Changes in Quality of Life Among Patients With Far-Advanced Otosclerosis After Stapes Surgery.

BACKGROUND Far-advanced otosclerosis (FAO) is characterized by profound hearing loss or deafness in the course of the disease, with an air conduction threshold of at least 85 decibels hearing level (dB HL) and a bone conduction threshold exceeding the audiometer's capacity. The aim of this study was to evaluate whether, in patients with FAO after stapedotomy, changes in word recognition score (WRS) are correlated with changes in quality of life. MATERIAL AND METHODS This study included 20 patients with FAO who underwent stapedotomy between 2005 and 2020. RESULTS The study investigated the rela

Predictive impact of the behavior of the Carhart's notch after stapedotomy on speech audiometry parameters.

Introduction: Stapedotomy improves air conduction thresholds and restores one of the components of bone conduction (BC): sound transmission to the inner ear via vibrations of the ossicular chain. This results in a measurable improvement in BC thresholds. Audiometric confirmation of pseudoperceptual hearing loss is the loss of the Carhart's notch (CN) observed after surgery. Sensorineural hearing loss is characterized by varying degrees of speech incomprehension impairment, particularly clinically apparent when the impairment affects frequencies essential for communicati

Heterozygous Nonsense Mutation in the Nuclear Transport Factor KPNA7, a Maternal Factor Active in Embryonic Tissues, Causes Autosomal Dominant Otosclerosis.

Otosclerosis is a common cause of conductive hearing loss thought to result from dysregulated bone remodeling in the embryonic tissues of the globuli interossei. Both familial and sporadic cases have been reported. To date, 10 published OTSC loci and four genes (FOXL1 (OTSC11), SMARCA4 (OTSC12), MEPE, SERPINF1) have been identified in autosomal dominant families. Using a combined genetic and genomics approach in five affected siblings, we identified a nonsense mutation in Karyopherin subunit α7 (KPNA7, c.49C>T, p.R17X), the newest of the importin-α family of nuclear transporters. KPNA7 is a ke

Management Options and Their outcomes in Bilateral Advanced Otosclerosis.

BACKGROUND: Otosclerosis is a hereditary disorder that causes progressive hearing loss in the adult population. Owing to its progressive nature, a multimodal treatment approach is often required. Management options were offered to patients following comprehensive audiological and radiological evaluation, incorporating the Merkus algorithm in clinical decision-making. This study aims to compare the audiological and speech-related outcomes of cochlear implantation (CI) and stapedotomy with hearing aid use in patients with bilateral advanced otosclerosis. METHODS: This cohort study included 38 p

CT imaging parameters of the oval window region can predict the extent of stapes footplate exposure in patients with otosclerosis.

BACKGROUND: Stapedotomy, including classic stapedotomy (CS) and reversed stapedotomy (RS), is the primary surgery for otosclerosis. RS is preferred when the stapes suprastructure is preserved and footplate exposure is adequate. OBJECTIVES: To explore the correlation between stapes footplate exposure and CT imaging parameters of the oval window region, and assess their value in preoperative surgical planning. MATERIALS AND METHODS: A retrospective study of 166 otosclerosis surgeries (105 RS, 61 CS) performed by a single senior surgeon (2018-2025) was conducted. Reconstructed preoperative temp

Reconsidering surgical thresholds for stapedotomy: multicenter outcome analysis.

INTRODUCTION: Stapedotomy provides excellent audiological outcomes in otosclerosis, yet no universally accepted criteria exist for surgical indication. Current recommendations vary substantially across countries, particularly regarding audiometric thresholds. This study aimed to evaluate whether performing stapedotomy at lower preoperative hearing thresholds compromises postoperative outcomes. MATERIALS AND METHODS: A retrospective multicenter cohort study was conducted including adult patients undergoing primary stapedotomy for otosclerosis.
