Vertigo-unsolved Mystery (Meniere's Disease) Care Redefined: Insights from a Tertiary Care Center.

BACKGROUND: Meniere's disease remains a poorly understood inner ear disorder since its first description by Prosper Meniere in the early 1800s. Characterized by episodic vertigo, fluctuating hearing loss, and tinnitus, it continues to challenge clinicians due to its unpredictable course and variable treatment response. Lifestyle modification alone often fails to adequately control vertigo episodes. AIM AND OBJECTIVES: To assess the effectiveness of oral pharmacotherapy in controlling vertigo and stabilizing hearing in patients with Meniere's disease, and to develop a practical, non-invasive t

Genome-wide analysis implicates inner ear development in Ménière disease.

Ménière disease (MD) is a chronic inner ear disorder characterized by recurrent vertigo, fluctuating sensorineural hearing loss, and tinnitus, but its etiology remains poorly understood. We performed a genome-wide meta-analysis of 8,969 MD case subjects and 1,962,542 control subjects across five biobanks, identifying five independent genome-wide significant loci and estimating an observed-scale SNP heritability of 7% (SE 0.8%), indicating a modest but significant contribution of common genetic variation to MD risk. The genome-wide significant signals comprise two independent variants at EYA4,

[Cell and gene therapies in Menière's disease: perspectives for a slowly progressive inner ear disease].

Menière's disease (MD) is classically understood as a pressure-related disease of the inner ear; however, new histopathologic and imaging findings call for reassessment of this model. The current article focuses on new aspects of MD pathophysiology and derives innovative causal treatment approaches for the future based on cell and gene therapy. The recent differentiation between two biologically distinct endotypes (hypoplastic vs. degenerative) shifts the therapeutic focus from purely symptomic pressure reduction toward targeted modulation of epithelial function and regeneration. Building upon

Advances in biomarkers for differential diagnosis of autoimmune and autoinflammatory inner ear disorders.

Autoimmune inner ear disease (AIED) represents a rare yet potentially reversible cause of sensorineural hearing loss, characterized by immune-mediated damage to cochlear and vestibular structures. Unlike autoimmunity, which is driven by adaptive immune responses and autoantibodies, autoinflammation stems from innate immune system dysregulation, typically involving inflammasome hyperactivation. This review synthesizes recent advancements in biomarkers for the differential diagnosis of AIED, emphasizing their role in distinguishing it from conditions such as Meniere's disease (MD) and chronic ot

The Gut Microbiome May Play a Role in the Pathogenesis of Meniere's Disease.

Meniere's disease (MD) was first described 650 years ago. It is now considered to be a multifactorial disorder involving immunological mechanisms, blood-labyrinth barrier breakdown, endolymphatic hydrops, vascular compromise, and genetic susceptibility. Chronic inflammation from both innate and adaptive immunity is evident in the inner ear, with autoimmunity and allergy possibly playing a role. Despite its long history, significant knowledge gaps in its pathogenesis remain. For example, there may be root causes from elsewhere that are contributing to these pathological processes occurring in t

Increased Adrenergic Receptor Expression in Meniere Disease.

HYPOTHESIS: There is increased expression of adrenergic receptors in the vestibular end organs of Meniere disease. BACKGROUND: Meniere disease (MD) is an inner ear condition characterized by episodic vertigo, hearing loss, tinnitus, and aural fullness. It is believed to be due to an imbalance in inner ear fluid, often leading to hydrops; however, the exact mechanism is unclear. Pupillometry data have suggested a role for autonomic dysfunction, specifically sympathetic hyperactivity, but few studies have investigated the molecular pathways involved in this. METHODS: Vestibular end organs from

Conversational Large Language Models for Vestibular Diagnosis in Outpatient Clinics: Prospective Multicenter Diagnostic Accuracy Study.

BACKGROUND: Vestibular disorders are common, burdensome, and frequently misdiagnosed, particularly in nonspecialist settings where history-taking is often incomplete or inconsistently structured. Digital health tools that standardize symptom elicitation could improve diagnostic triage, but most existing systems rely on static questionnaires or rule-based logic. Large language models (LLMs) offer a more flexible alternative through adaptive, natural-language consultations, but prospective evidence from real clinical workflows remains scarce. OBJECTIVE: This study aimed to benchmark LLM diagnos

Neurovascular-Immune Biomarkers in Ménière Disease: Insights From High-Throughput Proteomics.

BACKGROUND: Ménière disease (MD) is a chronic inner ear disorder characterized by recurrent vertigo, fluctuating sensorineural hearing loss, tinnitus, and aural fullness. Diagnosis is clinical, and no validated peripheral biomarkers are available. Although endolymphatic hydrops is a hallmark, its poor correlation with symptom severity suggests broader systemic mechanisms. METHODS: We performed large-scale serum proteomic profiling using the Olink Explore HT platform, quantifying 5400 proteins in patients with definite MD and matched controls. After quality control, 1037 proteins were analyzed

ZDHHC5 may regulate the function of NKT cells and the immune response in Meniere's disease.

Meniere's disease (MD) is a heterogeneous, rare inner ear disorder characterized by recurrent vertigo, fluctuating sensorineural hearing loss, tinnitus, and aural fullness. Its molecular mechanisms remain unclear due to significant clinical and immunological heterogeneity. In this study, we integrated multi‑omics genetic data to systematically screen for palmitoylation regulatory genes associated with MD risk. We first intersected 31 core palmitoylation genes with blood cis‑eQTL datasets to identify candidate expression‑related genes, then performed two‑sample Mendelian randomization (MR) to e
