Rapid Reversal of Forearm Supinator Rigidity With Topical Isomerized Potassium Linoleate: A Novel Perspective on Microbiome-Induced Tetany.

Microbes can alter host behavior, immunity, and neurological function at a distance without extension into the brain and spinal cord. Clostridia provides a predicate for such an infection in the periphery by causing "lock jaw" and generalized tetany. This case series presents five patients who showed rigidity or tetany of the forearm. All were diagnosed with vertigo of central origin by video nystagmography (VNG) and posturography. Each had an apparent slow-rolling tetany, most visible in the forearm. Each had a consistent focus of pain within the supinator, diminution of extended wrist rotati

Adoptive NK cell transfer confers neuroprotection by attenuating neuroinflammation and alpha-synuclein pathology in a mouse model of synucleinopathy.

UNLABELLED: Clevidipine (Cleviprex), a late-generation dihydropyridine calcium channel antagonist available as a lipid emulsion for intravenous infusion, is approved in the US for the reduction of blood pressure (BP) when oral therapy is not feasible or desirable. Intravenous clevidipine is effective in the treatment of both acute preoperative and postoperative hypertension in adult cardiac surgery patients, and with a rapid onset and short duration of action the drug can be easily titrated for predictable BP control. Moreover, in terms of controlling acutely elevated BP in this patient popula

Vestibular disorders in patients after COVID-19 infection.

INTRODUCTION: The COVID-19 clinical symptoms are primarily related to the respiratory system but may also be involved in many others, including the nervous system. Recently, vertigo or dizziness has been described as one of the clinical manifestations and possible complications of COVID-19. MATERIALS AND METHODS: This clinical study was designed to describe the otorhinolaryngological evaluation and videonystagmographic (VNG) findings in patients with an antecedent of COVID-19 infection in the last 6 months. In this study, we sought to investigate the presence of persistent vestibular damage i

A Rare Case of a Large, Deceitfully Quiet Brainstem Arteriovenous Malformation Presenting Only as Dizziness.

Dizziness is one of the most frequent complaints encountered in the medical practice affecting 15%-20% of adults yearly, and can be challenging to assess. Most patients use dizziness as a non-specific term, and thus suffer prejudice from the physicians' end and can be disregarded frequently. Dizziness can be a symptom of various diseases, some with sinister pathologies. We present a case of garden-variety vertigo that unfurled to be not-so-simple, emphasizing the importance of a thorough history and physical examination again even in the era of technology. A 32-year-old male patient with no pa

Vestibular evoked myogenic potentials and video head impulse test in patients with vertigo, dizziness and imbalance.

The aim of this study was to compare vestibular evoked myogenic potentials (VEMP) and video head impulse test (vHIT) results in patients presenting with vertigo and dizziness. We retrospectively analyzed data of all patients with the chief complaint of vertigo, dizziness, or imbalance that underwent VEMP and vHIT from January 2015 to January 2016. A total of 117 patients (73 females, mean age 53.92±16.76) fulfilled inclusion criteria: group 1 included patients with the final diagnosis of vestibular neuritis (VN) (N=31 (16 right and 15 left VN)), group 2 included patients with the final diagnos

[Benign paroxysmal positional vertigo].

Benign paroxysmal positional vertigo (BPPV) is the most common cause of vestibular vertigo. It is caused by dislodged otoconia that enters the semicircular canals. Its main symptom is relapsing positional vertigo. The provocative tests, in particular the Dix-Hallpike maneuver, are used to confirm the diagnosis. In case of positive result of the test, one of the maneuvers is peformed depending on the affected semicircular canal, for instance the Epley maneuver. It is important to differentiate BPPV from the vertigo of central origin.

[Vestibular neuritis: treatment and prognosis].

Vestibular neuritis is a sudden unilateral peripheral vestibular deficit of unknown origin without associated hearing loss. It is the second cause of peripheral vertigo after Benign Paroxysmal Positional Vertigo (BPPV). The etiology remains unclear and some treatments are still controversial. The prognosis is good. The differential diagnosis of the disease mainly includes an acute vertigo of central origin. This article summarizes the management and prognosis of vestibular neuritis.

[Causes and diagnostics of dizziness of central origin in children].

Dizziness and vertigo are symptoms caused by several etiological factors, they are result of general diseases, within anemia. They are associated with several diseases of the organ of hearing and balance, which are under laryngological care, and one of most important tests to distinguish vertigo of central and peripheral origin is videonystagmography. Several diseases of the central nervous system cause instability of posture and gait. Psychogenic feeling of being insecure and unsafe makes the diagnostics more complicated. Vertigo of central origin in children can be a first symptom of severe

[Results of transcranial Doppler examination of cerebral blood flow in children with vertigo].

BACKGROUND: Episodic recurrent vertigo in the developmental age is associated in majority of cases with disorders of the labirynth, however they can be also associated with epilepsy or migraine. Less often they are result of disorders of vertebro-basillar circulation. The important tool in the diagnosis of such disorders is Doppler examination. Transcranial Doppler (TCD) allows detection of hypoplasia or vasospasm, it helps also in evaluation of process of autoregulation and hemodynamics of circulation in the stimulation tests. AIM: The aim of our research was evaluation of circulation in car

Differentiation between cerebellopontine angle tumors in cancer patients.

OBJECTIVE: The possibility of metastasis to the cerebellopontine angle should be considered when a cancer patient has inner ear-related symptoms, although such metastasis is rare. Distinguishing between an independent tumor and a metastasis presents a challenge to the clinician once magnetic resonance imaging reveals a space-occupying lesion in the cerebellopontine angle. This study attempted to differentiate between primary benign and metastatic malignant cerebellopontine angle tumors in cancer patients. SETTING: University hospital. PATIENTS: A total of 174 cancer patients with inner ear-r

Intravenous Lorazepam versus dimenhydrinate for treatment of vertigo in the emergency department: a randomized clinical trial.

STUDY OBJECTIVE: To determine whether lorazepam is more effective than dimenhydrinate in relieving the symptom of vertigo in the emergency department setting. METHODS: A prospective, randomized, double-blind trial of intravenous lorazepam versus dimenhydrinate therapy was conducted in the ED of a county-owned, university-affiliated hospital. All adult patients who presented between January 24, 1998, and May 23, 1999, with the symptom of vertigo were eligible for inclusion. The intervention was varying the intravenous treatment between lorazepam, 2 mg, and dimenhydrinate, 50 mg. All patients r

The effectiveness of piracetam in vertigo.

Vertigo is a sensation of altered orientation in space and may be defined as an illusion of movement. It is a subjective symptom and therefore difficult to assess. Examination and diagnosis remain difficult. Although treatment should be directed at the underlying cause or disorder, the origin of vertigo is frequently unknown or untreatable. Pharmacotherapy is required for symptomatic treatment. Piracetam has been shown to be effective in vertigo of both central and peripheral origin. It is thought to act on vestibular and oculomotor nuclei in the brain stem and thus on the central control of b

Approach to the dizzy patient.

As dizziness can be caused by so many different pathophysiological mechanisms, it is crucial to determine the type of dizziness before proceeding with the diagnostic evaluation. Vertigo, defined as an illusion of movement, is an important subtype of dizziness that indicates a lesion somewhere within the vestibular system. Probably the most useful feature for differentiating between peripheral and central causes of vertigo is the associated symptoms. Vertigo of peripheral origin is typically associated with auditory symptoms such as hearing loss and tinnitus, while vertigo of central origin is

Dyscalculia in patients with vertigo.

This paper reports a hitherto undescribed relationship between vertigo of central origin and dyscalculia. Subjects with vertigo skipped and displaced decades when counting backwards by two. The error is not recognized when presented visually. The subjects also display decrements in ability to do mental arithmetic and in central auditory processing. The results are discussed in light of the relationship between the central vestibular/auditory system and structures involved in higher cognitive function. The relationship between balance disorders and children with learning disabilities is also ex

Sudden vertigo of central origin.

Intense vertiginous attacks with sudden onset and of several months' duration were observed in a group of patients following viral infection. Neurotological examination revealed different types of signs of involvement of the central vestibular apparatus. All of these patients became dizzier when medicated with vestibular depressor drugs, and could obtain symptomatic relief with corticosteroids. They became asymptomatic after a period of 3-4 months from the onset of the disease, but the signs of central vestibular involvement persisted for 6-12 months.
