Heat-shock pathway activation by TRC051384 protects spiral ganglion neurons from noise-induced hearing loss.

Noise-induced hearing loss (NIHL) is a major public health problem caused by damage to cochlear hair cells, synapses, and spiral ganglion neurons (SGNs). Since effective treatments are lacking, we investigated cellular stress responses induced by moderate and loud noise in a mouse model of cochlear synaptopathy. RNA sequencing and spatial transcriptomics revealed that noise exposure elicited a robust but transient upregulation of endoplasmic reticulum chaperones and proteasome subunits in SGNs and their supporting cells. To target this response, we administered TRC051384, a small-molecule acti

Oxaliplatin-chemotherapy induced cochlear synaptopathy.

Oxaliplatin is widely used as a first-line and reference anticancer agent for advanced colorectal cancer. However, it frequently causes peripheral neuropathy. It is generally accepted that oxaliplatin is significantly less ototoxic than the other platinum-based antineoplastic drugs, cisplatin and carboplatin. This study aimed to evaluate potential hearing damage induced by oxaliplatin in young male CBA/JRj mice. Mice received intraperitoneal injections of oxaliplatin (3 mg/kg) twice weekly for 3 weeks, for a cumulative dose of 18 mg/kg. Auditory function was assessed using electrophysiological

Aminoglycoside injury, but not genetic hair cell ablation, preferentially reduces Type Ic spiral ganglion neurons.

Spiral ganglion neurons (SGNs) are molecularly heterogeneous, and low-spontaneous-rate (Type Ic) SGNs are considered important for suprathreshold auditory processing, including encoding sounds in noisy environments. Preferential vulnerability of Type Ic SGNs has been reported in conditions in which primary neural injury is thought to play a major role, such as aging and noise exposure. However, it remains unclear whether preferential Type Ic vulnerability is shared across cochlear injury paradigms or varies with the nature of injury. Here, we investigated subtype-specific patterns of SGN degen

Aripiprazole as second-line pharmacotherapy for a heterogeneous cohort of refractory functional dizziness disorders: a pilot preliminary retrospective study.

BACKGROUND AND OBJECTIVE: First-line serotonergic antidepressants achieve clinical response in approximately 60-65% of patients with persistent postural-perceptual dizziness (PPPD), leaving a substantial refractory population. Aripiprazole, a serotonin-dopamine modulator with partial D2 and 5-HT1A agonism and 5-HT2A antagonism, may address dopaminergic and emotional dysregulation insufficiently controlled by serotonergic agents alone. This study evaluated the preliminary efficacy and tolerability of aripiprazole as second-line pharmacotherapy in patients with refractory functional dizziness di

Temporal enhancement patterns on 3D-FLAIR MRI in labyrinthine infarction.

BACKGROUND: Inner ear enhancement on contrast-enhanced three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) MRI reflects disruption of the blood-labyrinth barrier (BLB) and has been reported in several audiovestibular disorders. However, the temporal enhancement pattern in labyrinthine infarction remains poorly characterized. METHODS: We investigated time-dependent enhancement patterns on 3D-FLAIR MRI in four patients with labyrinthine infarction secondary to anterior inferior cerebellar artery infarction. All patients underwent comprehensive neuro-otological evaluation and contra

Bilateral hearing levels in Meniere's disease may fluctuate simultaneously.

BACKGROUND: We observe fluctuations in hearing thresholds in both ears of patients with unilateral Meniere's disease (MD), even during early stages. If hearing fluctuations on both sides are synchronized, this may be related to the pathophysiology of MD. OBJECTIVES: To examine whether hearing fluctuations in the affected and non-affected ears occur synchronously in patients with MD. METHODS: We retrospectively analyzed 2985 pure-tone audiometry tests performed in 217 patients with definite MD. Based on pure-tone audiometry test results, we calculated average thresholds for low-, middle-, and

Comorbidities and impact on quality of life in patients with Ménière's disease: a single-center cross-sectional study.

BACKGROUND: Ménière's disease (MD) is a chronic vestibular disorder that significantly impacts patients' quality of life (QoL). However, the influence of comorbid conditions on QoL in MD patients remains inadequately explored. OBJECTIVE: To investigate the prevalence of comorbidities in patients with MD and analyze their impact on patients' QoL. METHODS: A cross-sectional analysis was conducted on 473 MD patients who received treatment in our hospital. Demographic data, comorbidity information, psychological symptoms (assessed by Hospital Anxiety and Depression Scale, HAD) and QoL (evaluated

[Asymmetric low-frequency hearing loss as possible cause of pressure sensation in Menière's disease].

OBJECTIVE: The unilateral, low-frequency hearing loss typical of Menière's disease (MD) is often accompanied by a sensation of pressure in the ear. The pathophysiological mechanism remains unclear. The aim of this exploratory study was to investigate whether unilateral low-frequency hearing loss leads to the onset of a sensation of pressure and thus constitutes a possible mechanism for this symptom. MATERIAL AND METHODS: To simulate Menière's disease-typical hearing loss, 20 normal hearing subjects were presented binaurally with sentences of the Oldenburg Sentence Test (OLSA) with unilateral

The vestibular system: Unsteadiness, dizziness, and vertigo.

This chapter provides a detailed overview of the vestibular system's anatomy, physiology, and clinical relevance, emphasizing how inner-ear sensory structures and brainstem pathways interact to maintain balance, gaze stability, and spatial orientation. It begins with the peripheral vestibular apparatus, describing the semicircular canals and otolith organs in detecting angular and linear accelerations. These signals are transmitted via the vestibulocochlear nerve to the vestibular nuclei at the pontomedullary junction, where they integrate with visual and somatosensory inputs to drive coordina

Is the magnetic resonance imaging contrast agent gadoteric acid ototoxic?

OBJECTIVE: Contrast Magnetic Resonance Imaging (MRI) reveals endolymphatic hydrops in both symptomatic and asymptomatic ears. Radiological visualization of hydrops is important in the diagnosis of Meniere's disease; however, the ototoxicity of intratympanically administered gadoteric acid remains unknown. To investigate the ototoxicity of the intratympanic administration of gadoteric acid at different doses using an Auditory Brainstem Response (ABR) test and histopathological methods. METHODS: Forty healthy male Wistar Albino rats were randomly divided into four groups. The effect of differen

Endolymphatic hydrops in patients with migraine: a correlative study.

OBJECTIVE: Endolymphatic hydrops (EH), Menière's disease (MD) pathological hallmark, is visualized via 3 T delayed phase MRI. This study examines MRI correlations between EH severity and migraine brain biomarkers. METHODS: Retrospective review of 144 inner ear MRI scans yielded 108 patients with confirmed EH. Two blinded radiologists graded EH, assessed migraine markers (white matter lesions (WML), frontal predominance of WML, enlarged perivascular spaces), and measured cranial nerve volumes. Binary logistic regression tested associations. RESULTS: Mean age 53.6 ± 15.8 years (56.5% female).

Cochlear Functional Changes and CGRP Modulation in a Chronic Nitroglycerin Migraine-Like Rat Model.

Auditory and vestibular symptoms in vestibular migraine, cochlear migraine, and Ménière's disease overlap, yet mechanisms linking migraine biology to inner ear dysfunction remain unclear. Calcitonin gene-related peptide (CGRP) is a key mediator of trigeminovascular activation and a major therapeutic target in migraine, but its contribution to cochlear dysfunction is not well defined. Using a repeated nitroglycerin (NTG) paradigm in male Wistar rats, we investigated whether a systemic migraine-like state affects cochlear function and whether these effects are dependent on CGRP. Rats received sa

Home-based video-oculography during spontaneous vertigo attacks: A proof-of-concept feasibility study in Ménière's disease and vestibular migraine.

BackgroundEpisodic vestibular disorders like Ménière's disease (MD) and vestibular migraine (VM) pose diagnostic challenges because patients are often assessed between attacks, when clinical signs are absent. Recording ictal eye movements could provide objective phenotypic data; however, the practicality of using unsupervised home-based video-oculography in everyday clinical settings remains poorly studied.MethodsWe conducted a prospective proof-of-concept study in patients with episodic vertigo. Participants were given portable, wireless video-oculography goggles for unsupervised home use and

Genome-wide analysis implicates inner ear development in Ménière's disease.

Ménière's disease (MD) is a chronic inner ear disorder characterized by recurrent vertigo, fluctuating sensorineural hearing loss, and tinnitus. Despite these distinctive symptoms, its etiology remains poorly understood. We performed a genome-wide meta-analysis of 8,969 cases and 1,962,542 controls across five large biobanks, identifying five independent genome-wide significant loci and estimating an observed-scale SNP heritability of 7% (SE 0.8%), consistent with a modest but significant genetic contribution to MD risk. Fine-mapping and integrative functional analyses implicate two convergent

Case report: endolymphatic sac tumor masquerading as Meniere's disease.

INTRODUCTION: Endolymphatic sac tumors are rare, indolent tumors with an initially innocuous presentation of audiovestibular dysfunction mimicking benign, less sinister aetiologies. The objective of this study was to describe the clinical and pathologic features of endolymphatic sac tumors, highlighted by one such case presenting to a tertiary Neuro-otology and Skull Base center. PRESENTATION OF CASE: We report a case of a destructive endolymphatic sac tumor of the middle ear resulting in cranial neuropathies of the facial, vestibulocochlear, and vagus nerves. The acute clinical decline along

Understanding and Managing Ménière's Disease: A Comprehensive Case Report.

Ménière's disease (MD) is a chronic vestibular disorder characterized by recurrent vertigo, fluctuating sensorineural hearing loss (SNHL), tinnitus, and aural fullness, with an unpredictable course that can significantly impair quality of life. Endolymphatic hydrops (EH) remains the hallmark histopathological finding; however, its underlying pathophysiology is not fully understood and likely reflects a multifactorial process involving immune, viral, allergic, genetic/familial, and vascular mechanisms. Emerging evidence suggests that EH may represent a secondary manifestation rather than the pr

Periconceptional use of GLP-1 receptor agonists and the risk of major congenital malformations: a systematic review and meta-analysis.

Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are widely used for metabolic diseases, but their reproductive safety remains controversial. We assessed the impact of periconceptional GLP-1RA exposure on birth outcomes to inform their use in reproductive-aged women. PubMed, Embase, Web of Science (through December 2025) were searched for randomized controlled trials (RCTs)/cohorts reporting offspring or pregnancy outcomes in women exposed to GLP-1RAs within three months before conception or during pregnancy. The primary outcome was major congenital malformations (MCMs), compared between n
