Inner Ear Test Battery Aligned with Hydrops Magnetic Resonance Imaging to Differentiate Acute-Onset Sensorineural Hearing Loss.

INTRODUCTION: This study differentiates cases of acute-onset sensorineural hearing loss (SNHL) that did not meet the diagnostic criteria for either sudden SNHL or Meniere's disease, leading to a diagnostic challenge. METHODS: All 46 patients with acute-onset SNHL underwent an inner ear test battery followed by hydrops magnetic resonance imaging (MRI). Twenty-four patients (28 ears) who tested positive for endolymphatic hydrops (EH) on hydrops MRI were referred to the EH group. Another 22 patients (22 ears) who tested negative for EH were classified as the non-EH group. RESULTS: Significant d

Ménière's Disease: A Tri-Decade Scoping Review of Treatment Trends and Evidence Quality.

INTRODUCTION: Ménière's disease (MD) is a complex inner ear disorder defined by endolymphatic hydrops and a triad of episodic vertigo, fluctuating hearing loss, and tinnitus. Despite AAO-HNS diagnostic guidelines, disease heterogeneity and lack of treatment consensus persist. OBJECTIVE: This study conducts a tri-decade scoping review (1994-2024) to evaluate the MD evidence base, shifts in management, and methodological rigor regarding AAO-HNS and CONSORT compliance. MATERIALS AND METHODS: A systematic search of PubMed, CINAHL, and Cochrane (2014-2024) identified English-language RCTs and pro

Simultaneous Cochlear Implantation and Labyrinthine Surgery for Meniere Disease: Endolymphatic Sac Decompression Versus Labyrinthectomy.

INTRODUCTION: Patients with medically refractory Meniére disease (MD) may present with concomitant vertigo attacks and severe-profound hearing loss (SNHL). In such cases, simultaneous labyrinthectomy/cochlear implantation (CI) is a viable treatment option. Simultaneous endolymphatic sac decompression (ESD) and CI is less well described but represents a promising alternative. This study aims to analyze outcomes after simultaneous or staged ESD/CI as compared with simultaneous labyrinthectomy/CI. MATERIALS AND METHODS: MD patients undergoing simultaneous or staged ESD/CI and simultaneous labyri

Long-term hearing outcomes and prognostic factors in Meniere's disease.

BACKGROUND: While the management of vertigo in Meniere's disease (MD) has been extensively studied, there is limited understanding of the long-term hearing prognosis and associated risk factors. AIMS/OBJECTIVES: To assess the long-term hearing outcomes and identify key factors influencing hearing prognosis in patients with MD. MATERIALS AND METHODS: A retrospective review was conducted on 159 patients (179 affected ears) with MD, with a minimum follow-up of six months. Hearing outcomes were classified based on four-tone (500, 1000, 2000, and 3000 Hz) averages (FTA) into stages. Univariate an

Association between hearing severity and hematological findings at initial presentation in Meniere's disease.

BACKGROUND: Meniere's disease (MD) is characterized by progressive hearing loss; however, the factors determining its severity remain unclear. AIMS/OBJECTIVES: To investigate the clinical and hematological factors associated with hearing severity in MD. MATERIALS AND METHODS: This retrospective study included 112 patients with unilateral MD. Patients with bilateral disease or contralateral hearing loss ≥40 dB were excluded. Hearing severity was classified using AAO-HNS criteria based on pure-tone average (0.5, 1, 2, and 4 kHz), and patients were categorized into non-severe (stages I-II) and

Characterizing nystagmus using a portable eye movement recorder in patients with Meniere's disease and MRI-confirmed endolymphatic hydrops.

OBJECTIVE: Meniere's disease (MD) causes episodic vertigo and fluctuating hearing loss. During attacks, nystagmus may appear either toward (i.e., irritative) or away from (i.e., paralytic) the affected ear, but it is difficult to observe these patterns in real-time. This study captured nystagmus using a portable eye movement recorder (PEMR) at the onset of attacks and analyzed its characteristics in patients with endolymphatic hydrops (EH) confirmed on contrast-enhanced magnetic resonance imaging (CE-MRI). METHODS: This study enrolled 54 patients with MD diagnosed using the Japan Society for

Cochlear symptoms and MRI findings in vestibular migraine: a narrative review.

PURPOSE: Menière's disease (MD) and vestibular migraine (VM) are two of the most common episodic vertigos. Both are diagnosed by clinical history, manifestations and for MD a typical pattern of hearing loss audiometrically confirmed is needed. The description of concomitant cochlear symptoms during acute VM attacks makes the differential diagnosis between the two conditions challenging. The aim of this study was to assess if the presence of cochlear symptoms withing vertigo attacks always address towards MD or if they may also be found in definite VM. METHODS: We performed a narrative review

Tinnitus.

Tinnitus is the perception of sound without a corresponding external sound source. This condition affects approximately 14% of adults, with approximately 2% experiencing severe symptoms. Underlying mechanisms of tinnitus suggest involvement of both peripheral and central processes, in which cochlear injury and deafferentation may trigger maladaptive plasticity, increased central gain, and thalamocortical dysrhythmia, modulated by limbic and salience networks. Neuroinflammation, somatosensory-auditory coupling and other factors, such as stress, may contribute to chronicity. Clinical expression

Lymphatic and Glymphatic Alterations in Auditory Disorders: A Rapid Review-Informed Systematic Review and Meta-Analysis.

Background and Objectives: The inner ear has traditionally been regarded as an immunoprivileged and anatomically isolated organ. However, growing interest in neuro-lymphatic interactions has raised the hypothesis that glymphatic and lymphatic mechanisms may contribute to auditory pathology and its association with cognitive dysfunction. This systematic review aimed to synthesize current human evidence regarding anatomical, imaging, and clinical correlates of glymphatic mechanisms in the inner ear and audiological pathologies, and to quantitatively evaluate currently available biomarkers. Burden of novel and ultra-rare missense variants in the NF-κB pathway genes associated to Ménière's disease.

INTRODUCTION: Ménière's disease (MD) is an inner ear disorder characterised by tinnitus, sensorineural hearing loss and dizziness, although around 60% of patients also exhibit and immune dysregulation. Transcriptomic studies have identified three distinct immune phenotypes in MD, classified based on systemic inflammation, cytokine profiles and association to autoinflammatory/autoimmune comorbidities. Furthermore, genetic studies have shown aggregation of rare variants in MD patients; however, the relationship between genetic factors and immune responses remains poorly understood. METHODS: In

Evolution of understanding and clinical practice of Ménière's disease.

Since Prosper Ménière's original description more than 160 years ago, concepts of Ménière's disease (MD) pathophysiology and treatment have evolved through distinct phases. For much of the twentieth century, the field was dominated by the hydrodynamic paradigm, which attributed symptoms to pressure-driven endolymphatic hydrops and episodic membrane rupture. Over the past three decades, however, accumulating pathological, physiological, genetic, and imaging evidence has undermined this model. This review traces the transition from the classic hydrodynamic era to a modern framework that conceptu
