Anatomic variations of the cochlea and relations to other temporal bone structures.

A thorough knowledge of the normal range of variation of anatomy and topography of the cochlea is necessary for optimal reproduction of this structure and correct interpretation of the radiographs. Radiographic identification of incomplete cochlear coils is essential in the diagnosis of congenital malformations such as Modini's deformity. Furthermore, a diagnosis of otosclerosis/otospongiosis has to be based on recognition of changes in the otic capsule. The size and shape of the human cochlea and the normal ranges of variation of its dimensions were evaluated in 95 plastic casts, prepared fro

Stapedotomy technique and results: ten years' experience and comparative study with stapedectomy.

Fifty of the 810 patients who underwent stapedotomy for otosclerosis from 1969 through 1988 were randomly chosen for follow-up of at least 5 years. Most of the patients (65%) had follow-up of 10 years after stapedotomy, but another 50 patients who underwent stapedectomy had follow-ups of longer than 10 years. In 50 patients, stapedectomy under local anesthesia was performed by removal of the footplate of the stapes, using an endaural incision, and covering the oval window with Gelfoam. In the other 50 patients, stapedotomy was performed under general anesthesia, using an endopreauricular incis

[The propagation pathways of an excitatory stimulus to the receptor apparatus during the initiation of auditory sensations using amplitude-modulated focused ultrasound].

Studies have been made on thresholds of perception of amplitude-modulated focused ultrasound (AMFU) during modulation by 125-8,000 Hz in 6 healthy subjects, 100 patients with otosclerosis before and after operation, and 13 subjects with other disturbances in hearing. Perception thresholds for AMFU were compared with bone and air audiograms in the same subjects. Changes in the thresholds for AMFU after operation were of the same pattern as those in the thresholds for air audiometry, indicating mainly aerial pathway of signal transmission for AMFU. Perception of AMFU in otosclerotic patients is

Otic capsule bony lesions in the Palmerston North autoimmune mouse.

Otosclerosis is an otic capsule disorder of unknown etiology. While autoimmunity has been proposed as part of the etiopathogenesis of otosclerosis, no spontaneous autoimmune disease animal model has been identified. In the Palmerston North mouse, a model for systemic lupus erythematosus, sclerotic lesions consistently develop within the modiolus that are correlated with systemic autoimmune disease symptoms. No lesions were seen in 2-month-old mice, which is before autoimmune disease onset at 4 months. Lesions were first seen in mice at 6 to 8 months of age and increased in size and frequency t

Chondrogenic potential of tragal perichondrium: a cause of hearing loss following stapedectomy.

Tragal perichondrium is a widely used tissue seal in the oval window following stapes surgery. Autogenous and easily accessible, it is a suitable substance to cover the vestibule in total stapedectomy, and to seal around the prosthesis in small-fenestra stapedotomy. The incidence of complications from the use of perichondrium in this manner is exceedingly low. We report a case where tragal perichondrium in the oval window resulted in the proliferation of cartilage. The cartilage displaced the stapes prosthesis, resulting in a conductive loss. Although the chondrogenic potential of perichondriu

Molecular and cellular biology of the inner ear. The next frontier.

Molecular and cellular biology is the study of cellular ultrastructure and function. Current research in immune regulation, delineation of receptor control function (signal translation at the cellular level), and nucleic acid manipulation (genetic engineering) illustrates fundamental relationships among biochemistry, immunology, and molecular and cellular biology. Our knowledge already includes significant discoveries in membrane permeability, ionic gradients, and mediators of homeostasis. To these are added newer discoveries in bone metabolism, vascular anatomy and physiology, trace element d

[Evaluation of cochlear function in otospongiosis. Preliminary study].

One of the major therapeutical problems encountered in the natural history of otospongiosis are lesions affecting Corti's organ. We obtained audiometric measurements, including pure-tone and speech audiograms, tympanograms, and spontaneous and provoked otoemission determinations in 26 patients (40 ears), who benefited from total platinectomy and collumellar effect recuperation by means of temporal aponeurosis interposition and piston-cupping. Results show that the piston-cup procedure does not appear to modify the transfer function of the middle ear. On the other hand, even at a stage when pur

[Surgical treatment of otosclerosis. Effects and risks].

Surgical treatment of otosclerosis involves an opening of the labyrinth and, accordingly, a risk of complications from that organ: vertigo and sensorineural hearing loss or anacusis. During the past decade, the number of new patients with otosclerosis has stabilized at a low level in Danish ENT-departments. Thus, typically the number of stapedectomies per year per department is between 10 and 25. The study elucidates effects and risks at the operation, focusing in particular on the question whether this treatment ought to be centralized. In a seven-year period, a total of 75 stapedectomies in

The fate of the non-operated ear in otosclerosis.

Records of 300 consecutive patients who had only one ear operated on by stapedectomy and who received long-term followup were studied. These cases came from 3036 stapedectomy operations performed between January 1961 and April 1969. In general, the two ears behaved the same: if a "flat" sensorineural loss occurred in one ear, it was likely to develop in the other. Similarly, if one ear developed a high-tone loss, the other would do likewise. With the exception of acute fistula, there is no suggestion that the operation of stapedectomy predisposes an ear to late sensorineural problems. Patients

Mastoid pneumatization in otosclerosis.

The extent of mastoid pneumatization in 150 otosclerotic ears was compared with that of 150 healthy control ears. The size of mastoid pneumatization was measured by use of the Schüller lateral x-ray projection with the help of computed planimetry. The measurements showed the average pneumatized area in otosclerotic ears to be 17.4 +/- 5 cm2, in contrast to 12.9 +/- 4 cm2 for the healthy control ears. The difference between the two groups was highly significant (p less than .0001). While both groups showed a bell-shaped distribution of the measured pneumatized area, the curve of the otoscleroti

Sex ratios in otosclerotic families.

The sex ratio (proportion of males) of offspring of otosclerotics is significantly and substantially below expectation. To account for this and other features of the condition, it has been suggested elsewhere that otosclerosis is subject to autosomal dominant transmission with intrauterine selection against male heterozygotes. To test this hypothesis, a search was made for published data on the sexes of sibs of otosclerotics. These sibs do not have an unusual sex ratio, thus casting doubt on the hypothesis. Accordingly it is suggested here that both the condition and the sex ratios are causall

Immunohistochemical evidence of measles virus antigens in active otosclerosis.

Despite intensive investigation, the cause of otosclerosis remains uncertain. Recent studies of Paget's disease of bone have revealed a possible viral origin. Because of similarities between otosclerosis and Paget's disease, we have pursued investigation of a possible viral cause of otosclerosis. Four temporal bone specimens from patients with otosclerosis, processed for immunohistochemistry, demonstrated positive specific reactivity with monoclonal antibodies to measles virus antigens using the indirect immunofluorescent and immunoperoxidase techniques. Reactivity was most intense in active f

Radioanatomy of cochlear and stapedial otosclerosis.

Otospongiosis of the otic capsule is a primary focal disease, which can be reproduced by both conventional multidirectional tomography and computed tomography. The final stage of otospongiosis, otosclerosis has a complete different appearance in radiography. When the otospongiosis focus is located at the oval window a progressive conductive hearing loss will result, which in its final stage will have the character of otosclerosis with fixation of the stapes in the oval window. The radiographic diagnosis with either conventional multidirectional tomography or CT is more or less pathognomonic an

Surgically confirmed clinical otosclerosis among the Chinese.

We report herein on 163 cases of surgically confirmed otosclerosis among Chinese. Our results suggest significant racial differences between Chinese and whites in respect to the incidence of this disease, and the likelihood of a family history. Surgical and histopathological findings, while confirming otosclerosis identical to that experienced in whites, suggest that generally the disease is milder among Chinese. The sex ratio, age range, and bilateral involvement, on the other hand, were discovered to be basically similar between the two races. A detailed description is given of the postauric

Drugs affecting the inner ear. A review of their clinical efficacy, mechanisms of action, toxicity, and place in therapy.

Many drugs have a site of action within the inner ear. The list includes therapeutic, diagnostic and ototoxic agents. Therapeutic agents are most useful in cases of infections, endolymphatic hydrops, vascular insufficiency, vertigo of peripheral origin, autoimmune disease, otosclerosis (otospongiosis), sudden hearing loss and tinnitus. For infections, the most widely used anti-microbial agents are the penicillins and cephalosporins. There are no antiviral agents that have been proven useful for inner ear viral infections. However, steroids have been of some value for controlling some of the se

Otopathology of Egyptian mummy Pum II: final report.

Otoscopic examination of the ears of Egyptian mummy PUM II with the operating microscope revealed a small oval perforation of the right tympanic membrane in the posteroinferior quadrant. The smooth and concise margins suggested an etiology of middle ear infection. A method which allows decalcification of ancient specimens without disintegration of the eardrum and other soft tissues within the temporal bone was described. There was evidence of histological repair at the margins of the perforation consistent with sequela of otitis media. No other abnormalities were observed in the temporal bones

Diagnostic value of extratympanic electrocochleography in Menière's disease.

In order to examine the usefulness of the noninvasive extratympanic electrocochleography (ECochG) in the diagnosis of Menière's disease, the results of extratympanic ECochG, performed on 28 normally hearing ears, 3 ears with otosclerosis and 325 ears with sensorineural hearing loss of various origins including 56 Menière ears, were analyzed. The characteristics of summating potential (SP) parameters in Menière ears were different from those of ears with hair cell loss due to streptomycin and kanamycin ototoxicity and noise, while there was no difference between the action potential (AP) of the

Far-advanced otosclerosis. Cochlear implantation vs stapedectomy.

In 1981, our group, as part of a Food and Drug Administration investigator team directed by William House, performed Illinois' first cochlear implantation. We have since performed cochlear implantations in ten other persons. Since May 1980, approximately 35 severely to profoundly deaf persons per year have been referred to our clinic for evaluation. A small number of these patients had far-advanced otosclerosis. In these cases, exploratory tympanotomy and stapedectomy followed by rehabilitation with a hearing aid was a more appropriate management than cochlear implantation. Case histories and

Pathology of otosclerosis: a review.

Otosclerosis is a bone disorder of unknown etiology confined to the otic capsule. Failure of remodeling of newly formed vascular, woven bone (otospongiosis) results in sclerotic bone (otosclerosis) with abnormal osteons. Involvement of the oval window causes conductive hearing loss. Electron microscopic, histochemical, and biochemical studies identify normal cellular and matrix components of otosclerotic bone without providing clues to the abnormal bone formation and resorption. Plastic-embedded, nondecalcified histologic sections with in vivo tetracycline labels permit the study of mineraliza
