Visual and visual-proprioceptive training improve gait via sensory reweighting after chronic stroke: a randomized controlled trial.

BACKGROUND: Post-stroke gait recovery depends on multisensory integration, but vision- and gaze-stability training are rarely emphasized. We tested whether 12-week visual oculomotor training (VOT) or visual–proprioceptive integration (VPI) programs improve gait after chronic stroke. METHODS: In this single-center, assessor- and analyst-blinded randomized controlled trial, 45 participants were allocated to a control group (CG), VOT, or VPI. All groups received standardized gait training plus three 30-min group-specific sessions weekly for 12 weeks. Static and dynamic visual acuity were assessed

[New developments in inner ear imaging for Meniere's disease].

BACKGROUND: The current Bárány Society diagnostic criteria for Meniere's disease (MD) do not include radiological aspects. However, computed tomography (CT), native magnetic resonance imaging (MRI), and time-delayed contrast-enhanced inner ear MRI (iMRI) for visualization of endolymphatic hydrops (ELH) have been used in research for years. OBJECTIVE: The aim of this narrative review is to present the current state of research and provide an outlook on future developments. Furthermore, existing possibilities for quantifying inner ear imaging data are shown. MATERIALS AND METHODS: PubMed, MEDL

Ocular vestibular evoked myogenic potential late peaks are enhanced in superior canal dehiscence and prolonged in vestibular migraine.

BackgroundThe ocular vestibular evoked myogenic potential (oVEMP) n1 peak is the first of many that form part of the oscillating waveform. Previous work showed that up to 5 peaks can be elicited when an intact labyrinth is stimulated, with larger peaks contralaterally.ObjectiveWe investigated what happens to these later peaks when the n1 amplitude is enlarged, which is typically seen in superior canal dehiscence (SCD) and sometimes vestibular migraine (VM).MethodsWe measured oVEMPs elicited to air- (AC) and bone-conducted (BC) stimuli in 14 patients with SCD, 20 with SCD and classical migraine

The Treatment of Vertigo With a Digital Health App: Findings of the Prospective Randomized Controlled GEVE-I Trial.

BACKGROUND: Vestibular vertigo is common and frequently leads to inability to work. We studied the clinical efficacy and safety of a digital health app (VH90D) for the treatment of vestibular vertigo. METHODS: We conducted a confirmatory, prospective, single-blinded, randomized, group-controlled, single-center, two-arm superiority trial with 212 participants. 106 of them (the intervention group) were treated with the app, and the other 106 (the control group) received treatment as usual with standard physiotherapy. The primary endpoint was the vertigo symptom score (VSS-sf-VER) at 12 weeks. T

The Role of Biological Effective Dose in Gamma Knife Radiosurgery: A Systematic Review Across Multiple Indications.

Background: Gamma Knife radiosurgery (GKS) is widely used for the management of intracranial disorders. Emerging evidence suggests that incorporating the biological effective dose (BED) into GKS planning may improve the prediction of treatment efficacy and toxicity. This review aims to evaluate the role of BED in GKS across multiple intracranial indications. Methods: A qualitative review of published clinical studies was performed to assess the application of BED models in GKS for pituitary adenomas, vestibular schwannomas, meningiomas, arteriovenous malformations (AVMs), trigeminal neuralgia,

Automated vertigo diagnosis using video-oculography and artificial intelligence for clinic-based triage: Preliminary results.

BACKGROUND: Distinguishing dangerous from benign vertigo remains a diagnostic challenge. Our study aimed to develop and evaluate a machine learning model to differentiate between dangerous and benign vertigo in the outpatient setting across two medical institutions that used different equipment, testing methods, and protocols. METHODS: Patients in the dizziness clinics of Shuang Ho Hospital (SHH) and Taichung Tzu Chi Hospital (TTCH) were classified into "benign vertigo" and "dangerous vertigo" groups. The data of video-oculography (VOG) and video head impulse test were combined and preprocess

Bow and lean test reconsidered: anatomy, predictions, and a data visualization method for horizontal canal BPPV.

In the diagnosis of horizontal canal benign paroxysmal positional vertigo, it is challenging to lateralize the affected side. The "bow and lean test," and several methods built upon it, have been proposed. Many of these techniques are based on a model in which the horizontal canal's putative anatomy involves an anteromedial segment. The actual anatomy has a posteromedial segment. We show that predictions of a model based on the hypothetical anatomy differ from those based on the true anatomy. We further show that the true anatomical model predicts that for a roll maneuver that begins in the su

Functional overload and adaptive failure during dual-tasking in Ménière's disease.

BackgroundMénière's disease (MD) impairs cognitive function, yet the dynamics of cognitive-motor interference are not well understood. This study investigated dual-task performance in MD, hypothesizing a greater performance cost for patients compared to healthy controls under combined cognitive and postural loads.MethodTwenty-nine patients with MD and 29 healthy controls participated in a dual-task paradigm that combined computerized dynamic posturography (CDP) with a cognitive battery assessing visuospatial, executive, and working memory functions. Performance was measured across Sensory Orga

Vestibular Evoked Myogenic Potential in Vestibular Migraine: A Systematic Review of Diagnostic Utility.

Background/Objectives: Vestibular migraine (VM) is one of the most prevalent causes of episodic vertigo, yet it remains underdiagnosed due to overlapping features with other vestibular disorders and the absence of definitive diagnostic tests. Vestibular evoked myogenic potentials (VEMPs) assess otolith and vestibular nerve function and may help identify pathophysiological mechanisms in VM. This systematic review aimed to evaluate the usefulness of VEMP in understanding VM, synthesize existing findings, and explore its clinical implications. Method: A systematic search was performed in PubMed,

Intralabyrinthine MRI FLAIR as a predictive marker for hearing loss in vestibular schwannomas in Neurofibromatosis Type 2.

BACKGROUND AND OBJECTIVES: The onset of hearing loss due to vestibular schwannomas (VS) is inevitable but does not correlate with the size of the tumor. In patients with Neurofibromatosis Type 2 (NF2) and VS, we previously found an association between pre-contrast fluid-attenuated inversion recovery magnetic resonance imaging (FLAIR MRI) signal in the labyrinth and hearing loss. Here, we asked whether FLAIR hyper-intensity could serve as a predictive biomarker for hearing loss in NF2 patients with VS. METHODS: A prospective longitudinal study (NCT00598351) of NF2 enrolled 168 subjects between

Tension-Dominant Orthodontic Loading and Buccal Periodontal Phenotype Preservation: An Integrative Mechanobiological Model Supported by FEM and a Proof-of-Concept CBCT.

Background: Adult patients with a thin buccal cortical plate and fragile periodontal phenotype are at high risk of dehiscence, fenestration and recession during transverse orthodontic expansion. Conventional mechanics often create a cervical compression-dominant environment that exceeds the adaptive capacity of the periodontal ligament (PDL)-bone complex. Objectives: This study proposes an integrative mechanobiological model in which a skeletal-anchorage-assisted loading protocol (Bone Protection System, BPS) transforms expansion into a tension-dominant regime that favours buccal phenotype pre

Encephalitozoonosis in Pet Rabbits: Epidemiology, Pathogenesis, Immunology and Consensus on Clinical Management.

Members of the microsporidial genus Encephalitozoon have the capacity to infect both mammals and birds, and E. cuniculi is most commonly found in rabbits. With a seroprevalence ranging up to 85%, E. cuniculi can be a problem in pet rabbits as well as in food production and laboratory animal science. While most infections are likely subclinical, there are three main clinical presentations: neurological, renal, and ocular. Typical clinical signs including vestibular disease and phacoclastic uveitis may develop with initial or relapsing infection, while renal infection is usually progressive and

Clinical Soft Tissue Adaptation to Biomechanical Modulation with the Bone Protection System (BPS): A Two-Case Report in Thin-Biotype Patients.

Background: Patients with a thin gingival phenotype and a narrow buccal alveolar plate are highly susceptible to periodontal complications during orthodontic expansion. Traditional biomechanics often fail to maintain root control in thin alveolar housing. This report presents two clinical cases illustrating soft- and hard-tissue responses to a novel biomechanical approach, the Bone Protection System (BPS), designed to reduce buccal cortical overload during expansion. Case Presentation: Two adult patients with a thin gingival phenotype assessed by a standardized periodontal probe transparency t

Pre-operative blood-based inflammatory indices and long-term facial nerve outcomes following vestibular schwannoma resection.

OBJECTIVE: Inflammation contributes to tumor pathogenesis and growth in vestibular schwannoma (VS) patients, but associations with facial nerve outcome following surgical resection remain unclear. We aimed to evaluate whether blood-based inflammatory indices are associated with postoperative facial nerve outcomes following VS resection. PATIENTS AND METHODS: This retrospective cohort study included adult VS patients who consecutively underwent microsurgical resection at our institution between July 2016 and July 2024. Patients with neurofibromatosis, previous radiotherapy or resection, baselin

[Electrocochleography with an extratympanic tympanic membrane electrode : Age-dependent reference values for vertigo diagnostics].

BACKGROUND: Electrocochleography (ECochG) records electrical potentials of the inner ear and is used, among other applications, in the diagnostic work-up of Ménière's disease. For extratympanic recordings, age-dependent reference values are currently lacking, which limits clinical interpretation. OBJECTIVE: This study aims to establish age-stratified reference ranges for the summating potential (SP)/action potential (AP) amplitude and area ratios for noninvasive diagnostics in patients with suspected endolymphatic hydrops. Saliva as a Reliable and Non-invasive Sample for Detecting Influenza A in Severe Acute Respiratory Infection Cases.

Background Nasopharyngeal swab sampling remains the gold standard for influenza diagnosis; however, it has several limitations, including dependence on medical staff, invasiveness, potential for nosocomial transmission, and occupational exposure risk. Non-invasive alternatives, such as saliva and nasal vestibular swabs, may improve patient comfort and participation in clinical studies. In addition, diagnosis with reverse transcription real-time quantitative polymerase chain reaction (RT-qPCR) is often delayed because it requires trained laboratory technicians and facilities with appropriate la

HINTS+ to diagnose AICA stroke: systematic review of the diagnostic impact of acute unilateral hearing loss in the acute vestibular syndrome.

PURPOSE: Head-Impulse, Nystagmus, Test-of-Skew (HINTS) testing is recommended to identify stroke in the acute vestibular syndrome (AVS). Diagnostic accuracy depends on the vascular territory involved, with lower sensitivity for anterior-inferior cerebellar artery (AICA)- than posterior-inferior cerebellar artery (PICA)-territory strokes. Adding new-onset, acute unilateral hearing loss (AUHL) as a fourth central sign has been proposed ("HINTS+") to improve sensitivity for AICA-stroke detection. METHODS: We performed a systematic search (MEDLINE, Embase) to identify studies reporting diagnostic
