Homozygous RFC1 AAGGG Repeat Expansions Are Common in Idiopathic Peripheral Neuropathy.

OBJECTIVE: Biallelic intronic AAGGG repeat expansions in RFC1 cause cerebellar ataxia with neuropathy and vestibular areflexia syndrome and may also contribute to isolated sensory neuropathy. The clinical significance of both heterozygous and homozygous (biallelic) RFC1 expansions in more diverse patient populations remains unclear-partly due to the absence of accurate, user-friendly computational pipelines specifically tailored for tandem repeat analysis. METHODS: To discern the relationship between RFC1 expansions and idiopathic peripheral neuropathy (iPN), we performed whole-genome sequenc

Association of PHEX Gene Dosage With Meniere Disease and Related Audiovestibular Phenotypes in X-Linked Hypophosphatemia.

IMPORTANCE: Meniere disease (MD) is a heterogenous disorder whose underlying etiologies remain poorly understood. A subtype of MD characterized by endolymphatic sac (ES) underdevelopment (ES hypoplasia), frequent bilateral disease, and male predominance-termed MD-hp-has emerged as a promising model for genetic investigation. OBJECTIVE: To test whether the PHEX gene underlies the association of XLH with MD-hp. DESIGN, SETTING, AND PARTICIPANTS: This prospective, cross-sectional, study was conducted at tertiary academic centers in the US (Boston, Massachusetts) and Switzerland (Zurich) from Ja

Isolated oculomotor dysfunction with downbeat nystagmus as a rare presentation of anti-GAD autoimmunity.

BACKGROUND: Glutamic acid decarboxylase (GAD) antibodies are associated with a range of autoimmune neurological syndromes, including cerebellar ataxia (CA). Ocular motor disturbances such as nystagmus and diplopia are frequently observed in CA but rarely are seen as isolated manifestation. CASE PRESENTATION: We describe the case of a 60-year-old woman who presented with oculomotor dysfunction consisting of persistent diplopia and vertical nystagmus, in the absence of any other neurological manifestation. Brain MRI and PET-CT scans were unremarkable. Videonystagmography documented downbeat nys

The dual molecular identity of vestibular kinocilia bridges structural and functional traits of primary and motile cilia.

Vestibular hair cells (HCs) convert gravitational and head motion cues into neural signals through mechanotransduction, mediated by the hair bundle-a mechanically integrated organelle composed of stereocilia and a kinocilium. The kinocilium, a specialized form of primary cilium, remains incompletely defined in structure, molecular composition, and function. To elucidate its characteristics, we conducted single-cell RNA sequencing of adult vestibular and cochlear HCs, uncovering a selective enrichment of primary and motile cilia-associated genes in vestibular HCs, particularly those related to

Expression analysis of the TMC gene family in the auditory system during zebrafish embryonic development.

The transmembrane channel-like (TMC) gene family encodes membrane proteins required for hair cell mechanotransduction. While TMC1 and TMC2 are indispensable for mammalian hearing, the functions of other paralogs remain poorly defined. Using zebrafish, we examined nine TMC genes (tmc1, tmc2a, tmc2b, tmc3, tmc4, tmc5, tmc6a, tmc6b, and tmc8) through phylogenetic, transcriptomic, and spatiotemporal expression analyses. tmc1, tmc2a, and tmc2b were robustly expressed in hair cells of the otic vesicle and neuromasts, supporting their early roles in hair cell differentiation. tmc3 and tmc6a showed cl

Mouse models and translational research progress of hereditary vestibular dysfunction.

BackgroundHereditary vestibular dysfunctions (HVDs) are a group of diseases caused by genetic mutations, characterized by congenital or progressive vestibular dysfunction, often accompanied by hearing loss or other systemic damages. These diseases are divided into syndromic (e.g., Usher syndrome, CHARGE syndrome) and non-syndromic types, involving mutations in key genes such as MYO7A, COCH, SLC26A4, TMC1, etc. Although clinical phenotypes vary, the pathogenesis is complex, traditional diagnostic methods are limited, and effective treatments are lacking. Mouse models are important tools for stu

Type I hair cells of striolar and central zones in vestibular organs are essential for head stability and postural control.

The vestibular system of the inner ear provides head motion and orientation information required for maintaining balance and spatial orientation. Each of the five vestibular sensory organs contains type I and type II hair cells (HCs). Type I HCs are particularly notable for their evolutionary adaptability and unique calyceal synapses, in which the vestibular afferent nerve ending envelopes the HC body. In vitro studies indicate that calyceal synapses can transduce signals from HCs to afferents via nonquantal transmission, a mechanism proposed to be faster than conventional bouton synaptic tran

Balance Disorders: Insufficient Supply of Vestibular Examinations by the Italian National Health Service, 2021-2023.

Background: Vestibular tests are critical for an early detection of balance disorders, thereby reducing the risk of falls, particularly in older adults. The present is an ecologic study where the units of observation are the Italian Regions. Regional scores of access to essential levels of care (LEAs) were employed as predictors to investigate the hypothesis that vestibular examinations supplied by the Italian National Health Service (NHS) were insufficient to meet the needs of the Italian population. Methods: The number of first-level (clinical evaluations of vestibular function) and second-l
