Benign paroxysmal vertigo of childhood: a long-term follow-up.

We examined clinical aspects of Benign Paroxysmal Vertigo (BPV) in infancy and its most frequent differential diagnosis, in particular analogies and differences with forms of "migrainous vertigo" (MV) of later onset. During a long-term follow-up of 7 cases of BPV, diagnosed according to the Basser criteria, 5 of 7 BPV cases spontaneously resolved and 6 of 7 patients later developed migraine and other migraine-related symptoms. This course differs from that described for MV only in the age of onset of headache and in the chronological relationship with vertigo. The authors suggest that BPV can

Differential diagnosis of seizures.

Distinguishing epileptic events from nonepileptic paroxysmal neurologic events represents a common diagnostic challenge. Syncope, either cardiac or noncardiac, can appear similar to atonic and even convulsive seizures. Breath holding and benign paroxysmal vertigo in children may be confused with epilepsy. Classic migraine, transient global amnesia, and transient ischemic attacks may resemble epileptic seizures. Sleep disorders, including nocturnal movements, parasomnias, and narcolepsy also may resemble epileptic seizures. Most movement disorders are distinguished easily from epilepsy; however

[Vertigo in children].

The main difference between childhood and adulthood vertigo is that the evaluation of symptoms and examination pose special challenges in pediatric patients, depending on age. Vertigo in pre-teenage children is similar to vertigo in adults. In contrast, although benign paroxysmal vertigo, vestibular neuronitis, and Ménière's vertigo occur in children, the distribution of these conditions is different from that seen in adults. After vestibular and cochlear investigations as well as a CT scan or MRI study to outrule a tumor of the posterior cerebral fossa, the diagnosis of vestibular vertigo is

[Equilibrium disorders and their diagnosis in childhood].

Vertigo in children is rarer than in adults and the examiner in cases showing these symptoms must rely on the parents' or relatives' observations and details. Besides the equilibrium disorders caused by hereditary malabsorption or lesions in the peripheral or central vestibular structures, e.g. ototoxic drugs, tumours in the brain, meningitis, encephalitis, otitis, labyrinthine fistulas or head trauma, we only known of typical diseases, associated with vertigo, that develop during childhood. These are: so-called benign paroxysmal vertigo of childhood, benign paroxysmal torticollis, basilar mig

[Benign paroxysmal vertigo: comparison of 2 rehabilitation methods].

In case of non-self regression of Benign Paroxysmal Positional Vertigo (BPPV), reeducation is mandatory. Two reeducation protocols have been compared on 32 subjects with BPPV. Fourteen were handled by the classic habituation exercises, and 18 by the more recent Semont's manoeuvre. Moreover a specific rotatory nystagmus habituation has been investigated by caloric stimulation in Brunning III position. Semont's technique shows quicker and more efficient results. This manoeuvre doesn't provoke any habituation, in accordance with the hypothesis of mechanical dispersion of otoconia, based on the cu

[Benign paroxysmal vertigo in children].

Based on our examination of 176 patients, we first selected, according to the general standards of diagnosing benign paroxysmal vertigo of childhood (children aged up to ten years, paroxysmal vertigo, ataxia) a group of 78 patients. Using multidisciplinary approach we have thus performed in these children the following examinations: an exhaustive anamnesis, laboratory examinations, clinical ORL (ENT), ophthalmological, neuropediatric examinations, x-ray of the cranium and the temporal bone, audiometric, vestibulogenic and EEG examinations. In the following stage we have, on the basis of the ob

Benign paroxysmal vertigo of the horizontal canal.

Over a period of 4 years we observed 15 cases of benign positional vertigo (BPV) probably caused by deposition of otoliths in the horizontal semicircular canal. Rapid rolling onto one side in recumbent position provokes a paroxysmal, purely horizontal and geotropic nystagmus which nearly always spontaneously inverts direction. Rolling the patient onto the other side provokes a left intense geotropic nystagmus. BPV is more violent but resolves more rapidly than that of the better known posterior canal positional vertigo. Sometimes both syndromes are present together.

Vertigo in the pediatric and adolescent age group.

Since episodic vertigo in the pediatric and adolescent age group is unusual and therefore not well known to most otolaryngologists, we present six cases to show some of the various presentations and different underlying causes. These cases covered a wide diagnostic spectrum: meningioma, medulloblastoma, childhood migraine with vestibular symptoms, childhood Meniere's syndrome (one case due to perilymph fistula), and benign paroxysmal vertigo of childhood. The two patients with tumor and the patient with perilymph fistula were treated surgically; the other patients are being managed conservativ

[New aspects of differential vertigo diagnosis in childhood].

Eight cases of vertigo in childhood are reported. Four of them suffered from benign paroxysmal vertigo, and the other four from atypical migraine. History of a head injury and attacks of vertigo lasting only a few seconds are typical of benign paroxysmal vertigo. These children are older than 4 years and the vertigo is of peripheral origin. Vertigo due to atypical migraine is frequent in young children up to 4 years of age, shows a longer duration of vertigo (more than 1 min) and is of central origin.

Testing the vestibular-ocular reflexes: abnormalities of the otolith contribution in patients with neuro-otological disease.

Conventional vestibular rotation testing with the head centered on the axis stimulates the semicircular canals evoking compensatory eye movements. If the head is placed forwards of the axis in an eccentric position the otoliths are also stimulated by a tangential linear acceleration acting laterally to the skull. In normal subjects the additional otolithic stimulus evokes compensatory eye movements with a higher gain than with head centred, particularly for high frequency (greater than 0.1 Hz) stimuli. The responses with head centred and eccentric in various patients with known/suspected neuro

Benign paroxysmal vertigo of childhood.

Benign paroxysmal vertigo of childhood is a vestibular disorder characterized by multiple sudden brief episodes of true vertigo with nystagmus usually beginning after the age of 4. The diagnosis is based on the characteristic history, because the otologic and neurologic examinations, electronystagmogram, audiogram, and electroencephalogram are usually normal. The etiology of the disorder remains unknown, although a vascular disturbance of the posterior circulation with ischemia of the labyrinth or vestibular nuclei is most commonly postulated. A review of the literature and five new cases are

Horizontal canal BPV.

Benign paroxysmal vertigo (BPV) is generally attributed to a differential density condition in the posterior semicircular canal. Although the posterior canal is implicated because of its dependent position, the possibility exists that either the horizontal or superior canal could become involved. This paper reports on seven patients with a clinical picture consistent with horizontal canal BPV. The characteristic features are brief vertigo and horizontal nystagmus precipitated by head movement into or out of one of the lateral positions. Position change toward the left lateral position induces

Benign paroxysmal vertigo in childhood: a migraine equivalent.

The two main problems in defining and classifying the syndrome of benign paroxysmal vertigo (BPV) in childhood are the vestibular function pattern and the relationship between BPV and migraine. 13 children suffering from this syndrome were submitted to complete otoneurological examination, including caloric and rotational labyrinthine stimulation with ENG recording, and to headache provocation tests with nitroglycerin, histamine and fenfluramine. Vestibular responses were normal in all except 2 cases which presented signs of central vestibular impairment at the level of the vestibulocerebellar

Benign paroxysmal vertigo in childhood. Diagnostic significance of vestibular examination and headache provocation tests.

Sixteen children with benign paroxysmal vertigo (BPV) are presented. The great majority had a family history of migraine, neurological and autonomic signs associated with vertiginous attacks, and headache or other sign of the periodic syndrome (motion sickness, cyclic vomiting, abdominal pain) unrelated to the attacks. Vestibular examination, including bithermal caloric and rotational testing with ENG recording, showed normal or transiently decreased vestibular function. Headache provocation tests with nitroglycerin, histamine and fenfluramine were positive in 9 of the 13 patients examined, an

Dizziness in childhood.

Dizziness in childhood is not an infrequent symptom. Accurate history taking and close co-operation between otologist, paediatrician and neurologist are necessary in the approach to the dizzy child. Most cases of childhood dizziness settle in time and investigations should be carefully selected; those with severe and persistent dizziness or ataxia should be thoroughly investigated including: EEG, ENG, calorics and CT scan. The conditions causing dizziness in children are discussed and are illustrated with case histories from our series of 27 children. Dizziness of unknown aetiology, serous oti
