What Do Vestibular Tests Reveal in Vestibular Migraine?

INTRODUCTION: Vestibular migraine (VM) is a common cause of recurrent vertigo, yet no single vestibular test is specific to it. This study aimed to evaluate videonystagmography (VNG), caloric test, and video head impulse test (vHIT) findings in VM patients and to examine the relationships between these tests. METHODS: This retrospective study included 193 patients with definite VM diagnosed per Bárány Society/International Headache Society criteria (January 2016-May 2026). All patients underwent VNG, caloric testing, and vHIT on the same day, in all cases outside an acute attack. Oculomotor,

The molecular and genetic mechanisms of the vestibular disorders: A mini review.

The vestibular disorders, which generally manifest themselves as nausea, vomiting, and dizziness or vertigo, have troubled many people. Increasing evidence suggests that certain vestibular disorders are hereditary, and gene variants contribute to the onset and susceptibility of these disorders. This review summarizes the latest research progress, especially the molecular and genetic aspects of several common vestibular disorders, including Meniere's disease, vestibular migraine, benign paroxysmal positional vertigo, and motion sickness, with the aim of encouraging more fundamental research and

Flunarizine and cinnarizine in the prophylaxis of vestibular migraine and migraine-associated menière's disease: An 8-month retrospective study.

BACKGROUND: Vestibular migraine (VM) and migraine-associated Menière's disease (MAMD) are common causes of recurrent vertigo, often resulting in substantial disability and impaired quality of life. Preventive therapy is challenging, and calcium channel antagonists such as flunarizine and cinnarizine have been proposed as effective options, though comparative evidence remains limited. METHODS: This was a retrospective cohort study including 402 consecutive patients diagnosed with VM (n = 298) or MAMD (n = 104) at two vestibular specialized centers. Patients received either flunarizine (10 mg/d

Formulation development and evaluation of nasal in situ gel of promethazine hydrochloride- part II: Pharmacokinetic evaluation and bioavailability studies.

Promethazine hydrochloride, a first-generation H1 receptor blocking agent, possesses antimuscarinic, sedative, and serotonin antagonistic properties. It is widely used as an antiemetic for treating nausea and vomiting caused by motion sickness, vertigo, postoperative and drug-induced vomiting, and Meniere's disease. Although extensively absorbed after oral administration, it undergoes significant first-pass metabolism, resulting in poor bioavailability of about 25 %. To overcome this limitation and improve patient compliance, the nasal route was explored owing to its high vascularity, large su

Wood type influences tree responses to recurring hot and dry summer tree water deficits across European forests.

Ultrasound elastography is a promising new technique in the evaluation of the thyroid nodule. It allows for "virtual palpation" of the nodule, even smaller ones which may not be otherwise palpable clinically. Ultrasound elastography was developed to obtain information on tissue stiffness non-invasively. Due to the superficial location of the thyroid gland, lack of sonological interferences like bowel gas shadow, easy availability of USG, and no need for patient preparation, it is easy and feasible to obtain information regarding the stiffness in the organ or nodule objectively, even by beginne

Association between vestibular dysfunction and osteoporosis in adults: a systematic review and meta-analysis.

Multiple clinical studies have demonstrated that vestibular dysfunctions, such as benign paroxysmal positional vertigo (BPPV) and Meniere's disease (MD), are associated with osteoporosis or osteopenia in adults. Animal studies have also indicated that selective ablation of peripheral vestibular organs leads to reduced bone mineral density in mice, potentially through effects on the sympathetic nervous system. This review systematically evaluates the association between vestibular dysfunction and osteoporosis in adults and reports the prevalence of this comorbidity. We systematically searched P

Aripiprazole as second-line pharmacotherapy for a heterogeneous cohort of refractory functional dizziness disorders: a pilot preliminary retrospective study.

BACKGROUND AND OBJECTIVE: First-line serotonergic antidepressants achieve clinical response in approximately 60-65% of patients with persistent postural-perceptual dizziness (PPPD), leaving a substantial refractory population. Aripiprazole, a serotonin-dopamine modulator with partial D2 and 5-HT1A agonism and 5-HT2A antagonism, may address dopaminergic and emotional dysregulation insufficiently controlled by serotonergic agents alone. This study evaluated the preliminary efficacy and tolerability of aripiprazole as second-line pharmacotherapy in patients with refractory functional dizziness di

Temporal enhancement patterns on 3D-FLAIR MRI in labyrinthine infarction.

BACKGROUND: Inner ear enhancement on contrast-enhanced three-dimensional fluid-attenuated inversion recovery (3D-FLAIR) MRI reflects disruption of the blood-labyrinth barrier (BLB) and has been reported in several audiovestibular disorders. However, the temporal enhancement pattern in labyrinthine infarction remains poorly characterized. METHODS: We investigated time-dependent enhancement patterns on 3D-FLAIR MRI in four patients with labyrinthine infarction secondary to anterior inferior cerebellar artery infarction. All patients underwent comprehensive neuro-otological evaluation and contra

Bilateral hearing levels in Meniere's disease may fluctuate simultaneously.

BACKGROUND: We observe fluctuations in hearing thresholds in both ears of patients with unilateral Meniere's disease (MD), even during early stages. If hearing fluctuations on both sides are synchronized, this may be related to the pathophysiology of MD. OBJECTIVES: To examine whether hearing fluctuations in the affected and non-affected ears occur synchronously in patients with MD. METHODS: We retrospectively analyzed 2985 pure-tone audiometry tests performed in 217 patients with definite MD. Based on pure-tone audiometry test results, we calculated average thresholds for low-, middle-, and

[Asymmetric low-frequency hearing loss as possible cause of pressure sensation in Menière's disease].

OBJECTIVE: The unilateral, low-frequency hearing loss typical of Menière's disease (MD) is often accompanied by a sensation of pressure in the ear. The pathophysiological mechanism remains unclear. The aim of this exploratory study was to investigate whether unilateral low-frequency hearing loss leads to the onset of a sensation of pressure and thus constitutes a possible mechanism for this symptom. MATERIAL AND METHODS: To simulate Menière's disease-typical hearing loss, 20 normal hearing subjects were presented binaurally with sentences of the Oldenburg Sentence Test (OLSA) with unilateral

The vestibular system: Unsteadiness, dizziness, and vertigo.

This chapter provides a detailed overview of the vestibular system's anatomy, physiology, and clinical relevance, emphasizing how inner-ear sensory structures and brainstem pathways interact to maintain balance, gaze stability, and spatial orientation. It begins with the peripheral vestibular apparatus, describing the semicircular canals and otolith organs in detecting angular and linear accelerations. These signals are transmitted via the vestibulocochlear nerve to the vestibular nuclei at the pontomedullary junction, where they integrate with visual and somatosensory inputs to drive coordina

Is the magnetic resonance imaging contrast agent gadoteric acid ototoxic?

OBJECTIVE: Contrast Magnetic Resonance Imaging (MRI) reveals endolymphatic hydrops in both symptomatic and asymptomatic ears. Radiological visualization of hydrops is important in the diagnosis of Meniere's disease; however, the ototoxicity of intratympanically administered gadoteric acid remains unknown. To investigate the ototoxicity of the intratympanic administration of gadoteric acid at different doses using an Auditory Brainstem Response (ABR) test and histopathological methods. Endolymphatic hydrops in patients with migraine: a correlative study.

OBJECTIVE: Endolymphatic hydrops (EH), Menière's disease (MD) pathological hallmark, is visualized via 3 T delayed phase MRI. This study examines MRI correlations between EH severity and migraine brain biomarkers. METHODS: Retrospective review of 144 inner ear MRI scans yielded 108 patients with confirmed EH. Two blinded radiologists graded EH, assessed migraine markers (white matter lesions (WML), frontal predominance of WML, enlarged perivascular spaces), and measured cranial nerve volumes. Binary logistic regression tested associations. RESULTS: Mean age 53.6 ± 15.8 years (56.5% female).

Cochlear Functional Changes and CGRP Modulation in a Chronic Nitroglycerin Migraine-Like Rat Model.

Auditory and vestibular symptoms in vestibular migraine, cochlear migraine, and Ménière's disease overlap, yet mechanisms linking migraine biology to inner ear dysfunction remain unclear. Calcitonin gene-related peptide (CGRP) is a key mediator of trigeminovascular activation and a major therapeutic target in migraine, but its contribution to cochlear dysfunction is not well defined. Using a repeated nitroglycerin (NTG) paradigm in male Wistar rats, we investigated whether a systemic migraine-like state affects cochlear function and whether these effects are dependent on CGRP. Rats received sa

Home-based video-oculography during spontaneous vertigo attacks: A proof-of-concept feasibility study in Ménière's disease and vestibular migraine.

BackgroundEpisodic vestibular disorders like Ménière's disease (MD) and vestibular migraine (VM) pose diagnostic challenges because patients are often assessed between attacks, when clinical signs are absent. Recording ictal eye movements could provide objective phenotypic data; however, the practicality of using unsupervised home-based video-oculography in everyday clinical settings remains poorly studied.MethodsWe conducted a prospective proof-of-concept study in patients with episodic vertigo. Participants were given portable, wireless video-oculography goggles for unsupervised home use and

Genome-wide analysis implicates inner ear development in Ménière's disease.

Ménière's disease (MD) is a chronic inner ear disorder characterized by recurrent vertigo, fluctuating sensorineural hearing loss, and tinnitus. Despite these distinctive symptoms, its etiology remains poorly understood. We performed a genome-wide meta-analysis of 8,969 cases and 1,962,542 controls across five large biobanks, identifying five independent genome-wide significant loci and estimating an observed-scale SNP heritability of 7% (SE 0.8%), consistent with a modest but significant genetic contribution to MD risk. Fine-mapping and integrative functional analyses implicate two convergent
