The sensorineural component of a mixed hearing loss due to otosclerosis is generally accepted as due to the otosclerotic lesion. The existence of pure cochlear otosclerosis without stapes fixation has been questioned. However, we are documenting 7 such cases in a separate publication. We reviewed the records of 258 revision stapedectomy operations performed at the Otological Medical Group during an eight year period. Displacement of the prosthesis to the inferior edge of the window was the commonest cause of failure (41%) and occurred predominately in wire-Gelfoam pad cases. An oval window fistula, a short prosthesis or bony closure of the window were causes of failure in 9% each. Incus necrosis was the cause of failure in 5%. Significant post-stapedectomy cochlear deficit is associated with the following factors in an uncomplicated operation: (1) The age of the patient. (2) A pre-operative bone conduction loss, averaged over 0.25, 0.5, 1.0 and 2.0 KHz, of 20 db or more. (3) A left-to-right sloping bone conduction curve (descending audiometric curve), especially where there is a similar curve in the other ear.

The applications of computed tomography (CT) in temporal bone studies have increased because of thin sections (less than 2 mm), high spatial resolution (less than 0.8 mm) and high resolution reconstruction algorithms. Electronystagmographic tracings were correlated with the histopathologic findings in the peripheral vestibular system of 66 temporal bones of 34 patients who complained of dizziness. 468 stapes operations have been performed at the ENT-Department of the University of Zurich between 1970 and 1978. The effect of osmotic treatment on cases with fluctuating sensorineural hearing loss other than Menière's disease is described in this paper. The autosomal dominant hereditary syndrome of symphalangism and stapes fixation consists of multiple synostoses, most frequently symphalangism, in addition to synostoses of carpal and tarsal bones and radio-ulnar synostoses, various other malformations of the limbs (restricted joint movement or syndactylism) and variable degrees of conduction disorders due to stapes fixation, starting in childhood. The Temporal Bone Laboratory of Northwestern University Medical School has three sets of temporal bones from patients who had antemortem polytomographic examinations resulting in a diagnosis of otospongiotic involvement of the cochlea.
