Six cases of collective lightning-induced arc explosion injuries: A case report.

Currently, clinical studies on lightning injuries are mostly limited to single-patient cases, and there is still limited understanding of the mechanisms and clinical features of injuries that occur simultaneously among multiple individuals in a localized area. This article reports the environmental conditions, injuries, and treatment of six patients who suffered from arc explosion injuries caused by lightning within a 200-m radius. The incident occurred in late June on a rainy day at a coastal location (approximately 680 m elevation). Six patients were simultaneously injured by a lightning-ind

Electrospun PLA-gelatin composite scaffolds loaded with 3D-derived extracellular vesicles for repairing tympanic membrane perforation in rats.

BACKGROUND: The tympanic membrane is an oval, funnel-shaped, translucent structure that plays a crucial role in the auditory system. Individuals suffering from tympanic membrane perforation often experience symptoms such as a sensation of fullness in the ear, hearing impairment, and other discomforts, all of which significantly diminish the quality of life. Currently, autologous temporalis myofascia is the commonly utilized material for repair; however, it is limited by its scarce availability, the need for secondary surgical intervention, and suboptimal recovery outcomes. This study explores

Jugulotympanic Glomus: A Case Report on a Rare Presentation of a Vascular Middle Ear Tumor.

Jugulotympanic paragangliomas (JTPs) are neuroendocrine tumors, frequently benign, that originate from paraganglia cells. Due to their small initial size and slow growth, they tend to be asymptomatic for long periods, leading to delayed presentation. Our case report describes a JTP complicated by mastoiditis and a Bezold abscess. We report the case of a 70-year-old female with a history of hypertension and an ischemic stroke 20 years prior, who was diagnosed with an unresectable JTP via MRI. At the time of presentation, she reported progressive ear pain, fever, and a tender mass behind the ear

Usage of Cornea in Tympanoplasty: A Prospective Study.

INTRODUCTION: CSOM is inflammation of the mucoperiosteal lining of the ME cleft characterized by ear discharge, permanent perforation of the TM, and hearing impairment. If perforation fails to heal, surgical closure is done by M-plasty (Type-I T-plasty). The graft material used to reconstruct the eardrum is mainly TF. The goal is to reconstruct the TM and sound-conducting mechanism in a long-lasting way. Here, in this study, corneal homograft was considered for closure of TM perforation as part of primary T-plasty. To utilize unused cornea as homograft in T-plasty for closure of TM perforation

Evaluation of the safety of 1% voriconazole ear drops for the treatment of tympanic membrane perforation-associated suppurative otomycosis.

Otomycosis is a common otolaryngologic condition commonly complicated by tympanic membrane perforation (TMP). Although topical voriconazole has been found effective in TMP-free otomycosis, data on its safety in TMP-associated otomycosis are limited. This study was conducted to determine the safety profile of 1% ear drops of voriconazole in a rabbit model of TMP-associated suppurative otomycosis due to a clinical isolate of Aspergillus terreus. The local preparation was applied three times daily for seven consecutive days. Therapeutic efficacy was assessed by otoendoscopy, while safety was esta

Bezold's Abscess Secondary to a Subclinical Mastoid Infection: Diagnostic Challenges and Literature Review.

Bezold's abscess is a rare but serious extracranial complication of otitis media, typically resulting from erosion of the mastoid tip. We report the case of a 34-year-old male with a two-month history of recurrent right-sided otalgia, occurring two to three times and each treated with short courses of oral antibiotics, without any ear discharge, who presented with a progressively enlarging, painful, right cervical swelling and low-grade fever. The recurrent mild otologic symptoms, likely the result of repeated antibiotic exposure, delayed recognition of the more serious underlying mastoiditis.

Surgical Outcomes of Collagen Patch Versus Fat Plug in the Repair of Tympanic Membrane Perforations: A Prospective Study.

Small perforations of the tympanic membrane (TM) remain a common concern in otology, with potential consequences such as recurrent infections, conductive hearing loss, and cholesteatoma formation if left untreated. Conventional grafting materials like temporalis fascia are widely accepted, but minimally invasive office-based alternative, including collagen patch and fat plug, are increasingly recognized for their efficacy and patient convenience. This prospective study was conducted at Sree Balaji Medical College and Hospital, Chrompet, Chennai, and included 46 patients aged 20-50 years with s

Flapless Endoscopic Myringoplasty in Large Perforations: Is Tympanomeatal Flap Elevation Necessary?

The role of tympanomeatal flap elevation in type I tympanoplasty remains debated, particularly in cases of large tympanic membrane perforations. We report the case of a 59-year-old patient with a long-standing right-sided otorrhea and hearing loss. Otoscopic examination revealed a large inferior perforation involving the anteroinferior and posteroinferior quadrants. Audiometry demonstrated a 15 dB air-bone gap. The patient underwent flapless endoscopic myringoplasty using a 30° 4K endoscope. A bean-shaped cartilage with attached perichondrium was sculpted and positioned beneath the malleus han

Posterior Quadrants and Malleolar Perforations as Key Predictors of Hearing Loss in Chronic Otitis Media: A Clinicoradiological Correlation.

Introduction Chronic otitis media (COM) of the mucosal type, characterized by a central tympanic membrane (TM) perforation, is a common cause of preventable conductive hearing loss in developing countries. While the impact of perforation size on hearing loss is established, the independent and combined roles of perforation site, malleolar involvement, and mastoid pneumatization remain underexplored. The primary objective of the study is to assess the relationship between hearing loss and the size and site of TM perforation. The secondary objective is to assess the effect of the duration of dis
