Macrophage-derived CXCL8 as a mediator of inflammatory attacks in Meniere's disease.

BACKGROUND: Ménière's disease (MD) is a complex disorder whose pathogenesis extends beyond endolymphatic hydrops to involve dysregulated immune responses. While a subset of patients exhibits a "low-cytokine phenotype" during remission, the mechanisms underlying the transition to acute inflammatory attacks triggered by environmental factors remain poorly understood. METHODS: We employed an integrative multi-omics approach to explore the immune microenvironment of MD. This included bioinformatic analysis of differentially expressed genes (DEGs) from GSE109558, featuring PBMCs from MD patients a

The Impact of Genetic Factors in Ménière's Disease.

Ménière's disease is an idiopathic disorder of the inner ear whose causes and pathogenetic mechanisms remain insufficiently elucidated. Advances in genetic analysis technologies have shifted attention toward the role of hereditary components in Ménière's disease, generating a considerable number of studies investigating the relationship between human genetic variations and disease onset. The results of these investigations highlight the complex and heterogeneous nature of pathogenesis, involving both genetic and epigenetic alterations. Studies have identified multiple candidate genes involved

STING signaling in vestibular macrophages underlies Ménière's disease pathogenesis.

BACKGROUND: Ménière’s disease (MD) is a disabling inner-ear disorder. Although the key pathologic alteration of MD is characterized as endolymphatic hydrops (EH), the underlying pathogenic mechanism remains unclear, and no definitive curative treatments are available. Emerging evidence implicates oxidative stress and innate immunity as key contributors to MD pathogenesis. METHODS: Human vestibular end organs were collected to characterize the reactive oxygen species (ROS) levels and the STING signaling activation, findings that were further corroborated in an LPS-induced EH mouse model. GATA4-Driven Transcription of HtrA1 Promotes Cellular Senescence in Ménière's Disease and Age-Related Audio-Vestibular Dysfunction.

Ménière's disease (MD), a chronic inflammatory disorder with age-related increased incidence, exhibits poorly understood pathogenesis and limited therapeutic options. Here, we demonstrate that cellular senescence, marked by mitochondrial damage, reactive oxygen species accumulation, and senescence-associated secretory phenotype (SASP), is prevalent in the vestibular tissue of MD patients and an endolymphatic hydrops mouse model. The transcription factor GATA4 is upregulated in MD and mice, and its genetic deletion in hair cells alleviates LPS-induced audio-vestibular dysfunction and cellular s

Causal cross-trait mapping at single-cell resolution identifies shared immunogenetic drivers of migraine and Meniere's disease.

BACKGROUND: Migraine and Meniere’s disease (MD) show high clinical comorbidity, shared symptoms such as vertigo and overlapping mechanisms like neurogenic inflammation suggest common pathophysiology. However, the core immunogenetic drivers underlying this comorbidity, particularly at cellular resolution, remain uncharacterized. METHODS: We integrated cross-trait genetic analyses using summary statistics from large genome-wide association study (GWAS) and single-cell expression quantitative trait locus (sc-eQTL), followed by Bayesian colocalization. Candidate genes were validated with independe

Cochlear Neural Degeneration Is Widespread in Ménière's Disease: A Temporal Bone Study.

PURPOSE: Ménière's disease presents with endolymphatic hydrops and disproportionately poor word-recognition compared to other forms of sensorineural hearing loss, yet the underlying patterns of cochlear degeneration are not well defined. This study aimed to characterize cochlear pathology in Ménières, compare it to age-matched controls and clinically unaffected contralateral ears, and evaluate how hydrops severity and clinical endotypes relate to tissue degeneration. METHODS: We analyzed 97 human cochleas, including 43 Ménière's ears, 10 contralateral (clinically unaffected) ears, and 44 age-

Rare missense variants in MYO7A and OTOP2 genes in a South Korean Meniere's disease cohort.

BACKGROUND: Meniere's disease (MD) is a polygenic condition defined by episodes of vertigo associated with sensorineural hearing loss and tinnitus. Genetic studies in familial MD in East Asian populations are limited, and the potential MD genes remain to be established in non-Finnish European populations. METHODS: By exome sequencing and rare variant analysis, we have searched for existing and novel genes associated with MD in a South Korean cohort of 16 MD individuals with bilateral sensorineural hearing loss. RESULTS: We have found one individual with two rare missense variants in the OTOP

Can video head impulse testing aid differential assessment of suspected vestibular migraine attacks?

Vestibular migraine is a clinically defined disorder characterized by recurrent vestibular symptoms associated with migraine features, and its acute attacks may be difficult to distinguish from peripheral vestibular disorders such as acute unilateral vestibulopathy/vestibular neuritis, Ménière's disease, and vestibular schwannoma. This review synthesizes current evidence on the clinical interpretation and proposed pathophysiological context of video head impulse test findings during acute vestibular migraine episodes, with particular attention to vestibulo-ocular reflex gain, refixation saccad
