Elevated interictal plasma CGRP in Meniere's disease: A headache-independent candidate biomarker distinguishing Meniere's disease from vestibular migraine.

ObjectiveTo test whether interictal plasma or salivary calcitonin gene-related peptide (CGRP) distinguishes Ménière disease (MD) from vestibular migraine (VM), chronic migraine (CM), and healthy controls (HC).MethodsAdults with MD (n = 53), VM (n = 81), CM (n = 73), and HC (n = 59) provided morning interictal plasma and saliva. CGRP was measured in duplicate by ELISA. The primary contrast was plasma CGRP in MD versus VM; secondary analyses included multiplicity-corrected pairwise contrasts and ROC analysis.ResultsPlasma CGRP differed among groups (p < 0.001). Plasma CGRP in MD was higher than

Observation of MRI-Based Endolymphatic Hydrops and Clinical Symptoms in Patients With Definite Meniere's Disease Over Time: A Case Series.

PURPOSE: The assessment of endolymphatic hydrops (ELH) with specific MRI sequences underlines the ability to evaluate intralabyrinthine changes objectively and radiologically. The visualization of ELH by MRI allows correlations to be made with clinical patterns and supports the diagnosis of Ménière's disease (MD). Because MD cannot be assumed to be a static clinical condition, follow-up observations are of particular clinical interest. Our examination aimed to compare the grading of MRI-based ELH in patients over time, considering clinical symptoms and possible interventions. MATERIAL AND MET

Evaluation of neural transmission in implanted patients with Meniere's disease.

INTRODUCTION: Meniere's disease (MD) is characterised by fluctuating hearing loss. When hearing loss is severe or profound, rehabilitation requires cochlear implantation. The aim of this retrospective study was to compare cochlear implant (CI) fittings and electrode impedances between patients implanted because of MD and those with other aetiologies. MATERIALS AND METHODS: This study involved patients who received cochlear implantation with MED-EL CI at our centre between 2005 and 2022. We first compared speech recognition in a quiet environment at one year after cochlear implantation. We the

Labyrinthectomy and Cochlear Implantation in Patients With Meniere Disease: A Systematic Review.

OBJECTIVE: Ménière disease (MD) may progress to disabling vertigo and severe hearing loss despite conservative management. In patients with nonserviceable hearing, labyrinthectomy can provide effective vertigo control, while cochlear implantation (CI) offers the potential for auditory rehabilitation. This systematic review aimed to evaluate audiological, vestibular, tinnitus-related, and postoperative outcomes following labyrinthectomy combined with CI in patients with MD. DATABASES REVIEWED: MEDLINE/PubMed, Scopus, and Embase. METHODS: A systematic review with qualitative synthesis was cond

Pressure Therapy in Bilateral Ménière's Disease: A Prospective Study.

BACKGROUND: Ménière's disease (MD) is an inner ear disorder characterized by unpredictable vertigo attacks, tinnitus, and aural fullness, ultimately leading to a decline in audiovestibular function. Linked to endolymphatic hydrops (EH), MD often progresses to bilateral involvement (BMD). When conservative therapies fail, MD requires destructive treatments. However, this is a high-risk choice in bilateral cases for hearing preservation. Pressure therapy (PT), a non-invasive treatment using low-pressure pulses to the middle ear, has shown potential in reducing EH and alleviating MD symptoms. Thi

Lateral Semicircular Canal Occlusion in Patients with Refractory Meniere's Disease.

BACKGROUND: Meniere's disease remains a controversial condition not only in terms of its etiology and diagnosis but also its treatment. Approximately 2%-5% of patients do not improve or reach the control of their vestibular symptoms with any pharmacological therapy. Surgery of lateral semicircular canal (LSCC) occlusion could be an alternative to control the symptoms. METHODS: Twenty consecutive patients with clinical and oto-neurological diagnosis of Meniere's disease were included. Twelve women and 8 men with a mean age of 51 years (range, 29-72) were studied. All affected with unilateral o

Association of Bilateral Vestibulopathy With the ZNF91/LINC01224 Locus: A Genome-Wide Association Study.

BACKGROUND AND OBJECTIVES: Bilateral vestibulopathy (BVP) is a chronic disorder resulting from bilateral loss of peripheral vestibular function. Although several etiologies have been identified, such as ototoxic exposure, bilateral Menière disease, and, more recently, monogenic causes including RFC1-related disorders and FGF14-GAA ataxia (SCA27B), a substantial proportion of cases remain idiopathic. METHODS: We performed a genome-wide association study (GWAS) in a European cohort of 132 individuals with idiopathic BVP and 3,410 unaffected controls. Clinical and demographic data were collected

Integrative genomic and single-cell transcriptomic analyses suggest a putative zinc transport-immune axis in Meniere's disease.

OBJECTIVE: Meniere's disease is a disabling inner-ear disorder whose molecular basis remains poorly understood. Immune dysregulation and genetic susceptibility have been implicated in disease etiology, but the genes and immune-cell programs linking genetic susceptibility to disease-associated molecular alterations remain incompletely defined. Focusing on zinc transport and immune signaling, we aimed to prioritize genetically supported candidate genes and identify the immune-cell programs in which they are embedded. METHODS: We combined bulk peripheral blood transcriptomes, single-cell RNA seq

Role of platelet-rich plasma in management of sensorineural hearing loss diseases.

UNLABELLED: Sensorineural deafness is a form of loss of hearing. It happens from damage to the cochlea. Platelet-rich plasma (PRP) is an autologous management derived from a case's blood and was observed to have pro-nerve regenerative characteristics. Intratympanic injection of PRP improves the cells of the hair of the inner ear, therefore enhancing the hearing. OBJECTIVE: To show the effectiveness and safety of PRP in the management of sensorineural hearing loss (SNHL) diseases. MATERIAL AND METHODS: 150 patients who had sensorineural hearing loss because of different otological diseases, s

Functional overload and adaptive failure during dual-tasking in Ménière's disease.

BackgroundMénière's disease (MD) impairs cognitive function, yet the dynamics of cognitive-motor interference are not well understood. This study investigated dual-task performance in MD, hypothesizing a greater performance cost for patients compared to healthy controls under combined cognitive and postural loads.MethodTwenty-nine patients with MD and 29 healthy controls participated in a dual-task paradigm that combined computerized dynamic posturography (CDP) with a cognitive battery assessing visuospatial, executive, and working memory functions. Performance was measured across Sensory Orga

[Electrocochleography with an extratympanic tympanic membrane electrode : Age-dependent reference values for vertigo diagnostics].

BACKGROUND: Electrocochleography (ECochG) records electrical potentials of the inner ear and is used, among other applications, in the diagnostic work-up of Ménière's disease. For extratympanic recordings, age-dependent reference values are currently lacking, which limits clinical interpretation. OBJECTIVE: This study aims to establish age-stratified reference ranges for the summating potential (SP)/action potential (AP) amplitude and area ratios for noninvasive diagnostics in patients with suspected endolymphatic hydrops. MATERIALS AND METHODS: A total of 47 ears from 26 normal-hearing volu

Mouse models and translational research progress of hereditary vestibular dysfunction.

BackgroundHereditary vestibular dysfunctions (HVDs) are a group of diseases caused by genetic mutations, characterized by congenital or progressive vestibular dysfunction, often accompanied by hearing loss or other systemic damages. These diseases are divided into syndromic (e.g., Usher syndrome, CHARGE syndrome) and non-syndromic types, involving mutations in key genes such as MYO7A, COCH, SLC26A4, TMC1, etc. Although clinical phenotypes vary, the pathogenesis is complex, traditional diagnostic methods are limited, and effective treatments are lacking. Mouse models are important tools for stu
