Increased carotid artery intima-media thickness in patients with tympanosclerosis: Common risk factors with atherosclerosis?

We investigated the relationship between tympanosclerosis, known atherosclerotic risk factors, and the intima-media thickness of bilateral carotid arteries using ultrasonography. A total of 122 patients admitted to our clinic with chronic otitis media between 2005 and 2010 were included in the study. The study group consisted of 61 patients with tympanosclerosis; the control group comprised 61 patients without tympanosclerosis. Internal carotid artery intima-media thickness (CAIMT), total cholesterol, triglyceride, low- and high-density lipoprotein cholesterol, C-reactive protein, and homocyst

Manubrio-stapedioplasty: new surgical technique for malleus and incus fixation due to tympanosclerosis.

OBJECTIVE: This paper reports the authors' technique of manubrio-stapedioplasty using glass ionomer cement for malleus and incus fixation due to tympanosclerosis. METHODS: A retrospective case review was conducted of five patients with conductive hearing loss (mean pre-operative air-bone gap of 42.75 dB) treated in a tertiary referral centre. The hearing results of a manubrio-stapedial bone cement ossiculoplasty technique, utilised on the five patients, were analysed. All cases were Wielinga and Kerr tympanosclerosis classification type 2 (attic fixation of the malleus-incus complex with a mo

Long term complications of ventilation tube insertion in children with otitis media with effusion.

BACKGROUND/AIM: Otitis media with effusion (OME) is characterized by the prolonged presence of fluid (longer than 12 weeks) of different viscosity in the middle ear, without perforation of the eardrum or signs of acute inflammation. The conservative treatment does not always provide satisfactory recovery, so surgical treatment may be unavoidable. The aim of the study was to determine the incidence, type and frequency of complications caused by ventilation tube insertion as a part of treatment for OME in children, and specifically, to evaluate the evolution of these changes over the extended pe

Associations between peripheral vertigo and gastroesophageal reflux disease.

We hypothesize that peripheral vertigo is associated with gastroesophageal reflux disease (GERD). Two mechanisms could be considered – gastric acids may directly irritate the respiratory mucosa and cause inflammation, or Helicobacter pylori (H. pylori) could be present and cause local infection. Reflux material (Hydrochloric acid (HCl) and pepsin) could get into the middle ear via Eustachian tube and affect osseous structures directly. Disturbance of ossicles could cause tinnitus, which is more common for peripheral vertigo. H. pylori could also get in the esophagus and in the upper respirator

Surgical anatomy and pathology of the middle ear.

Middle ear surgery is strongly influenced by anatomical and functional characteristics of the middle ear. The complex anatomy means a challenge for the otosurgeon who moves between preservation or improvement of highly important functions (hearing, balance, facial motion) and eradication of diseases. Of these, perforations of the tympanic membrane, chronic otitis media, tympanosclerosis and cholesteatoma are encountered most often in clinical practice. Modern techniques for reconstruction of the ossicular chain aim for best possible hearing improvement using delicate alloplastic titanium prost

Grommets and speech at three and six years in children born with total cleft or cleft palate.

OBJECTIVE: Grommets may be considered as the treatment of choice for otitis media with effusion (OME) in children born with a cleft. But the timing and precise indications to use them are not well established. The aim of the study is to compare the results of hearing and speech controls at three and six year-old in children born with total cleft or cleft palate in the presence or not of grommets. METHODS: This retrospective study concerns non syndromic children born between 1994 and 2006 and operated for a unilateral cleft lip palate (UCLP) or a cleft palate (CP) alone, by one surgeon with th

The Efficacy of Adjuvant Intratympanic Steroid Treatment for Otitis Media with Effusion in Children.

INTRODUCTION: Otitis media with effusion (OME) is a leading cause of hearing impairment in children. Therefore, early and proper management is essential. OBJECTIVES: The objective of this research is to assess the efficacy of intratympanic (IT) steroids injection for management of otitis media with effusion (OME). METHODS: This study involved 42 children (84 ears) with bilateral OME. We used tympanometry to confirm the childreńs middle ear effusion and pure tone audiometry to determine hearing threshold. We performed myringotomy and inserted ventilation tubes (VTs) bilaterally, followed by a

Role of Chlamydia pneumoniae and Helicobacteria pylori in the development of tympanosclerosis.

The etiology of tympanosclerosis (TS) is not known, but TS commonly develops secondary to acute and chronic otitis media (COM). Since calcification process in TS resembles that of atherosclerosis (AS), pathogens that are related to pathogenesis of AS may be involved in development of TS. This prospective and controlled study, performed at a tertiary referral center, investigated a possible relationship between the presence of Chlamydia (C.) pneumoniae and Helicobacter (H.) pylori and the development of a tympanosclerotic plaque. The presence of C. pneumoniae was examined in the surgical specim

Immunohistochemical investigation of inducible nitric oxide synthase, osteopontin, and calcium-sensing receptor in a myringosclerosis/tympanosclerosis model.

HYPOTHESIS: To investigate roles of types of inflammation, inducible nitric oxide synthase (iNOS), osteopontin (OPN), and calcium sensing receptor (CaSR) in the tympanic membrane and middle ear in etiopathogenesis of myringosclerosis/tympanosclerosis (MT). BACKGROUND: Etiopathogenesis of myringosclerosis/tympanosclerosis is still unclear. Clinical and experimental observations demonstrate that hyperoxygenation might induce tympanosclerosis. METHODS: Seventy-five rats were divided into 3 groups: ventilation tube (VT) insertion, the Eustachian tube (ET) obliteration, and both procedures. Right

The bone conduction implant--first implantation, surgical and audiologic aspects.

OBJECTIVE: To report on preoperative assessment, surgery, and audiologic outcome of the first patient implanted with the bone conduction implant (BCI). BACKGROUND: The BCI is a bone conduction hearing device with an intact skin solution where the transducer is implanted close to the ear canal opening. By avoiding a percutaneous screw attachment to the skull, the BCI is anticipated to reduce complications associated with the Bone-Anchored Hearing Aid (BAHA) solution. METHODS: The first patient to receive a BCI was a 42-year-old woman with a unilateral mixed hearing loss due to tympanosclerosi

Endoscopic findings of the external ear canal in a group of clinically normal horses and horses with head shaking or vestibular disease.

Since there is a lack of information about the normal appearance or pathological findings of the equine external ear canal (EEEC) and tympanic membrane (TM), we aimed to find a practical way to perform the otoscopic examination in standing, sedated horses. Therefore, we worked with common veterinary video endoscopes, which are normally used for gastroscopy or bronchoscopy. Both ears each of 38 randomly selected, chemically restrained horses were otoscopically examined. 33 of those horses had no history or signs of potentially ear-associated diseases. However, two horses with vestibular disease

[Direct acoustic cochlear stimulation for therapy of severe to profound mixed hearing loss: Codacs™ Direct Acoustic Cochlear Implant System].

OBJECTIVE: Implantable hearing aids have become a valid option for the therapy of various forms of hearing loss. The Codacs™ Direct Acoustic Cochlear Implant System is the first vibratory implant available for patients with severe to profound mixed hearing loss (MHL). By directly coupling sound energy into the perilymph, a very high maximum power output (MPO) is achieved over a broad frequency range. Via a conventional stapedotomy, the vibratory energy of the electromagnetic actuator is transferred directly to the perilymph through the oval window. PATIENTS AND METHODS: This article describes

Diagnosis of stapedial footplate fixation in archaeological human remains.

This study analyses changes in the region of the oval window suggestive of stapedial footplate fixation in archaeological human skeletal remains. We endoscopically investigated 621 temporal bones of 385 individuals from five medieval sites in Germany to identify fixations of the stapedial footplate. For differential diagnosis, four cases suspicious of representing stapes fixation or remnants of the fixed footplate were further investigated using microscopic techniques (brightfield and darkfield imaging, phase-contrast microscopy, fluorescence microscopy, CLSM, SEM-BSE imaging), and EDX-analysi
