic cholesteatoma is defined as an epidermoid cyst found in the attic. This is differentiated from an infected retraction pocket of the pars tensa or a retraction pocket cholesteatoma. Stratified squamous epithelium may also be present in the middle ear as other clinical or pathological entities, such as metaplastic islands of the mucosa in chronic ears with central perforations. Histological examination of 22 temporal bones with attic cholesteatomas has shown them to reside mainly medial to the ossicular chain. This explains the difficulty they have in self-cleansing, as well as the ensu

There are two main types of chronic otitis media in children. The "benign" type with a central tympanic membrane perforation may heal spontaneously with conservative management. However, if the central tympanic membrane perforation fails to heal, it should be closed surgically--preferably at the age when the incidence of ear disease in children is lowest. Chronic otitis media with cholesteatoma usually has an attic or marginal perforation and requires surgical treatment to prevent serious complications. The incidence of this disease has been decreasing because of awareness by primary physician
