In this study, we present an innovative approach in medicine, leveraging the potential of artificial intelligence (AI) for the diagnosis of tympanic membrane perforations. Due to its central role in sound transmission, the tympanic membrane is a key element in clinical assessment and therapeutic decision-making. Our research focuses on the automated detection of tympanic membrane perforations using advanced AI algorithms, capable of accurately identifying whether a perforation is marginal or not. This study evaluates the performance of a convolutional neural network (CNN) optimized by a geneti

BACKGROUND: Tympanic membrane perforation repair continues to evolve, with a shift toward endoscopic approaches and diverse grafting materials. Subtotal perforations with minimal anterior margins pose a significant clinical challenge. This study evaluated the efficacy of a novel composite graft (partial-thickness tragal cartilage overlaid with temporalis fascia) compared to established techniques: full-thickness tragal cartilage (butterfly graft) and temporalis fascia.
METHODS: This observational retrospective study was conducted at a tertiary care setting. The medical records of 70 consecuti

Overlay myringoplasty is a well-established technique for repairing challenging tympanic membrane perforations, particularly anterior and subtotal defects. We report our experience with an endoscopic variant using cartilage grafts, which appears feasible and may offer favorable graft stability. This retrospective case series included five patients who underwent endoscopic cartilage overlay myringoplasty between February 2024 and February 2025. The grafts were placed laterally to the fibrous layer and medially to the tympanic membrane skin and canal skin flap. Anatomical and functional outcomes

INTRODUCTION: Retraction pockets and marginal perforations of the pars tensa of the tympanic membrane (TM) are most commonly found at superior posterior quadrant (SPQ). The patulous Eustachian tube tends to manifest in the same quadrant. Variation in the structure of the TM may explain these observations.
MATERIAL AND METHODS: A line defined by the manubrium was used to divide the pars tensa into anterior and posterior portions. A transverse line centered on the umbo divides the pars tensa into superior and inferior parts, resulting in four quadrants. Surface areas of each of the TM quadrants

PURPOSE: A trinucleotide repeat expansion in TCF4 is thought to cause Fuchs endothelial corneal dystrophy (FECD) in ~70% of European patients. In addition, strong evidence exists for the involvement of rare variants in COL8A2 and SLC4A11 in a small number of FECD cases, and more controversially, it has been suggested that variants in ZEB1, AGBL1, and LOXHD1 may also be involved. We screened patients without a TCF4 repeat expansion for causative variants in the other candidate FECD genes.
METHODS: Genomic DNA from blood was genotyped for expansion of the CTG18.1 repeat in intron 2 of TCF4 usin

OBJECTIVES/HYPOTHESIS: The inlay "butterfly" cartilage tympanoplasty was first described as a treatment for small, central ear drum perforations. This technique can also be applied in large, marginal perforations, whereas the cartilage graft is anchored on the bony annulus. The aim of this study was to present the technique, to evaluate the results of butterfly tympanoplasty in marginal perforations, and to compare with the results of butterfly tympanoplasty in patients with nonmarginal perforations.
STUDY DESIGN: Retrospective case series.
METHODS: The files of patients who underwent inlay

This prospective study aimed to assess the spontaneous healing mechanisms of human dry and endogenous wet traumatic tympanic membrane perforations (TMPs). A total of 106 patients with traumatic TMPs were included. Based on the presence or absence of endogenous bloody or watery exudates on the residual eardrum and/or at perforation margins, 73 perforations were classified as dry perforations and the remaining 33 as endogenous wet perforations. In 65 of the 73 dry perforations, centripetal migration of an epithelial layer was the first event seen during the spontaneous healing process, which was

CONCLUSIONS: Scutum erosion has an important role in the diagnosis of attic cholesteatoma, but it does not appear in other types of cholesteatoma in temporal bone. Without scutum erosion, the existence of cholesteatoma cannot be excluded.
OBJECTIVE: To evaluate the value of scutum erosion in the diagnosis of cholesteatoma in temporal bone and its limitations.
SUBJECTS AND METHODS: CT scanning and otoscopy were used in all patients who were suspected of developing, or were probably diagnosed with, cholesteatoma in temporal bone. The condition of the scutum was recorded by otoscopy, CT scannin

UNLABELLED: Otic blast injury is caused by arrhythmic air blast wave. The perforation of the tympanic membrane is the commonet finding associated with lacerations of mucosa in the middle ear. Makki [6] published 34 cases of myringoplasty after war blast injuries. However, healing of such perforations is common; Kerr [7] noted a healing rate of 83% after the blast injury. The aim of the study was to evaluate different therapic procedures of otic blast injuries. There were 74 patients with bilateral otic blast injuries, who underwent otomycroscopic examination. The following parameters were note

Mast cells are known to reside in pars flaccida membranae tympani and other parts of the middle ear mucosa in both rats and humans. However, the normal pars tensa contains no or only a few mast cells along the manubrial vessels. We examined the pars tensa of 25 rats in an experimental model of acute otitis media and found 13 mast cell clusters in 6 membranes. A majority of clusters were located in the anterior, superior quadrant and contained from 3 to 23 cells. The mast cells resided immediately under the inner epithelial lining or in the intermediate, fibrous layer. In two membranes mast cel

Persistent retraction pockets causes erosion by adhering to ossicles and other structures of the middle ear. These are characterized by lysis of fibrous layer. Being in contact the squamous epithelium and the middle ear mucosa, it comprises the precursor mechanism of cholesteatoma. The present study discards the classical theories assuming epithelial migration through marginal or attic perforation; and demonstrates that these secondarily develop as a result of cholesteatoma. Retraction pocket specimens obtained from 10 cases of adhesive otitis media were examined with light and transmission el
