A precision therapy paradigm for noise-induced hearing loss: integrating transcriptome-guided drug repurposing with in situ hydrogel delivery.

Noise-induced hearing loss (NIHL) constitutes a growing global health burden, yet effective pharmacological interventions remain elusive. Current therapeutic development is severely constrained by two critical bottlenecks: the lack of rationally identified molecular targets and the absence of dosage forms tailored to the physiological barriers of the inner ear. Addressing these limitations, we propose an integrated precision therapy strategy. First, to overcome the unpredictability of drug selection, we employed a transcriptome-guided network pharmacology approach, which rationally identified

A delayed pharmacological treatment strategy attenuates noise-induced tinnitus in rats.

It is widely accepted that tinnitus arises from maladaptive neuroplasticity in the central auditory system, which compensates for reduced peripheral input; notably, the loss of inner hair cell (IHC) synapses is considered a key underlying factor. IHC synapses can immediately and permanently degenerate after noise exposure, independent of hair cell loss, whereas the cell bodies and central projections of cochlear neurons can survive for months to years, providing an extended therapeutic window for improving afferent input through synapse regeneration. Using NHPN-1010 (a combination of 2,4-disul

Multifunctional Self-Assembling Peptide System for Preventing Noise-Induced Hearing Loss.

Functional self-assembling peptides prepared by linking short functional peptides to basic self-assembling peptides via solid-phase synthesis are often single-function and insufficient for preventing complex diseases, including noise-induced hearing loss (NIHL). Herein, a strategy to design multifunctional self‑assembling peptides (NPR36) for preventing NIHL is presented. Compared to the conventional approach of constructing multifunctional self-assembling peptide systems through co-assembling distinct functional peptides, this strategy integrates multiple functions into a single self-assembli

Metabolic-dysfunction-associated steatohepatitis impairs cochlear integrity and delays auditory recovery after noise trauma.

BACKGROUND: Chronic hearing impairment represents a major global health burden, profoundly diminishing quality of life due to its association with depression and cognitive decline. Recent studies have identified metabolic-dysfunction-associated steatohepatitis (MASH) as an independent systemic risk factor for accelerated hearing loss, implicating a functional liver–inner ear axis. However, the molecular and vascular mechanisms through which MASH-induced systemic inflammation compromises cochlear integrity remain uncertain. METHODS: We investigated the impact of MASH on cochlear pathology and a

Compound Danshen Dripping Pills: A potential anti-oxidative drug for the treatment of noise-induced hearing loss in mice.

Acoustic damage is a common cause of hearing loss, and excessive oxidative stress has been identified as a key pathology in noise-induced hearing loss (NIHL). Despite its widespread occurrence, there are currently no effective therapeutic drugs available for the treatment of NIHL. Therefore, antioxidant therapy is widely used for the treatment of NIHL. Compound Danshen Dripping Pills (CDDP) is a traditional medicine with antioxidant, anti-inflammatory and microcirculatory improvement properties. However, it remains unknown whether CDDP has a hearing-protective function. In this study, we demon

Cochlear histopathology in macaques after noise-induced temporary threshold shifts.

Noise exposures causing transient hearing loss were previously considered benign. However, recent work has revealed that temporary noise-induced threshold shifts may be associated with long-lasting cochlear histopathology. One such effect is cochlear synaptopathy, i.e. changes to the afferent synapse between inner hair cells and auditory nerve fibers. Noise-induced synaptopathy has been extensively characterized in several rodent models, and temporal bone studies suggest similar age-related changes in humans. However, it remains unclear how noise-induced temporary threshold shifts affect cochl

Marine-derived carbon dots: a safe and effective solution for noise-induced hearing loss.

Noise-induced hearing loss (NIHL) remains highly prevalent in occupational settings, and no pharmacological agent or combination thereof has demonstrated a protective or restorative impact against noise-induced damage in clinic till now. Biomass-derived carbon dots (CDs) are known for their unique biological activities and excellent safety. Marine organisms account for 78% of the planet's biomass. In this vast and diversified habitat, marine animals make and gather significant amounts of compounds with distinct structural and physiological properties. Here, using eight different marine fauna a

Attention to speech modulates distortion product otoacoustic emissions evoked by speech-derived stimuli in humans.

Humans are remarkably skilled at understanding speech in noisy environments. While segregation of different audio streams is mostly accomplished in the auditory cortex, neural feedback connections run from the cortex to the brainstem and to the cochlea. The latter organ not only houses the mechanosensitive hair cells, but also possesses an active process enabling it to amplify sound in a frequency-dependent manner. A physiological correlate of the active process are distortion-product otoacoustic emissions (DPOAEs) that can be measured non-invasively from the ear canal. Here we employed speech

Dual hearing protection effectively preserves outer hair cell function during 7 T MRI.

INTRODUCTION: Exposure to high acoustic noise levels generated during 7 Tesla (T) magnetic resonance imaging (MRI) may affect auditory function. To better understand these effects, this study examines how such noise exposure influences outer hair cell function in healthy adults, with a focus on the efficiency of a dual hearing protection method. METHODS: In this within-subjects longitudinal study, outer hair cell function was assessed in 39 participants (aged 18-41 years) using distortion product otoacoustic emissions (DPOAEs). Participants completed two 7 T MRI sessions on the same day while

Brimonidine Therapy for Protection From Noise-Induced Hearing Loss.

Noise exposure is a known cause of hearing loss, and only a few effective preventive drugs are available. Therefore, in this study, we aimed to investigate the protective effects of brimonidine on noise-induced inner ear hearing impairment in mice and explore its underlying mechanisms and long-term outcomes. Mice were randomly divided into control, noise exposure, and brimonidine groups. A 62-week follow-up was conducted after noise exposure. Brimonidine inhibited the noise-induced increase in inner ear glutamate concentration and downregulated inflammatory factors and immunoglobulins. Brimoni

Dexamethasone attenuates TNF-α-induced ototoxicity via Peroxiredoxin 6 upregulation in murine auditory hair cells and cochlea of noise-exposed mice.

Tumor necrosis factor-alpha (TNF-α) is one of the major cytokines that triggers damage to auditory hair cells, exerting sensorineural hearing loss. Dexamethasone (DEX) is widely used to reduce inflammation and is known to upregulate the expression of Peroxiredoxin 6 (Prdx6), an antioxidant enzyme. We previously reported that DEX pretreatment protected auditory hair cells from TNF-α-triggered damage. However, the protective molecular mechanism of DEX against TNF-α-triggered ototoxicity remains to be elucidated. In this study, we investigated the involvement of Prdx6 in the protective effect of

Dimethyl fumarate alleviates oxidative stress and inflammation in noise-induced hearing loss by activating Nrf2/HO-1 signaling in cochlear hair cells.

OBJECTIVES: Noise-induced hearing loss (NIHL) is an acquired sensorineural hearing impairment resulting from acoustic overexposure, primarily mediated by oxidative stress, inflammation and synaptic excitotoxicity. However, no FDA-approved drugs exist currently for NIHL. Dimethyl fumarate (DMF), an FDA-approved Nrf2 activator, is known for its antioxidant, anti-inflammatory and neuroprotective properties. This study aimed to investigate the therapeutic potential of DMF in mitigating NIHL by alleviating oxidative stress and inflammation. METHODS: Oxidative damage and apoptosis models were estab

Drug Discovery Targeting Nicotinic Acetylcholine Receptors for Hearing Loss.

Hearing loss is detrimental to human health, and currently, more than 1.5 billion people are affected by hearing loss. Active military personnel and construction workers are examples of individuals in the workplace who are exposed to loud noise and are at serious risk of hearing loss. While there is currently no therapy for hearing loss, evidence supports investigating the enhancement of the Medial Olivocochlear (MOC) system, an efferent pathway for hearing that serves as a gain-control for hearing loss protection. Selectively modulating the α9α10 nicotinic acetylcholine receptor (nAChRs) foun

Human Alpha1-Antitrypsin Demonstrated Protective Effects Against Auditory Impairment in Murine Model of Noise-Induced Hearing Loss.

OBJECTIVE: The pathophysiology of noise-induced hearing loss (NIHL) encompasses excessive inflammation, tissue damage, production of reactive oxygen species, and apoptotic processes, culminating in irreversible damage to hair cells. Currently, no clinical interventions are available to mitigate this condition. Alpha1-antitrypsin (AAT) is a circulating molecule with tissue-protective properties that increases during inflammatory states and facilitates the resolution of inflammation. Transgenic mice expressing elevated levels of human AAT have demonstrated enhanced tissue repair in various in vi

Studies of chitosan-Prussian blue nanozyme in auditory protection: from cellular mechanisms to in vivo validation.

Oxidative stress and inflammation are interconnected drivers of cellular damage in pathologies ranging from neurodegenerative disorders to sensorineural hearing loss. We aimed to develop a chitosan-Prussian blue nanozyme (CS-PB) to target these processes in sensorineural hearing loss. CS-PB (35 μg/mL) pretreatment of H2O2-injured HEI-OC1 cochlear cells for 4 h markedly decreased the levels of reactive oxygen species (ROS) inside cells (from 3.8 to 2.4 relative fluorescence intensity) by approximately 37% (P < 0.001)-and concomitantly significantly decreased expression of the oxidative damage m
