Prediction of benign paroxysmal positional vertigo recurrence in postmenopausal women: a machine learning-based clinical study.

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is a common peripheral vestibular disorder with a high recurrence rate that significantly impairs patients' quality of life. Multiple studies have confirmed that postmenopausal women, due to endocrine disorders and other physiological characteristics, represent a high-risk population for both BPPV onset and recurrence. However, efficient recurrence prediction models are currently lacking, and traditional assessment methods have limited efficacy, making it difficult to meet the needs of individualized intervention. The aim of this study wa

Cramps as the initial presentation of CANVAS/RFC1-related disorder.

Biallelic mutations in the RFC1 gene are associated with CANVAS syndrome (cerebellar ataxia, neuropathy and vestibular areflexia). Peripheral nerve involvement in the form of sensory neuronopathy and small fiber autonomic axonopathy is a core feature of its presentation. Motor manifestations, in the other hand, remain poorly understood and incompletely characterized. We present two cases referred for evaluation of muscle cramps as the main clinical symptom, who were ultimately diagnosed with CANVAS. We also conducted a literature review on cramps and other motor manifestations as the predominan

Merlin restoration prevents schwannoma progression in a genetically engineered mouse model of NF2 -SWN.

PURPOSE: Patients with NF2 -related schwannomatosis ( NF2 -SWN) present with hallmark bilateral vestibular schwannomas, but also schwannomas on other cranial, spinal, and peripheral nerves, as well as meningiomas and ependymomas, caused by germline mutations in the tumor suppressor gene NF2 . Current therapies involving surgery and radiosurgery are effective for individual tumors but are not always a viable option for patients with multiple tumors, and harbor significant risk of neurological deficits and morbidity. Gene replacement therapy is becoming a promising new treatment strategy for sev

The role of magnesium in otology - A narrative review.

Magnesium is believed to have a positive and protective effect on inner ear disorders. We conducted a systematic literature search on the topic. The mesh terms "magnesium", "tinnitus", "sudden hearing loss", "vestibular disorders", "ototoxicity", and "otoprotection" were used for the Medline search. Based on this approach, a narrative review with a structured literature search was conducted. There is good evidence that supplementation with >200 mg magnesium daily protects against the development of age-related hearing loss. A similar protective effect can be assumed for protection against nois

Clinical characteristics and exploratory serum biomarker findings in vestibular migraine: a cross-sectional case-control study.

OBJECTIVE: Vestibular migraine (VM) is a common disorder in vertigo clinics, but its diagnosis remains challenging because of symptom overlap with other conditions and the lack of widely accepted objective biomarkers. This exploratory cross-sectional case-control study aimed to characterize the clinical features of VM, examine clinical and serum factors associated with VM case status, and assess the apparent ability of serum biomarkers to distinguish VM cases from healthy controls. METHODS: Between May and December 2025, 145 patients with VM and 50 healthy controls were enrolled at the First

OBJECTIVE: ChatGPT has gained popularity for its user-friendly applications in medicine, including otolaryngology. However, concerns have surfaced about accuracy of information, especially in languages other than English. This study assessed ChatGPT's proficiency in answering questions in English and French about benign paroxysmal positional vertigo (BPPV), a common ENT presentation. We aim to address concerns regarding its performance with medical queries in a language beyond its primary training set. METHODS: ChatGPT 3.5 was tested with questions taken from the guidelines of the American Ac

Distinct small-fiber dysfunction profiles in CMT1A and RFC1 disease: a multimodal study.

BACKGROUND: Charcot-Marie-Tooth disease 1A (CMT1A) and Cerebellar Ataxia, Neuropathy, Vestibular Areflexia Syndrome (CANVAS)/Replication Factor Complex subunit 1 (RFC1)-related disease affect large nerve fibers, but small fiber dysfunction may contribute to pain and dysautonomia. We applied a multimodal protocol, including potentials elicited by a micropatterned electrode targeting intraepidermal nerve endings, to characterize small fiber involvement. METHODS: In this cross-sectional study, healthy controls (HC) and patients with CMT1A or RFC1 disease underwent neurologic examination, autonom

Antisecretory Factor in rat cerebellar granule cells augments inhibitory current via GABAA receptors: a mechanism for vertigo reduction in Menière's disease?

UNLABELLED: In this review we propose a mechanism by which the antisecretory factor AF may decrease vertigo symptoms in Menière’s disease patients. This review is based on our previously published work and suggests a way of interpretation of the results obtained in view of the clinical effect of AF factor against vertigo caused by Menière’s disease. The Antisecretory Factor (AF) is an endogenous protein regulating water and ion transport across cellular membranes. AF was originally discovered and isolated from rat and pig pituitary gland but later it has been isolated from all mammalian and no

Aberrant Proliferation and Cell Fate Underlie Oral Defects in a Mouse Model of EvC Syndrome.

Primary cilia are microtubule-based organelles protruding from the surface of mammalian cells, which function as hubs for transducing both biochemical signals (such as Hedgehog) and mechanical force. Defects in cilia are associated with a group of genetic disorders called ciliopathies that disrupt the normal development of multiple organs, including the craniofacial skeleton. One such example is Ellis-van Creveld (EvC) syndrome, a ciliopathy mainly caused by mutations in EVC (EvC ciliary complex subunit1), a key component of the primary cilium essential for the transduction of Hedgehog signali

Case Report: Positioning head tilt observed in six dogs with meningoencephalitis of unknown origin.

Positioning head tilt (PHT) is a dynamic clinical neurological sign that is characterized by a head tilt to the opposite side of a voluntary lateral turn of the head. Based on recent publications, various etiologies are proposed for the occurrence of PHT in dogs and cats. One suggested cause is a lack of inhibitory input to the vestibular nuclei due to dysfunction of the cerebellar nodulus and uvula (NU). In that category, it has been reported in dogs with hypoplasia of the NU, dogs with lysosomal storage diseases, and in a dog with a cerebellar tumor. Other proposed causes of PHT include redu

Multimodal MRI of the reorganization of multisensory and sensorimotor networks in chronic bilateral vestibulopathy.

Chronic bilateral vestibulopathy (BVP) occurs preferably in elderly patients presenting with postural imbalance and head movement induced oscillopsia. The condition is often incomplete with residual functions in both ears. Beyond the vestibular reflexive deficits, an impairment of spatial orientation and navigation has been described associated with an atrophy of the hippocampal formation. However, this finding was inconsistent in various studies on rodents and humans. In the current MRI study on 15 BVP patients and 15 healthy controls (HC) we combined analyses of whole brain voxel-based morph

Clinical outcomes of percutaneous versus peroral intraoperative neuromonitoring strategies in transoral endoscopic thyroidectomy vestibular approach: A retrospective analysis of 2,201 patients.

BACKGROUND: Recurrent laryngeal nerve palsy remains a critical complication in transoral endoscopic thyroidectomy vestibular approach. While intraoperative neuromonitoring is employed, the optimal strategy for reducing vocal cord palsy remains a topic of debate. METHODS: This large-scale, retrospective cohort study, based on a prospectively collected database, enrolled 2,201 patients who underwent transoral endoscopic thyroidectomy vestibular approach. We compared outcomes among 3 distinct groups: nonintraoperative neuromonitoring (n = 109), peroral intraoperative neuromonitoring (n = 582, ye

Eight years of vestibular screening in children at increased risk: Results and perspectives.

BACKGROUND: Since vestibular hypofunction (VH) in children can affect motor and cognitive development, standardized vestibular screening is recommended for high-risk children. However, clinical evidence on screening outcomes remains limited. This study aimed to evaluate eight years of vestibular screening results and to investigate variations across different underlying conditions. METHODS: A retrospective chart review was performed, including 229 children (aged 0-18 years) who underwent vestibular screening between 2017 and 2024. Screening indications included congenital cytomegalovirus infe

Palliative care symptoms of people living with rapidly progressive prion diseases: a systematic review.

BACKGROUND: Creutzfeldt-Jakob disease (CJD) is a rapidly progressive, fatal dementia with an average prognosis of four to six months. Palliative care, as a holistic person-centred approach to supporting people and families affected by life-limiting illness, may be beneficial. However, despite the rapid disease trajectory, relatively little is known about the palliative care symptoms and concerns of this population. This systematic review identifies and reports the palliative care symptoms of people living with sporadic, iatrogenic, and acquired CJD. METHOD: Studies were included if they report

QTL mapping of starch, protein, and fatty acid traits reveals pleiotropic loci governing rice eating quality in a recombinant inbred lines population.

The circling rat is an autosomal recessive mutant (homozygous ci2/ci2) that displays lateralized circling behaviour, locomotor hyperactivity, ataxia and stereotypic head-movement. These abnormal behaviours occur in phases or bursts either spontaneously or in response to stress. Heterozygous (ci2/+) littermates display normal spontaneous behaviours. We have previously found that ci2/ci2 rats of both genders have a lower tissue content of dopamine in the striatum ipsilateral to the preferred direction of rotation, indicating that the rats turn away from the brain hemisphere with higher striatal

Enhanced activity of apramycin and apramycin-based combinations against Mycobacteroides abscessus.

BACKGROUND: Mycobacteroides abscessus is a rapidly growing non-tuberculous mycobacterium that causes chronic lung and soft tissue infections. Treatment options are severely limited. Amikacin has historically been a mainstay of combination treatment regimens. However, irreversible hearing loss and vestibular toxicity have led to a search for alternative agents. Apramycin is a novel aminoglycoside currently in Phase I clinical trials that may offer lower potential for ototoxic and renal toxic side effects. OBJECTIVES: The goal of this study was to compare apramycin's in vitro activity with amik
