Borrelia Infections Under B Cell-Depleting Therapies: A Systematic Review of Diagnostic Challenges and Outcomes With Special Focus on Neurological Forms.

CONTEXT: B-cell-depleting therapies such as rituximab and newer anti-CD20 agents may impair humoral immune responses and reduce the reliability of serological testing. This systematic review aims to summarize reported cases of Lyme borreliosis and relapsing fever (RF) Borrelia infections in patients receiving B-cell-depleting therapies, focusing on clinical manifestations, diagnostic challenges, and treatment outcomes. METHODS: A systematic literature search was conducted using PubMed and the Web of Science Core Collection employing search terms linking B-cell-depleting therapies to Lyme borr

Xenodiagnosis to search for Borrelia burgdorferi after antibiotic treatment of Lyme disease: a prospective cohort study.

BACKGROUND: Some patients report non-specific symptoms after antibiotic therapy for Lyme disease (LD), raising questions about ongoing infection, despite no compelling evidence. We investigated whether xenodiagnosis could detect Borrelia burgdorferi in such patients, and if positive results correlated with symptoms. METHODS: Participants were adults who completed antibiotic treatment for LD 3-12 months earlier (post-therapy, n=40) or had persistent symptoms for ≥ 12 months after treatment, (post-treatment LD symptoms [PTLDS], n=20). Controls included one patient with erythema migrans (EM), on

Borrelia burgdorferi-Induced Neuroinflammation in Lyme Disease: A Potential Driver of Alzheimer's Disease Pathology?

Emerging evidence suggests that chronic infections may contribute to neurodegenerative diseases such as Alzheimer's disease (AD). One such infection is caused by Borrelia burgdorferi sensu lato (Bbsl), the spirochete complex responsible for Lyme disease, which can invade the central nervous system (CNS) and trigger Lyme neuroborreliosis (LNB). Bbsl infection is associated with persistent neuroinflammatory responses and immune evasion mechanisms, which may contribute to long-term neurological sequelae in a subset of patients. Neuroinflammation is increasingly recognized as a contributing factor

Long-term Cardiac Sequelae in Lyme Carditis: A Review.

INTRODUCTION: Lyme disease, primarily caused by Borrelia burgdorferi in North America, is a common spirochetal infection transmitted via tick bites. The dissemination of Lyme spirochetes can result in multisystem complications, including Lyme carditis. METHODS: This review incorporated data from observational studies, retrospective analyses, systematic reviews, and case reports involving patients with confirmed or presumed Lyme carditis. Key outcomes included prevalence, reversibility, and chronicity of cardiac involvement, as well as diagnostic and prognostic implications. RESULTS: Atrioven

Transcriptomic response to Borrelia afzelii infection in the skin of wild bank voles.

Bank voles are one of the main reservoirs of tick-transmitted spirochete Borrelia afzelii, a causative agent of Lyme disease in humans in Europe. How the immune system deals with infection at the site of entry, that is, the skin, has not been explored in this species. Here, we used RNA sequencing to explore the transcriptomic response in the ear skin of wild bank voles infected with B. afzelii. We identified 54 differentially expressed genes, of which 37 showed upregulation, and 17 showed downregulation in infected voles compared to uninfected ones. Weighted gene co-expression network analysis

Lyme Carditis Identified by a Novel Syndromic Surveillance Definition, New York State, 2017-2021.

OBJECTIVES: Lyme carditis is a rare, potentially fatal manifestation of Lyme disease. Although Lyme disease is nationally notifiable, data on clinical manifestations are not collected systematically in the United States. We developed a syndromic surveillance definition to identify patients with Lyme carditis in New York State during 2017-2021. METHODS: We developed a definition of Lyme carditis by using diagnostic codes and keywords to identify emergency department visits related to Lyme carditis through the National Syndromic Surveillance Program's BioSense Platform. We systematically abstra

Automated Enrichment of DNA Biomarkers from Large-Volume Samples: Detection of B. burgdorferi Cell-Free DNA in Urine.

Liquid biopsies enable rapid and noninvasive analysis for disease biomarkers. However, in the early stages of a disease, biomarkers are often present in extremely dilute concentrations, making them difficult to isolate and quantify. We have developed an Automated Magnetically and Thermally Induced Conical Separator (Auto-MATICS), which enables purification and enrichment of trace biomarkers from large volume biological samples (up to 15 mL) using magnetic beads and thermally responsive alkane partitions (TRAPs). Here, we quantify the enrichment capabilities of the Auto-MATICS and demonstrate a

Protective properties of a candidate C-terminal domain OspA vaccine for prevention of Lyme disease.

Lyme disease is a growing public health concern that is geographically focused in regions where ticks that carry the causative bacteria, Borrelia burgdorferi sensu lato (s.l.), are endemic. Outer surface protein A (OspA) is expressed by B. burgdorferi s.l. spirochetes during the tick phase and OspA antibodies introduced during tick feeding can block transmission and prevent B. burgdorferi infection. Candidate Lyme disease vaccine VLA15 is comprised of the C-terminal domains of the six B. burgdorferi s.l. OspA serotypes (ST) prevalent in North America and Europe. We report herein that non-human

Geographic variation in risk of blacklegged tick-borne coinfections in the eastern United States.

Blacklegged tick (Ixodes scapularis)-associated diseases are increasing in incidence and geographic range in the United States. Pathogen range expansion may increase the risk of coinfections, which could complicate accurate diagnosis and treatment of tick-borne diseases. To identify regions of coinfection risk across the eastern United States, we used a national database of tick-borne pathogen occurrence to identify areas of co-occurrence between four I. scapularis-borne human pathogens: Anaplasma phagocytophilum human-active variant (Apha; anaplasmosis), Babesia microti (Bam; babesiosis), Bor

Streamlining Borrelia burgdorferi cultivation using quantitative PCR screening.

Introduction. Direct detection of Borrelia burgdorferi by culture is considered the gold standard for confirming Lyme disease (LD). However, B. burgdorferi culture is not routinely used in clinical practice or research due to its lengthy protocol and low success rate. This study aimed to streamline the process by integrating a specific quantitative PCR (qPCR) screening early into the B. burgdorferi culture workflow for identification of cultures that are likely to yield viable spirochetes.Methods. Thirty-two blood plasma and 11 cerebrospinal fluid (CSF) samples were collected from 32 children

Telomere-to-telomere assembly detects genomic diversity in Canadian strains of Borrelia burgdorferi.

Borrelia burgdorferi, the bacteria causing Lyme disease, has a complex genome comprising a linear chromosome and multiple linear and circular plasmids. The atypical hairpin telomeres and the highly paralogous plasmids complicate genome assembly. We develop a genome assembly pipeline using both long and short reads to overcome these challenges. Using long reads, we assemble the hairpin telomeres of the linear replicons, an lp28-1a plasmid subtype, and circular plasmids of nine B. burgdorferi strains from five regions across Northwest Ontario and Manitoba, Canada. Although similar across the cor

Sex and menopause-based differences in presentation of early Lyme disease: A prospective cohort study.

Although prior research has established sex and menopausal status-based differences in immune response, susceptibility, and severity to a variety of pathogens, their relevance in early Lyme disease is understudied. We examined the clinical and serologic presentation of patients with early Lyme disease, stratified first by sex then by menopausal status. We also explored the hypothesis that males would present with more severe early Lyme disease. In this prospective cohort study from the Mid-Atlantic US, 243 adult, antibiotic-naïve patients were enrolled with a diagnostic erythema migrans rash p

Borrelia mayonii induces carditis but not arthritis in Lyme-susceptible mice.

BACKGROUND: Borrelia mayonii, a Borrelia burgdorferi sensu lato (Bbsl) genospecies, is a cause of Lyme disease in the upper midwestern United States. B. mayonii infection can present with some atypical clinical manifestations, including unusually high spirochetemia. Previous studies have demonstrated the infectivity of B. mayonii in laboratory mice and found evidence of widespread dissemination to internal tissues, however, did not report evidence of high spirochetemia. In the current study, we sought to directly compare spirochetemia as well as typical Lyme disease pathology including arthrit

The influence of Borrelia burgdorferi and Anaplasma phagocytophilum bacteria on phospholipid metabolism in the blood of patients.

Ticks carry various pathogens, including the spirochete Borrelia burgdorferi, which causes Lyme disease (LD) in humans, and the Gram-negative bacterium Anaplasma phagocytophilum, which causes human granulocytic anaplasmosis (HGA). These diseases pose both diagnostic and therapeutic challenges. It is known that tick-borne diseases are typically accompanied by oxidative stress and inflammation, and consequently, other metabolic changes. Therefore, the aim of the study was to evaluate the modification of ROS- and enzyme-dependent metabolism of phospholipids and free polyunsaturated fatty acids (P

Seroprevalence and seroconversion of Lyme borreliosis among tick-bitten individuals: A multi-assay serosurveillance study.

Lyme borreliosis (LB), caused by different species belonging to the Borrelia burgdorferi sensu lato group, is the most common tick-borne disease in Europe. However, its true burden remains difficult to assess, partly due to the diagnostic variability of commercial assays. This study evaluated seroprevalence and seroconversion following tick bite, using four serological assays with distinct antigenic compositions. Participants provided blood samples within three days of the tick bite and three months after to assess seroconversion. We evaluated the seroprevalence, seroconversion, inter-assay va

Survey of tick control practices on public lands across 4 states reporting high incidence of Lyme disease.

Ticks and tick-borne diseases pose a significant public health threat in the United States, particularly in the Northeast and Upper Midwest. Tick control operations are predominately focused on private residential properties. Surveys of publicly funded vector control programs have indicated that high use public lands may be viable targets for future tick control activities if funding is available. However, little is known about the feasibility of implementing tick control activities on these properties. We administered a survey to understand current tick control and tick bite prevention educat

Citizen Science Tick Observations Serve as an Early Warning System for Tick-Borne Diseases.

INTRODUCTION: Tick observation data collected through citizen science is increasingly utilised to map tick-borne infection risk areas indirectly, that is, based on the rate of tick encounters or occurrence of ticks. However, direct associations between tick observations and Lyme borreliosis (LB) cases have received little attention. In the current study, associations between weekly tick observations and LB cases were studied on a nationwide scale in Finland, in order to determine if tick observations precede cases in a predictable manner, and whether tick observations could be used to predict

Strain-specific immune response patterns to Borrelia burgdorferi infection: a comparative transcriptomic analysis in C3H and C57BL/6 mice.

Borrelia burgdorferi (Bb), transmitted through tick vectors, induces Lyme arthritis (LA), with disease progression intimately correlated with host genetic characteristics. Laboratory investigations have demonstrated marked disparities in infection responses among distinct mouse strains: C57BL/6 mice have mild arthritis and rapid tissue repair, whereas C3H mice exhibit severe arthritic manifestations. Comparing these strains has helped identify genetic and immune factors important for arthritis development. In this study, female C57BL/6 and C3H mice were inoculated with Bb via bilateral footpad

Clinical data on symptomatic cutaneous reinfections due to Borrelia burgdorferi sensu lato in Slovenia.

BACKGROUND: Symptomatic reinfections with Borrelia burgdorferi sensu lato occur, but their clinical and microbiologic characteristics are insufficiently defined. It, therefore, remains unclear whether having had a primary infection influences the presentation or course of reinfection. METHODS: We compared the clinical and microbiologic findings in patients with primary infection versus reinfection, with each episode presenting with erythema migrans (EM). The analysis included a cohort of patients diagnosed with EM between 1990 and 2014 at an outpatient clinic in Slovenia. RESULTS: A total of
