This report reviews 12 cases of congenital cholesteatoma of the middle ear, mastoid, and petrous pyramid seen mostly in adult and older juvenile patients between 1984 and 1992. Featured herein are two unique instances of purulent discharge external to an intact tympanic membrane via a fistula caused by extensive mastoidal erosion, three cases that illustrate the importance in Oriental populations of the use of high resolution computed tomography to obviate misdiagnosis when all other diagnostic techniques suggest unilateral otosclerosis, and two instances of multicentric cholesteatomas in well

Previous studies have indicated that the masking-level difference is reduced in listeners having conductive hearing loss, even when sensation level is controlled. According to the crosstalk hypothesis, this deficit can be explained in terms of reduced binaural cues at the level of the cochleas, due to acoustic contributions from the bone conduction route. It is possible that such a deficit can be overcome through the use of insert earphones, because such transducers provide a high degree of interaural attenuation. In this study, NoSo and NoS tau thresholds were measured in normal-hearing subje

The recent advances in MRI of the temporal bone brought about by the availability of intravenous contrast agents sensitive to the disruption of blood-brain barrier have further expanded the role of MRI in the evaluation of patients with vestibulocochlear symptoms. Not only can MRI be used to diagnose vestibular schwannomas (and other intracanalicular and cerebellopontine angle lesions mimicking it) with a high degree of accuracy, but it can now identify a variety of inflammatory and neoplastic processes of the membranous labyrinth which, in the past, were only made presumptively and occasional

From the 1950s onwards ossicular bone autografts have been used to restore the middle ear sound conduction mechanism. Controversy still exists regarding the appropriateness of autologous ossicular bone grafts in chronic middle ear diseases. This communication is based on a study of 149 ossicles surgically removed from 120 patients with different ear diseases, at the Department of Otolaryngology, Head and Neck Surgery of the University of Wuerzburg. It is the object of this study to systematically investigate the histological findings in the ossicles in cholesteatoma and chronic suppurative oti

We examined 105 patients who underwent partial stapedectomies at the age of 65 years or older as treatment for combined hearing losses due to otosclerosis. Among other things investigated was the influence of the operation on the hearing of both the operated and unoperated ears, as well as on tinnitus existing preoperatively and the use of a hearing aid. Findings showed that tinnitus decreased or vanished in 75% of the cases. Sixty-one percent of the patients needed a hearing aid continuously preoperatively, with 48% not needing it post-operatively and 13% using it only occasionally. The degre

Basic fibroblast growth factor (bFGF) has a mitogenic effect on fibroblasts and osteoblasts for matrix proliferation and on endothelial cells for neovascularization. Because otic capsule osteogenesis in autoimmune disease subjects often involves abnormal matrix and vascular changes, bFGF may serve as a potential mediator for such bone disorders. To investigate this relationship, bFGF was evaluated in the Palmerston North autoimmune strain mouse, which develops otic capsule sclerotic lesions during the progression of its systemic disease. Inner ears from PN mice, along with control CBA/J and C3

For the first time, otosclerotic stapes have been distinguished from unafflicted controls at a high level of significance by using a spectrum of elements measured by energy-dispersive spectrometer-electron probe microanalyses (EDS/EPMA). Discriminant analyses of the maximum concentration of 13 elements measured at several sites within each of 32 stapes differentiated otosclerotic from unafflicted individuals well above the 95% confidence level. Eight of the 9 control (unafflicted) and 21 of the 23 afflicted stapes were correctly classified. In descending order of contribution to the discrimina

This histopathologic review illustrates that otosclerosis is a very pleomorphic bone dyscrasia. It may fix the stapes by a tiny bridge of abnormal bone or may totally obliterate it. It may spread to the cochlea to produce either a sensorineural loss of varying degree or deafness. Lesions may be sclerotic, spongiotic, or fibrous but usually comprise all these histologic types. This diversity of size and anatomic distribution coincides with the variable types of hearing loss, from a mild conductive deficit to a total loss. In spite of more than a century of investigation of otosclerosis, an etio

A method was developed for fast and efficient isolation of DNA from formalin-fixed, decalcified, celloidin-embedded human temporal bone sections for subsequent use in polymerase chain reaction (PCR) DNA amplification. The method relies on the use of an enzymatic digestion with proteinase K to release and solubilize the patient's DNA from an individual 20- to 25-microns temporal bone section. The method described should be of great value to those investigators extracting DNA from archival individual human temporal bone sections for polymerase chain reaction assays of specific genetic alteration

The autopsy report of Ludwig van Beethoven written by Dr Johann Wagner in 1827 reveals that he had renal calculi that had not been diagnosed during his lifetime, together with perirenal fibrosis. The most comprehensive interpretation of this autopsy finding is that the regular calcareous deposits in every one of his renal calices represented calcified necrotic papillae. Severe urinary obstruction or diabetes as possible causes of papillary necrosis were not present. Analgesic abuse because of headaches, back pain, and attacks of rheumatism or gout may be presumed on the basis of Beethoven's un

The etiology of otosclerosis is unknown. Some investigators showed the presence of high levels of antibodies titers against type II collagen, the major cartilage collagen, proposing that the pathogenesis of otosclerosis is related to cartilaginous remnants contained in the otic capsule. Other collagens like type IX and XI, namely "Minor cartilage collagens", appear to be restricted to cartilaginous tissue. The aim of this study was to test otosclerosis patients for the presence of antibodies to these minor collagens using enzyme-linked-immunosorbent assays. Levels of antibodies to collagens ty

UNLABELLED: At Glostrup Hospital, Copenhagen, exceptionally few stapedectomies were performed during the period 1979-1985. The audiological results five to eleven years after the operation were investigated in 64 patients. About half of the patients were operated using large fenester technique, the other half using small fenester technique. All patients but two were operated by the same expert surgeon. The initial results showed good stability with an unchanged median hearing gain of 30 dB. Eighty-five percent of the patients had an SRT < or = 35 dB HL in contrast to 89% at the 1-year follow-u

The purpose of this investigation was to examine the effects of prolonged monaural amplification on the suprathreshold speech-recognition scores in the unaided ears of 46 persons with bilateral essentially symmetric and conductive hearing impairment and aided ears of 46 other persons with bilateral essentially symmetric and conductive hearing impairment. All subjects were less than 50 years of age. The suprathreshold speech-recognition loss scores for taped monosyllabic German PB words were analyzed. The frequency distribution of pure-tone averages was essentially equivalent for the aided and

The results of stapedectomy were compared between 162 otosclerotic ears operated on using the posterior crus technique and 182 otosclerotic ears undergoing Teflon piston stapedectomy. The large fenestra technique with fascia seal to the oval window was used in all cases. Mean follow-up period was 9.6 years. Neither short-nor long-term hearing results showed any significant differences between the two surgical technique groups. Complications of surgery were more common in patients undergoing Teflon piston stapedectomy. On the other hand, a re-operation for recurring conductive deafness was perf

A 2-year prospective double-blind study was performed to evaluate the role of etidronate disodium for the treatment of progressive hearing loss in patients with otosclerosis. A pulsed dosage regimen was used during the 2-year period and the patients were followed up with otologic and audiometric examinations. Although statistically significant differences were not achieved between the study and control groups, the study did reveal a trend toward stabilization or improvement in air conduction thresholds in some frequencies (1000 and 4000 Hz) and in bone conduction thresholds at other frequencie

Although many persons with sensorineural hearing loss (SNHL) seeking hearing care do not have active ear disease, audiologists must appreciate the potentially dynamic and treatable nature of some etiologies of SNHL. This is particularly important now as audiologists are increasingly employed in private practice or other clinical settings that may serve as the point of entry for hearing care. In this paper, we review a variety of clinical entities affecting newborn infants, children, and adults that are associated with dynamic SNHL. Using a case report format, we illustrate treatment-related ch

It has long been known that fluoride "hardens" mineralized tissues. Fluoride ingestion through drinking water in areas naturally rich in fluoride leads to osteosclerosis, known as endemic fluorosis. The first suggestion that fluoride be used in the treatment of osteoporosis was made in 1964. However, despite 30 years of research, the treatment remains controversial. Fluoride has a dual effect on osteoblasts. On the one hand, it increases the birthrate of osteoblasts at tissue level by a mitogenic effect on precursors of osteoblasts, while on the other hand it has a toxic effect on the individu

PURPOSE: To assess the value of MR in patients with sensorineural hearing loss (SNHL) caused by lesions other than acoustic neuromas.
METHODS: MR studies of 51 patients with SNHL were retrospectively reviewed; patients with acoustic neuroma were excluded to focus on the more uncommon causes.
RESULTS: Twenty patients had labyrinthine lesions. Six patients had viral labyrinthitis, one patient had bacterial labyrinthitis, and one patient had luetic labyrinthitis. Three patients had hemorrhage in the labyrinth, two posttraumatic and one spontaneous from an adjacent temporal bone tumor. Only one

Recently, it has been suggested that otosclerosis represents the host's ongoing immunologic response to measles or other viral antigens. Documentation of past inflammation within the inner ear would serve as further evidence that this mechanism may be at play in the pathogenesis of the disease. Among the characteristic signs of prior inflammation in the inner ear is the presence of lamellar bone at the site of inflammation. This has been described in the temporal bone of a patient with immune-mediated deafness and with the temporal bones of experimental models of immune-mediated inner ear dise

Atelectatic ears, which by definition are poorly aerated, are also usually associated with poor mastoid pneumatization. On the other hand, otosclerotic patients, whose middle ears are usually exceptionally well aerated, also have excellent mastoid pneumatization. Three unusual cases are presented, in which partial atelectasis developed in stapedectomized patients. In each case the mastoid was later found to be nonpneumatized, and further analysis revealed that their stapes fixation had in effect most probably been of non-otosclerotic origin. Thus, although these three cases had at first appear
