Eustachian Tube Disorders in Children.

Chronic pediatric middle ear disease is commonly ascribed to Eustachian tube dysfunction (ETD). This article reviews the role of obstructive and patulous ETD in otitis media with effusion, tympanic membrane retraction, cholesteatoma, and perforation. It highlights the lack of reliable diagnostic tests to measure ETD. The contribution of different causes of obstructive ETD is discussed, including the influence of mechanical obstruction, inflammation, allergy, mucociliary dysfunction, and craniofacial anomalies. ETD management has traditionally bypassed the Eustachian tube, for example, with tym

Adenoidectomy with tympanostomy or with myringotomy alone: A systematic review of pediatric randomized controlled trials.

BACKGROUND: Otitis media with effusion (OME) is a leading cause of acquired conductive hearing loss in children. When performing an adenoidectomy for OME, surgeons must decide whether to supplement the procedure with a simple myringotomy or a myringotomy with tympanostomy tube (TT) insertion. While TT offers extended middle ear ventilation, it carries a risk of long-term complications. This systematic review of randomized controlled trials (RCTs) aimed to compare the efficacy and safety of these two approaches. METHODS: A comprehensive search of PubMed, Scopus, CENTRAL, and Web of Science was

Scalable distributed agreement from LWE: Byzantine agreement, broadcast, and leader election.

BACKGROUND: European cities are placing a larger emphasis on urban data consolidation and analysis for optimizing public transport in response to changing urban mobility dynamics. Despite the existing efforts, traffic data analysis often disregards vital situational context, including large-scale events, weather factors, traffic generation poles, social distancing norms, or traffic interdictions. Some of these sources of context data are still private, dispersed, or unavailable for the purpose of planning or managing urban mobility. Addressing the above observation, the Lisbon city Council has

Comparative Study of Tympanic Membrane Regeneration Therapy for Chronic Otitis Media.

OBJECTIVE: To examine the effectiveness of tympanic membrane regeneration therapy (TMRT) for chronic otitis media (COM). METHODS: Of 154 patients (166 ears) with COM and tympanic membrane perforation (TMP), 53 (54 ears) underwent conventional tympanoplasty (CTP), and 101 (112 ears) underwent TMRT. TMRT was performed using an endoscope to create a fresh wound around the TMP edge and remove most of the remaining TM. After high-pressure saline irrigation of the tympanic cavity and removal of the lesion, a basic fibroblast growth factor-infiltrated gelatin sponge was placed and covered with fibri

A Novel Decellularized Fibrocartilage Graft Promotes Tympanic Membrane Repair.

Tympanoplasty repairs tympanic membrane (TM) perforations using grafts. In pediatric patients, cartilage grafts are preferred over fascia due to superior mechanical properties that prevent complications from negative middle ear pressure, including graft failure, retraction, and cholesteatoma formation. However, autologous grafts cause donor-site morbidity and increased surgical time. To overcome these limitations, we engineered an allogeneic porcine meniscus decellularized (MEND) fibrocartilage graft with a unique microchannel structure created by selective elastin and vascular digestion to pr

Artificial Intelligence at the Service of the ENT: A New Perspective on Tympanic Membrane Perforations.

In this study, we present an innovative approach in medicine, leveraging the potential of artificial intelligence (AI) for the diagnosis of tympanic membrane perforations. Due to its central role in sound transmission, the tympanic membrane is a key element in clinical assessment and therapeutic decision-making. Our research focuses on the automated detection of tympanic membrane perforations using advanced AI algorithms, capable of accurately identifying whether a perforation is marginal or not. This study evaluates the performance of a convolutional neural network (CNN) optimized by a geneti

StyleGAN-based synthetic image augmentation for multi-class otoscopy image classification.

Accurate diagnosis of eardrum abnormalities is pivotal for effectively managing various ear conditions. Otoscopy, a non-invasive diagnostic procedure, provides detailed views of the ear canal and eardrum to identify pathologies such as otitis media, tympanic membrane perforations, and other abnormalities. Despite advancements, classifying otoscopy images remains challenging due to lighting variations, motion blur, and diverse abnormalities, exacerbated by limited labeled datasets and subjective assessments. A significant barrier to accurate diagnosis is the lack of an objective evaluation meth

Beyond the Tradition in Tympanoplasty: A Retrospective Cohort Study of a New Modified Palisade Tympanoplasty Technique.

BACKGROUND & AIM: Tympanic membrane perforation is a common cause of conductive hearing loss and otologic morbidities. The most common cause is trauma, which tends to heal spontaneously. Conversely, perforations caused by infections or middle ear pathology require surgical intervention, such as tympanoplasty. While cartilage tympanoplasty provides benefits of durability and resistance to retraction, concerns persist regarding graft rigidity and its adverse effects on hearing outcomes. This study was implemented to assess the anatomical and functional success of an innovative and novel form of

Sedation-Assisted, Non-sedated, and Escalated Removal of Pediatric ENT Foreign Bodies in Emergency Settings: A Systematic Review.

Pediatric ear, nose, and throat (ENT) foreign bodies are among the most common procedural challenges in emergency medicine. Substantial uncertainty exists regarding the optimal approach to removal, including sedation-assisted versus non-sedated techniques, and the appropriate thresholds for escalation to otolaryngology or the operating room. A systematic literature search was conducted in PubMed, the Cochrane Library, Web of Science, and Scopus (search date: June 2026). Studies were included if they reported outcomes of ENT foreign body removal in pediatric or predominantly pediatric emergency

Right aural myiasis in a hypertensive patient: a rare and disturbing ear, nose, and throat emergency (case report).

Aural myiasis is an uncommon parasitic infestation involving the external auditory canal by fly larvae, typically seen in tropical climates, especially among individuals with poor hygiene or immunocompromised status. Though rare, it can lead to serious complications, including tympanic membrane perforation or intracranial invasion. We describe a 55-year-old man with hypertension who had been experiencing pain and bleeding in his right ear for four days. Live maggots were found in the right external auditory canal during otoscopic examination. It was impossible to see the tympanic membrane. Sys
