Peridomestic and Neighborhood-Level Deciduous Land Cover Associated with Lyme Disease Risk: Results from a Case-Control Study in the United States.

INTRODUCTION: Lyme disease (LD), the most common vector-borne disease in the United States, is caused by the transmission of bacteria via the bite of infected Ixodes ticks including Ixodes scapularis. Deciduous forests are a highly suitable habitat for I. scapularis survival and development. This case-control study investigated how LD risk was impacted by residential proximity to deciduous forest land cover. MATERIALS AND METHODS: Participants were selected from the Optum Research Database and mailed a one-time survey regarding LD history, outdoor activities, residential characteristics, and

Ticks in the Tropics: Challenging the Myth of a Lyme-Less Florida.

OBJECTIVES: Few analyses have differentiated locally acquired Lyme disease infection from travel-associated infection, limiting the ability to interpret whether case counts reflect true in-state transmission or increased detection among mobile populations. We analyzed and compared trends in Lyme disease incidence at the national level and in the state of Florida. METHODS: We primarily used Lyme disease surveillance data from the Centers for Disease Control and Prevention from 2010 through 2023. The data included total case numbers, demographic characteristics, and county-level distribution ma

Lyme borreliosis.

Lyme borreliosis is the most common tick-borne disease in the northern hemisphere. It is a zoonosis caused by several species of Borrelia burgdorferi sensu lato and transmitted by the bite of infected ticks of the Ixodes ricinus complex. Lyme borreliosis in North America and Europe differs in certain respects, likely reflecting the different Borrelia species that cause human disease in these locations. The earliest manifestation of Lyme borreliosis is the skin lesion erythema migrans, which develops at the tick bite site, typically 7-14 days after the bite. Some untreated patients will then (w

Ecoepidemiological determinants of Borrelia infection in sigmodontine rodents from the Delta and Parana Islands ecoregion, Argentina.

We screened lung tissue from 770 sigmodontine rodents sampled in the Delta and Paraná Islands ecoregion for Borrelia spp. and obtained sequences clustered within the Borrelia burgdorferi sensu lato (s.l.) complex. Seventeen individuals were PCR-positive (prevalence 2.2%), distributed mainly in Akodon azarae (n = 11) and across five additional species. Using GLMMs that accounted for site and trapping session, infection probability was positively associated with individual body length and with ecological proxies of prior host and vector abundance: Ak. azarae abundance two sessions earlier (lag S

[Lyme carditis].

BACKGROUND: Lyme carditis is a rare manifestation of systemic Lyme disease, typically occurring one to two months after infection. The most common feature is conduction system disturbance, often involving the atrioventricular (AV) node. CASE PRESENTATION: A young woman was admitted to the emergency department after experiencing palpitations. She was haemodynamically stable and had no respiratory distress. An ECG revealed an atrioventricular block with a PR interval of 380 ms. Her medical history included a recent tick bite. Potential causes of conduction disturbances were ruled out and blood

Clinical Manifestations of Lyme Borreliosis in Europe: Burden of Lyme Disease Study (BOLD), 2021-2022.

Lyme borreliosis (LB), the most common European tick-borne disease, can manifest as an erythema migrans (EM) rash or as disseminated LB. The prospective Burden of Lyme Disease (BOLD) study evaluated the frequency of LB clinical manifestations, including signs, symptoms, and treatment patterns in 14 healthcare practices in endemic regions of six European countries: the Czech Republic, Germany, Poland, Slovakia, Slovenia, and Sweden. Between April 2021 and December 2022, patients with suspected LB were evaluated using predefined case definitions that were applied by investigators to identify med

A Bayesian modelling framework for estimating tick-borne pathogen transmission dynamics at the host-tick interface.

Understanding the transmission dynamics of tick-borne pathogens at the host-tick interface is challenged by the presence of multiple pathways for tick infection, including (i) host-to-tick transmission, (ii) tick-to-tick (cofeeding) transmission, and (iii) pre-existing infection through vertical transmission or prior feeding. Assessing parameters governing these pathways is critical for identifying the main transmission drivers and, consequently, key prevention and control points. Here, we developed a Bayesian modelling framework that estimates key parameters describing the probability of each

The Role of Borrelia Burgdorferi in the Etiology of Chronic Urticaria.

BACKGROUND: Lyme borreliosis/European borreliosis (LB/EB) is a multisystemic infection caused by the bacterium Borrelia burgdorferi, which is transmitted to humans by the bite of ticks and other hematophagous insects. Transmitted borrelia can occupy any organ where it causes a silent inflammation, which in sensitive persons is intermittently repeated chronically. OBJECTIVE: The aim of this paper was to determine how much Borrelia burgdorferii participates in the occurrence of chronic urticaria in children and adults. METHODS: In the 13-year period from January 1, 2013 to December 31, 2025, a

Lyme Neuroborreliosis With Acute Encephalopathy Despite Early Antibiotic Therapy: A Case Report.

BACKGROUND Lyme disease is a tick-borne infection caused by spirochetes of the Borrelia burgdorferi sensu lato species complex (Bb). Lyme neuroborreliosis occurs in up to 15% of untreated Lyme disease cases and most commonly presents with painful radiculitis, cranial nerve palsy, and meningitis; progression to encephalitis occurs in approximately 3.3% to 9% of cases. Lyme neuroborreliosis can develop despite appropriate antibiotic therapy of early Lyme disease. Diagnosis of Lyme neuroborreliosis is based on a combination of compatible neurological symptoms, serologic evidence of Lyme disease,

Geographical dataset of questing nymphal density for three key tick vector species of Lyme disease.

Nymphal ticks of Ixodes pacificus, Ixodes scapularis, Ixodes ricinus, and Ixodes persulcatus are primary vectors of Lyme disease, which affects both animal and human health. Understanding their population dynamic is therefore critical for public health risk assessment. Here we present a focused dataset of questing nymph density, comprising 3579 records from 136 publications from 1980 to 2024. The dataset includes three tick species that represent major Lyme disease risk in the Northern Hemisphere. Records were compiled through systematic literature review and preserved original density measure

Making Invisible Illnesses Visible: Recognizing and Responding to Infection-Associated Chronic Conditions.

The emergence of post-COVID conditions (PCCs) has renewed attention to infection-associated chronic conditions and illnesses (IACCIs), including myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) and Lyme disease-associated chronic symptoms. Millions of Americans are affected by these debilitating, misunderstood conditions, which share symptom profiles and pathophysiologic abnormalities. IACCIs have received insufficient clinical attention and research investment. We outline elements of a patient-centered approach to care, emphasizing validation of patients' experiences, multidiscipli

Use of administrative data for evaluating trends in medically-attended Lyme disease, Manitoba, Canada, 2010-2021.

While Lyme disease (LD) is the most common tick-borne disease reported to public health surveillance in North America and is increasingly recognized as a public health threat in Canada, it's incidence may be underreported. Our study aims to estimate medically-attended LD incidence in Manitoba, Canada, using administrative health data. We identified medically-attended LD cases in Manitoba from 2010 - 2021 in a claims database (Manitoba Population Research Data Repository), which contains the health records of >95% of the residents of Manitoba, using diagnostic codes, antibiotic dispensations, a

Lyme disease in Poland in 2023.

BACKGROUND: Lyme disease is a vector-borne disease transmitted by ticks of the Ixodes genus, caused by a heterogeneous group of Borrelia burgdorferi sensu lato. Human infection occurs through a tick bite and its feeding on the skin. Due to the diverse symptoms, diagnosing Lyme disease is challenging and usually involves a two-step serological approach. Currently, there is still no vaccine against Lyme disease, but in 2024, data on the immunogenicity and safety of the VLA15-221 vaccine, which is in the second phase of clinical trials, was announced. OBJECTIVE: The aim of the study is to discus

From forest floor to doctor's office: the immunological journey of Borrelia burgdorferi through vertebrate hosts.

Lyme disease, caused by the spirochete Borrelia burgdorferi, is the most prevalent vector-borne infection in the Northern Hemisphere and continues to expand geographically. Although B. burgdorferi has a streamlined genome and minimal virulence repertoire, it establishes persistent infection through coordinated modulation of mammalian host immune responses. Here, we synthesize recent advances in the immunobiology of B. burgdorferi using a stage-structured framework that traces a tick-mediated vertebrate infection through systemic dissemination, tissue colonization, and chronic immune engagement

Innate immune responses to Borrelia burgdorferi during tick-feeding: mechanistic insights relevant to Lyme disease.

Immune responses to tick-transmitted Borrelia burgdorferi (Bb) have not been characterized in vivo. We analyzed the reservoir host's local and systemic immune responses to Bb during tick feeding. C3H/HeN mice were challenged via infected or uninfected nymphal Ixodes scapularis ticks or by subcutaneous injection of cultured multi-strain Bb. Skin and spleen tissues were evaluated by flow cytometry, serum was evaluated by cytokine proteome array, and all data were analyzed by comparison between each of the three challenged groups against the subcutaneously inoculated PBS control. Flow cytometry p

Coinfection ecology and pathogen emergence in a Borrelia-endemic landscape: 5 years of Borrelia burgdorferi, Anaplasma phagocytophilum, and Babesia microti surveillance in Maryland.

The emergence of tick-borne pathogens depends on ecological opportunity and barriers to persistence within vectors and hosts. Borrelia burgdorferi is well established in the mid-Atlantic, whereas Babesia microti and Anaplasma phagocytophilum remain patchily distributed. Five years of integrated surveillance (2020-2024) at three Maryland sites allowed us to track B. microti and A. phagocytophilum establishment by screening questing Ixodes scapularis nymphs, Peromyscus leucopus-fed nymphs, and P. leucopus samples by qPCR, contextualized with county-level human case data. B. burgdorferi was consi

Spatial modeling of Borrelia genospecies in human-biting ticks from the French citizen science programme CiTIQUE.

In Europe, Lyme borreliosis is the most common vector-borne human disease, mainly caused by Borrelia afzelii and Borrelia garinii, two species of the Borrelia burgdorferi sensu lato (Bbsl) complex transmitted by Ixodes ricinus. Assessing the spatial risk of human exposure to these pathogens is essential for efficient public health surveillance, yet conventional monitoring often fails to produce geographically explicit, large-scale data capturing exposure heterogeneity and its drivers. Focusing on continental France, we used data from the CiTIQUE citizen science programme to analyse spatial var

Evaluation of standard and modified two-tiered testing algorithms using well-characterized early Lyme disease samples.

UNLABELLED: Current laboratory testing for Lyme disease (LD) relies on serology. We evaluated the performance of standard two-tiered testing (STTT) and modified two-tiered testing (MTTT) algorithms using samples obtained from well-characterized patients with early LD in the U.S. East Coast and Upper Midwest. Participants with signs and symptoms of early LD (cases) and controls were enrolled by Lyme Disease Biobank. We compared the performance of four FDA-cleared STTT or MTTT algorithms using serum samples from 251 participants (107 cases, 69 with a convalescent draw; 144 endemic controls). At

OspA antibodies inhibit the in vitro transmigration of Borrelia burgdorferi.

Antibodies against Outer surface protein A (OspA) block tick-to-mammal transmission of the Lyme disease spirochete, Borrelia burgdorferi. Evidence suggests that antibodies ingested by a tick during a blood meal entrap OspA-expressing B. burgdorferi within the tick midgut and prevent spirochete migration to the salivary gland and host dermis. In this study, we employed a recently described Transwell system to quantitate the impact of OspA antibodies on B. burgdorferi transmigration. We demonstrate that spirochete movement from the lower Transwell chamber to the upper chamber over a 20-h period

Broadly conserved protective epitopes on the lyme disease vaccine antigen, OspA.

Lyme disease, caused by the spirochete, Borrelia burgdorferi sensu latu (Bbsl), is a tickborne infection of increasing incidence in North America, Europe and Asia. While vaccines based on Outer surface protein A (OspA) have proven highly efficacious at blocking Bbsl tick-to-human transmission, the high degree of antigenic variability among the major OspA serotypes (ST) has made the development of a broadly cross protective vaccine difficult. Recent profiling of protective human monoclonal antibodies (mAbs) has suggested the existence of conserved epitopes situated within OspA's central β-sheet
