Molecularly defined auditory neuron subtypes show different vulnerabilities to noise- and age-related synaptopathy in mice.

Neuronal subtype-specific synaptopathy is a hallmark of many forms of neurodegeneration. We examined the cellular basis for synaptic vulnerability in the auditory system, where three subtypes of spiral ganglion neurons (SGNs)-Ia, Ib, and Ic-carry acoustic information from the cochlea to the brain. In response to noise and aging, a subset of synapses between inner hair cells and SGNs are lost, but it is unclear how this loss varies across SGN subtypes. Using genetic labeling, we showed that Ia SGNs have larger post-synaptic densities (PSDs) than Ib and Ic SGNs and are the most resilient subtype

The role of magnesium in otology - A narrative review.

Magnesium is believed to have a positive and protective effect on inner ear disorders. We conducted a systematic literature search on the topic. The mesh terms "magnesium", "tinnitus", "sudden hearing loss", "vestibular disorders", "ototoxicity", and "otoprotection" were used for the Medline search. Based on this approach, a narrative review with a structured literature search was conducted. There is good evidence that supplementation with >200 mg magnesium daily protects against the development of age-related hearing loss. A similar protective effect can be assumed for protection against nois

Comparing physiological signatures of synaptopathic noise exposure and carboplatin-induced inner-hair-cell dysfunction in chinchillas.

Aging and noise over-exposure lead to complex mixtures of cochlear degradation that impair the structure and function of outer hair cells (OHCs), inner hair cells (IHCs), and the cochlear nerve. However, IHC damage and cochlear synaptopathy (CS) remain pathologies "hidden" from the audiogram. This study aimed to compare and contrast the physiological signatures of these two popular and distinct exposure paradigms using promising non-invasive assays: Envelope Following Responses (EFRs), Auditory Brainstem Response (ABRs), Wideband Middle-Ear Muscle Reflexes (WB-MEMRs), and Distortion Product Ot

Perilymphatic ATP plays a critical role in modulating cochlear function to protect from loud sound induced hearing loss.

BACKGROUND: Extracellular adenosine triphosphate (ATP) signalling via purinergic receptors plays a key role in cochlear adaptation to loud sound. Traditionally, ATP activation of purinergic receptor P2X receptors has been proposed to induce a cation shunt, reducing the endolymphatic potential and the driving force for sound transduction. However, direct evidence for this protective mechanism remains limited. Here, we provide direct experimental evidence identifying a distinct, compartment-specific ATP signalling pathway that modulates cochlear function. METHODS: In mature Dunkin-Hartley guine

From receptor targets to drug delivery: pharmacological strategies against sensorineural hearing loss in the Cochlea.

This literature review explores in depth the molecular pharmacology and signaling networks in the cochlear microenvironment, a key organ for hearing. The cochlea converts sound vibrations into electrical signals using hair cells and a complex network of receptors and ion channels. Sensorineural hearing loss affects more than 1.5 billion people and is irreversible in adult mammals, underscoring the importance of understanding the molecular mechanisms underlying this condition to develop new therapies. Auditory transduction depends on mechanosensitive channels (TMC1/TMC2) and ionotropic and meta

Peripheral phoneme encoding and discrimination in aging and hearing impairment.

Speech perception difficulties in noise are common among older adults and individuals with hearing impairment, but also occur among younger adults whose audiometric thresholds are within the clinically-normal range. We examined how aging, cochlear synaptopathy (CS), and outer hair cell (OHC) damage affect speech encoding and phoneme discrimination. Envelope-following responses (EFRs) to rectangular amplitude-modulated (RAM) tones and speech-like phoneme pairs were recorded in quiet using EEG, and behavioral discrimination was assessed in quiet, and with ipsilateral and contralateral noise. Exploring the Feasibility of Mitigating Alcohol-Induced Instability Using Galvanic Vestibular Stimulation.

Alcohol consumption significantly impairs postural stability by disrupting vestibular function, increasing the risk of falls. This study investigated the feasibility of using galvanic vestibular stimulation (GVS) to mitigate alcohol-induced postural instability in healthy adults, employing acute alcohol intake as a transient model of vestibular dysfunction. Sixty participants were assigned to groups with or without alcohol consumption (estimated blood alcohol concentration [BAC] 0.06%-0.08%) and randomly received one of three GVS types: random noise (rnGVS), direct current plus random noise (r

Anatomy-based fitting for cochlear implant recipients.

PURPOSE OF REVIEW: Frequency-to-place mismatch is the discrepancy between the frequency information delivered by a cochlear implant electrode and the natural cochlear place frequency location that is stimulated. Frequency-to-place mismatch has been shown to negatively influence cochlear implant outcomes. Anatomy-based fitting (ABF) aligns filter frequencies with cochlear frequency place to minimize or eliminate frequency-to-place mismatches. This article reviews the current evidence on variability in electrode array placement, influence of frequency-to-place mismatches on cochlear implant reci

Noninvasive indirect intracochlear pressure monitoring during cochlear implantation.

Outcomes following cochlear implantation remain highly variable, particularly with respect to hearing preservation. While surgical techniques aim to minimize intracochlear trauma, no objective intraoperative method currently exists to monitor intracochlear pressure dynamics. We present a feasibility study of indirect, noninvasive monitoring of intracochlear pressure using a calibrated probe microphone positioned in the external auditory canal. Measurements were performed in fresh-frozen temporal bones and during five clinical cochlear implantation procedures. Ear-canal pressure signals were tr

An engineered ferritin nanocage co-delivery system for targeted hair cells protection and hearing loss attenuation.

RATIONALE: Hearing loss is a common sensory disorder with few treatment options, mainly due to the blood-labyrinth barrier and lack of effective targeted delivery systems. Small molecules such as LLY283 and RG108 protect hair cells (HCs), but clinical use is limited by poor cellular uptake, off-target toxicity, and inefficient cochlear delivery. METHODS: We developed a ferritin (Fn)-based nanocage (LR@Fn) to co-deliver RG108 and LLY283 via local injection through the round window membrane. Fn offers natural HC targeting, a hollow cavity, and good biocompatibility. Drugs were loaded using a pH

Temporal Modulation Enhances Medial Olivocochlear Reflex Efficacy: A TEOAE Suppression Study With Amplitude-Modulated Broadband Noise.

INTRODUCTION: The medial olivocochlear reflex (MOCR), an efferent feedback pathway, modulates cochlear gain to enhance signal detection in noise. It is commonly assessed non-invasively via contralateral suppression of transient-evoked otoacoustic emissions (TEOAEs). While unmodulated noise is a standard elicitor, ecologically relevant amplitude-modulated (AM) noises may be more effective activators, though literature findings are conflicting regarding the optimal modulation rate. This study aimed to compare the efficacy of unmodulated white noise (WN) and WN modulated at 10, 50, and 100 Hz (AM

Synaptic zinc plasticity shapes adaptive and maladaptive cortical plasticity following cochlear injury.

Cochlear damage triggers compensatory primary auditory cortex (A1) plasticity that amplifies responses to residual sensory inputs, thereby contributing to the restoration of both cortical responsiveness to sound and perceptual sound detection threshold. However, this adaptation can become maladaptive, producing neuronal hyperactivity that contributes to tinnitus and hyperacusis. The neuromodulatory mechanisms governing these adaptive and maladaptive changes remain unknown. Here, we demonstrate that noise-induced cochlear injury triggers bidirectional synaptic zinc signaling plasticity that pot

Convergent 3-6 kHz cochlear vulnerability in aircrew: matching threshold sound conditioning targets to operational flight profiles.

Commercial and military aircrew operate within a multi-hazard auditory environment that combines cabin acoustic noise, chronic hypobaric hypoxia under regulatory cabin-pressure caps, and altitude-dependent cosmic ionizing radiation. Their convergent action on cochlear vulnerability has not been quantified, and no decision framework exists for matching threshold sound conditioning (TSC) target frequencies to operational flight profiles. This work is a computational, hypothesis-generating framework rather than an empirical study. We propose that the dominant frequency band of cochlear vulnerabil

Normative Ranges for Auditory Brainstem Response Wave I Amplitude: A Potential Diagnostic Indicator of Cochlear Deafferentation.

PURPOSE: While cochlear synaptopathy has limited impact on auditory thresholds, there is increasing evidence that cochlear deafferentation is associated with auditory perceptual deficits. However, there is currently no means for diagnosing synaptopathy or deafferentation in individual patients. The objectives of this study were to establish normative ranges for auditory brainstem response (ABR) Wave I amplitude, a measure sensitive to synaptopathy in animals, in a population at low risk for cochlear deafferentation and then compare a population at high risk for deafferentation to the normative

Effects of noisy galvanic vestibular stimulation on reactive postural and neuromuscular responses to surface perturbations.

Postural control requires continuous sensorimotor feedback to maintain balance and prevent falls. Noisy galvanic vestibular stimulation (nGVS), a weak stochastic electrical stimulus applied to the peripheral vestibular apparatus, can reduce postural sway during unperturbed steady standing. Although nGVS is proposed to modulate balance by enhancing vestibular-mediated feedback control, the specific underlying neural mechanisms are unknown. Whether nGVS modulates balance during destabilizing perturbations has not been explored. We therefore interrogated the effect of nGVS on dynamic balance in r

FOXA2 Transcriptionally Activates SIRT1 to Inhibit Cochlear Ferroptosis in Noise-Induced Hearing Loss.

Background: Noise-induced hearing loss (NIHL) is a pervasive occupational health challenge, yet the individual susceptibility mechanisms remain unclear. Ferroptosis, an iron-dependent form of programmed cell death, has been implicated in cochlear hair cell damage. This study aims to elucidate the regulatory role of the transcription factor FOXA2 and the deacetylase SIRT1 in ferroptosis and to investigate the association between SIRT1 genetic polymorphisms and NIHL susceptibility in occupational populations. Methods: We employed a multi-level study design combining epidemiological investigation
