BACKGROUND: Active central nervous system (CNS) infection can resemble acute immune polyradiculoneuropathy, and the two processes may coexist. We describe a patient with neurosyphilis, rapidly progressive paraparetic weakness, and anti-GT1a IgG detected in cerebrospinal fluid (CSF) but not serum by a qualitative line immunoblot.
CASE PRESENTATION: A 41-year-old man developed rapidly progressive paraparesis, urinary retention, and left abducens palsy. On hospital day 2, CSF opening pressure was 260 mmH2O, protein 1.56 g/L, white-cell count 272 × 106/L, CSF TPPA was reactive, and CSF RPR was 1:

The global resurgence of syphilis, marked by a dramatic 150% increase in cases over the last decade and the rise of the dominant SS14-Ω strain cluster, represents a critical threat to public health. This review explores the complex molecular mechanisms by which Treponema pallidum functions as a "stealth pathogen," establishing suppressive chemotactic microenvironments that facilitate neuroinvasion and persistent central nervous system (CNS) damage. We delineate a biomechanical and molecular cascade wherein the spirochete utilizes its unique "naked" outer membrane and hypervariable Treponema pa

OBJECTIVE: Serrated polyposis syndrome (SPS) is the most common form of colorectal polyposis syndrome. Most SPS cases are sporadic, and the environmental factors underlying its pathogenesis remain poorly defined. Intestinal spirochaetosis (IS), an infection of the colonic epithelium by Brachyspira species, is frequently asymptomatic and its clinical relevance is uncertain. Here, we examined the potential association between IS and SPS.
METHODS: In this retrospective case-control study at two centres in Japan, IS prevalence was assessed in 64 patients with SPS and 382 non-SPS controls who unde

TORCH infections (toxoplasmosis, other [syphilis, varicella-zoster, parvovirus B19], rubella, cytomegalovirus, herpes) constitute the classic group of congenital infections that may result in severe fetal and neonatal consequences. Transplacentally transmitted infections have been a critical component of prenatal care for decades. Although screening for this group of infections has been performed in Hungary since 1971, diagnostic approaches and screening strategies have undergone significant transformation. Current international recommendations, as well as national professional guidelines, no

This study reports the detection of a relapsing fever Borrelia in the gilded boar tick, Dermacentor auratus, extracted from the ear canal of a human patient in Kedah, Peninsular Malaysia. Molecular analysis of the flaB gene identified the Borrelia species as identical to Borrelia sp. isolate HNF1F2 from China (ON060668) and closely related (99.60%) to Borrelia theileri isolate B22 from Colombia (ON135433). This article also provides a comprehensive review of human otoacariasis cases in Malaysia and discusses the limitations associated with these cases.

Seabirds are hosts of several tick species, but there is little information on their infestation by soft ticks and pathogens they harbour is available. In this study, we evaluated soft tick infestation in gull chicks from two breeding colonies in Portugal and assessed the prevalence of anti-Borrelia antibody in adults gull and shearwaters from these colonies, as well as from two additional urban colonies. The prevalence of infestation by Ornithodorus maritimus ticks in gull chicks varied between (17.6 %) in three yellow legged gull (Larus michahellis) and 84.8 % in the Audouin's gull (Ichthyae

Monitoring programs that track natural fluctuations in tick activity, human exposure, and disease incidence are limited in their ability to detect shifts in tick-borne disease (TBD) risk. We evaluated an integrated approach combining field-based tick surveillance, Google search trends, and national Lyme neuroborreliosis (LNB) records in Denmark from 2017-2024. Tick nymph activity was modelled using meteorological data from six forest sites and validated against independent 2024-2025 data. The model showed strong predictive performance (Pearson's r = 0.76, normalised root-mean-square error = 0.

Borrelia miyamotoi is an emerging human tick-borne pathogen that has been confirmed in multiple tick species, including Ixodes ricinus and Dermacentor reticulatus. In 2022–2024, 2263 ticks removed from human skin were collected for research, mainly of two species: I. ricinus (2123) and D. reticulatus (137). Detection of B. miyamotoi was performed by PCR, amplifying a fragment of the p66 and gplQ genes, confirmed by sequencing. In total, the pathogen prevalence in human-derived ticks was 3.0%. Among 63 positive I. ricinus (3.0%), B. miyamotoi was detected in females and nymphs with prevalence 3

OBJECTIVES: Bovine digital dermatitis (BDD) is a globally endemic, painful infectious foot disease of cattle; however, gaps in knowledge regarding treatment efficacy limit our ability to control it. Our objectives were to collect data on minimum inhibitory concentrations and minimum bactericidal concentrations (MICs and MBCs) for conventional and naturally derived therapeutic agents against specific Treponema strains important in BDD pathogenesis in the context of the need for responsible use of antimicrobials. Since antibacterial metals are widely used to control infectious lameness in rumina

Three Treponema sp.-T. medium, T. pedis, and T. phagedenis-are associated with bovine digital dermatitis (BDD). This painful infectious disease has been reported worldwide, causing economic and animal welfare problems. Although detection by real-time PCR (rtPCR) is the method of choice, published assays differ in specificity and sensitivity. We multiplexed target regions of 2 published rtPCR assays and slightly optimized another one to compare their performance. We then used the 2 novel TaqMan rtPCR assays to determine the prevalence of the 3 Treponema sp. in a random sample of hind feet from

BACKGROUND: According to European Academy of Neurology guidelines, a positive Borrelia burgdorferi antibody index is required for diagnosing definite Lyme neuroborreliosis. Exceptions to the typical antibody production may be seen in the earlier phases of Lyme neuroborreliosis and in immunocompromised patients with Lyme neuroborreliosis, which can present diagnostic challenges.
CASE PRESENTATIONS: We present four Norwegian immunocompetent patients (three male patients aged 52, 61 and 64 years old and one female patient aged 52 years) with neurological symptoms typical of Lyme neuroborreliosis

PURPOSE: Neuroborreliosis is a common cause of peripheral facial palsy (PFP) in children and is traditionally diagnosed with lumbar puncture. While serum Borrelia burgdorferi (Bb) antibodies can reduce the need for lumbar puncture, their use is limited by processing time. Prompt results for Bb antibodies may accelerate clinical decisions. We aimed to evaluate the diagnostic accuracy for neuroborreliosis of a point-of-care lateral flow assay for Bb IgG and IgM in children with PFP.
METHODS: Between October 17, 2019, and November 27, 2023, serum samples from children with PFP were collected acr

Lyme neuroborreliosis (LNB), a disseminated manifestation of Lyme borreliosis (LB), arises when Borrelia burgdorferi sensu lato (Bbsl) spirochetes disseminate within the host and damage the peripheral nervous system and meninges, and in rare cases, also the parenchyma of the central nervous system (CNS). While early-stage LB is diagnosed clinically, accurate diagnosis of LNB requires cerebrospinal fluid (CSF) analysis, demonstrating pleocytosis and intrathecal synthesis of Bbsl-specific antibodies. There are, however, limitations in current LNB diagnostics, such as the unspecific nature of ple

Bovine digital dermatitis (BDD) is an infectious disease in cattle associated with multiple Treponema species yet information on the immune responses of circulating immune cells to these pathogens are limited. In this study, we characterized the transcriptomic responses of bovine peripheral blood mononuclear cells (PBMCs) following stimulation with combined whole cell lysates derived from three BDD-associated Treponema species. PBMCs were stimulated for 12 h and differentially expressed genes between non-stimulated control and stimulated groups were identified. Functional enrichment of DEGs wa

Alzheimer's disease (AD) is a progressive neurodegenerative condition marked by the accumulation of amyloid-β (Aβ) plaques, tau hyperphosphorylation, and persistent neuroinflammation. Emerging research indicates that Treponema denticola (T. denticola), a prominent periodontal pathogen, plays a role in AD development through various neuroinvasive mechanisms. This systematic review synthesizes current evidence regarding T. denticola ability to penetrate the central nervous system via trigeminal and hematogenous routes, disrupt the blood-brain barrier, and create enduring biofilms within neural t
