This study aimed to discuss the clinical implications associated with the absence of the stapedius muscle and concomitant otosclerosis. The causes of mixed hearing loss are presented based on its embryonic development, audiological tests, and high-resolution computed tomography (CT) imaging. This case describes a 35-year-old female patient diagnosed due to bilateral conductive hearing loss. The patient complained of a significant deterioration of hearing and speech understanding lasting for several months. Preoperative audiometric work-up revealed air-bone gap with the presence of Carhart's no

Temporal bone imaging has historically suffered from spatial resolution issues because the spatial resolution of conventional high-resolution computed tomography (CT) is 0.5 mm, while the smallest structure of the middle ear, the stapes, has very thin components, as thin as 0.19 mm, and small structures, such as small channels containing nerves and arteries, have historically been beyond its spatial resolution. Photon-counting and ultra-high resolution CT allow for improved spatial resolution and reduced radiation dose compared to conventional high-resolution CT. This article provides a techni

Fibrodysplasia ossificans progressiva (FOP) is the most dramatic form of progressive heterotopic ossification of soft tissues. Hearing impairment in FOP patient is a common feature, reported by about 50% of affected patients. However, wide case series considering audiologic features are lacking. We report the audiologic history of a group of FOP patients in order to define the hearing loss characteristics, genotype association, and possible rehabilitative options. We report audiometric history of 16 FOP patients referred to our Institute from 2014 onwards without ear malformations or other kno

OBJECTIVE: Management of advanced otosclerosis (AO) has evolved over the past 20 years with the availability of cochlear implant (CI) in addition to stapes surgery. Both procedures are reliable treatment options for AO with similar success rates and currently there are no standard guidelines regarding the surgical treatment of AO. The aim of this paper is to report the outcomes and complications of a series of patients with AO submitted to CI at our Institution. An extensive review of the literature is also provided.
METHODS: The study group was composed of 31 adult patients affected by AO, c

Endoscopic stapedotomy offers a panoramic transcanal view of middle-ear structures while minimizing canal trauma. Reverse stapedotomy, in which the footplate fenestration precedes the removal of the stapes superstructure, may improve footplate stability and procedural safety. The use of CT-based three-dimensional (3D) virtual endoscopy for surgical planning in otosclerosis remains uncommon. We present the case of a 35-year-old female with bilateral otosclerosis (stage 1B on high-resolution CT) and a 35 dB air-bone gap in the poorer-hearing ear. Preoperative CT-based 3D virtual endoscopy simula

PURPOSE OF REVIEW: This paper aims to contribute to improved diagnostic accuracy, review outcomes of surgery, and provide guidance on how patients and caregivers are counselled about treatment options for juvenile otosclerosis (JO) and congenital stapes footplate fixation (CSFF).
RECENT FINDINGS: In JO, there is abnormal bone resorption and recalcification leading to progressive conductive or mixed hearing loss depending on the location of affected bone. There is a higher rate of obliterative otosclerosis in children compared to adults and thus consideration should be given to proceed with ea

INTRODUCTION: Direct piston-stimulation of the oval window with a middle ear implant, following a stapedotomy or stapedectomy, is one treatment option for patients with otosclerosis. Here, we experimentally investigated whether increasing the surface area of the actuated piston enhances transmission efficiency in oval window (OW) stimulation.
METHODS: Fresh-frozen human cadaveric temporal bones (N = 14) were used to evaluate the output of different oval window couplers (OWCs) of A = 0.38 mm², 1.26 mm2, 3.06 mm2 by intracochlear pressure difference measurements between scala vestibuli (PSV) an

This systematic review investigates the influence of fenestration size and prosthesis diameter on hearing outcomes in patients undergoing primary stapedotomy for otosclerosis. A total of 11 studies were included, comprising randomized controlled trials, cohort studies, and one cross-sectional study, with follow-up durations ranging from three months to one year. Fenestration sizes most commonly ranged from 0.5 mm to 0.8 mm, while prosthesis diameters varied between 0.4 mm and 0.6 mm. Across studies, postoperative air conduction thresholds improved by 20-30 dB, and air-bone gap (ABG) closure wi

BACKGROUND: Temporal bone computed tomography (CT) is essential for evaluating ear diseases but requires awareness of normal variations to avoid misdiagnosis. The cochlear cleft, seen as pericochlear CT hypoattenuation, reflects incomplete endochondral ossification and mainly occurs in children. Although a normal variant, it can mimic conditions such as otosclerosis. Data on its prevalence and extent in children, especially in relation to hearing status and other temporal bone findings, remain limited.
OBJECTIVE: The aim of our study is to evaluate the overall prevalence of cochlear clefts in

Otosclerosis and endolymphatic hydrops (EH) are distinct inner ear pathologies that may coexist within the same temporal bone, creating diagnostic and therapeutic challenges. Their interaction remains controversial, with proposed mechanisms suggesting that otosclerotic involvement of the vestibular aqueduct or cochlear endosteum may obstruct endolymphatic flow, leading to secondary hydrops. This review summarizes current evidence on the coexistence, pathophysiology, diagnostic tools, and clinical implications of otosclerosis-associated EH. Histopathologic studies have demonstrated EH adjacent
