Cochlear otosclerosis 30 years after stapedectomy confirmed by CT, MRI.

A 64-year-old Japanese male patient, status post-stapedectomy, presented with progressive, bilateral SNHL of 30 years' duration. CT showed extensive demineralization of the otic capsule, suggestive of otospongiotic change in both ears. There were no hyperostotic lesions found, which would have been suggestive of otosclerotic change. MRI showed a morphologically intact membranous labyrinth in both ears, with no obstruction of the cochlear lumen. It was concluded that the patient has progressive otospongiotic change of the otic capsule, but no otosclerotic change.

[Progressive sensorineural hearing loss in cochlear otosclerosis].

Otosclerosis is a bone dysplasia limited to the otic capsule causing abnormal resorption and redeposition of bone. The existence of the entity "pure labyrinthine otosclerosis" or "cochlear otosclerosis" is not accepted by all authors; however, there is clinical and histologic evidence to support the existence of a progressive sensorineural hearing loss due to otospongiotic-otosclerotic lesions of the labyrinthine capsule, although diagnosis of this condition may be difficult. The involvement of the inner ear is described as degenerative changes in the spiral ligament, stria vascularis, organ o

Axial HRCT, two-dimensional and maximum intensity projection reconstructions in temporal bone lesions.

Axial and coronal high resolution computer tomography (HRCT) of the temporal bone is an important imaging technique to evaluate hearing loss. However the acquisition of the coronal imaging proves uncomfortable to the patient and is not always technically possible in old and ill patients. Clinical diagnoses of chronic otitis media, mastoiditis, cholesteatoma and cochlear otosclerosis in 10 patients were studied on a third generation CT scanner. Axial 1 mm HRCT slices were acquired and the images were transferred to an Advantage Windows workstation where two-dimensional (2D) multiplanar and sagi

CT screening for temporal bone abnormalities in idiopathic bilateral sensorineural hearing loss.

Bilateral sensorineural hearing loss can be caused by a variety of temporal bone abnormalities including primary cochlear otosclerosis, local and systemic bony diseases and some metabolic conditions. These may be identified using computerized tomography (CT), with attenuation recordings taken across the cochlear capsule (CT densitometry). Eighty patients with bilateral sensorineural hearing loss were screened over a period of six and a half years using this technique, and only three cases (3.8 per cent) of treatable disease were detected. Positive yields may be increased by screening selected

[A case of Paget's disease with progressive sensorineural hearing loss involved in the bilateral bony labyrinths].

Paget's disease of the bone is a chronic, progressive disease of unknown etiology characterized by abnormal bony resorption and deposition. It is a common skeletal disease in Europe and North America, while in Japan it is very rare. Paget's disease of the temporal bone has been reported to cause hearing loss frequently. We report a 50-year-old woman with Paget's disease who had progressive bilateral sensorineural hearing loss and right vestibular dysfunction. CT and 3 Dimensional CT (3D-CT) images demonstrated resorption of the entire bony labyrinth and its surroundings on both sides and that

The effect on tinnitus of stapes surgery for otosclerosis.

Among 42 Japanese patients with otosclerosis who underwent stapes surgery, 22 patients including 10 received total stapedectomy, 6 partial stapedectomy and 6 stapedectomy, were investigated on preoperative and postoperative tinnitus by sending questionnaire. The tinnitus was evaluated by "tinnitus score" advocated by the Japan Audiological Society, and the changes of the scores were analyzed according to pre- and postoperative hearing change, type of stapes surgery and pitches of preoperative tinnitus. In these 22 patients, tinnitus score was improved in 68%, unchanged in 27% or worsened in 5%

Age-related hearing loss and bone mass in a population of rural women aged 60 to 85 years.

Demineralization of the cochlear capsule in conjunction with age-related bone mass loss may be one biologic factor contributing to hearing loss in the elderly. In other metabolic bone diseases, including Paget's disease of the bone and cochlear otosclerosis, demineralization of the cochlea has been associated with sensorineural hearing loss. In 1988/1989, the relation between hearing loss and bone mass of the radius and femoral neck was studied cross-sectionally in 369 women aged 60 to 85 years from three rural communities. Hearing sensitivity was measured using audiometry, and bone mineral de

Facial nerve stimulation produced by cochlear implants in patients with cochlear otosclerosis.

Thirty-eight adult cochlear implants have been performed at the University of Minnesota. Facial nerve stimulation by the implant in response to sound has been noted in four of these cases. Three of the four were patients whose sensorineural hearing loss was caused by cochlear otosclerosis. In each case it was possible to place the electrodes, however multiple leads had to be deprogrammed in order to avoid facial nerve stimulation. In each case characteristic radiographic findings of cochlear otosclerosis could be identified on preoperative temporal bone computed tomography scans. Although faci

[The radiological pattern of cochlear otosclerosis. A CT study of 14 patients].

In this review of cochlear otosclerosis 14 cases were studied by CT scan aiming to establish a densitometric pattern of the capsular foci and relating it to the hearing and vestibule dysfunctions. Severe demineralization with characteristics of probable activity (increased lucency of 30-40%) was demonstrated in the capsular foci. These were mainly cochlear with endosteal involvement (93%): large (64%) or discrete (29%). Cochlear otosclerosis was widespread in 64% of the patients, with coexisting foci in the semicircular canals (38%), vestibule aqueduct (43%) and internal auditory canal (43%).

Sensorineural hearing loss: more than meets the eye?

PURPOSE: To assess the value of MR in patients with sensorineural hearing loss (SNHL) caused by lesions other than acoustic neuromas. METHODS: MR studies of 51 patients with SNHL were retrospectively reviewed; patients with acoustic neuroma were excluded to focus on the more uncommon causes. RESULTS: Twenty patients had labyrinthine lesions. Six patients had viral labyrinthitis, one patient had bacterial labyrinthitis, and one patient had luetic labyrinthitis. Three patients had hemorrhage in the labyrinth, two posttraumatic and one spontaneous from an adjacent temporal bone tumor. Only one

Value of computed tomography (CT) in the diagnosis of cochlear otosclerosis.

The temporal bones of 40 patients with surgically confirmed otosclerosis were studied with high-resolution CT. The hearing of these patients was re-examined 5 years later. Foci of demineralization in the cochlear capsule were detected in 36% of operated ears and in 44% of otosclerotic ears not operated on. Mean bone conduction (BC) thresholds of ears with positive radiological findings did not differ significantly from those of otosclerotic ears with normal CT, either at the time of CT examination or 5 years later. Over 5 years, elevation of BC thresholds by 10 dB or more was observed signific

Myths in neurotology.

For many diseases the etiology of disordered function is unknown and therapy is either symptomatic or empirical. The empirical approach to therapy is acceptable to many physicians, particularly if a treatment can be based on some attractive, although unproven, concept of pathogenesis. A hypothetical explanation of disordered function can become widely popular in spite of a serious lack of scientific support. When evidence emerges that refutes the logic of a concept of pathogenesis, then that concept becomes a myth. Human temporal bone studies have identified several myths in neurotology that d

Temporal bone histopathology of a patient with a nucleus 22-channel cochlear implant.

The temporal bone histopathology of a patient with profound sensorineural hearing loss attributable to cochlear otosclerosis is presented. The patient had a Nucleus 22-channel cochlear prosthesis implanted in the right ear prior to his demise. Unintentional scala media insertion of the electrode array resulted from labyrinthitis ossificans obliteration of the scala tympani. Inadvertent facial nerve stimulation necessitated program exclusion of electrodes 11 to 16 located in proximity to the labyrinthine and geniculate segments of the facial nerve. The patient received significant benefit from

The detrimental effect of aminohydroxypropylidene bisphosphonate (APD) in otospongiosis.

Two middle-aged female patients with active stapedial otosclerosis and sensorineural hearing loss have been treated with aminohydroxypropylidene bisphosphonate (APD). No effect on the Schwartze sign was observed either during or following therapy. No changes were seen in the audiometric data of the patients during the 1st year. Then, within a few weeks, both patients complained of sudden deterioration of the existing hearing losses in both ears, with an alteration of tinnitus. One patient became totally, bilaterally deaf, while the other retained minimal auditory function in the low frequencie

Stapedectomy: long-term hearing results.

The initial improvement in hearing following stapedectomy usually deteriorates with the passage of time. We studied the long-term results of stapedectomies performed on 42 patients (49 ears) between 1959 and 1969 who had a minimum follow-up of 18 years. Both air conduction (AC) and bone conduction (BC) thresholds progressively deteriorated over the long term. The degree of BC loss paralleled that expected from presbycusis alone. A greater deterioration was noted in the AC levels, producing a recurrent conductive hearing loss in the speech frequencies. Age at the time of surgery had no effect o

Cochlear otosclerosis: does bone formation affect cochlear implant surgery?

This study aimed to demonstrate that new bone formation in the scala tympani of patients deaf from otosclerosis does not preclude cochlear implant surgery. In seven temporal bones from patients with otosclerosis, we measured the extent of new bone from the round window to the distal part of the new growth. We compared results to surgical data on the extent of drilling and depth and ease of placement of the electrode in 20 patients deaf from otosclerosis. We also examined clinical performance and voltage requirements for long-term implant use in patients with and patients without ossification o
