Dietary Intake of Enzymatically Processed Bee Larvae Mitigates Progressive Noise-Induced Hearing Impairment in Mice.

Bee larvae (BL) have a long history of use as both traditional medicine and food. Given that BL contain anti-inflammatory lipids and immune-modulating components, they may be efficacious against hearing loss and tinnitus. In this study, we investigated the effects of enzymatically processed BL on the suppression of hearing loss using mouse models of acute acoustic injury and noise-induced hearing loss (NIHL). In an assay where C57BL/6 mice were exposed to a single 5-min burst of 110 dB white noise, pre-treatment with BL was found to reduce transient acoustic damage. Furthermore, under conditio

Management of conductive and mixed hearing loss intolerant to air-conduction hearing aids: A stepwise algorithm and narrative review from a Japanese perspective.

OBJECTIVE: Conductive and mixed hearing loss in the complicated ear, including chronically inflamed ears with recurrent otorrhea, postoperative cavities, tympanic membrane lateralization, canal stenosis/atresia, and congenital malformations, remains a frequent, consequential problem in cases where conventional air-conduction hearing aids (ACHAs) are unusable or provide insufficient functional benefits. The expansion of the therapeutic landscape from non-implantable (e.g., cartilage conduction hearing aids [CCHA] and adhesive bone-conduction systems) to implantable (e.g., bone-conduction implan

Normative Ranges for Wideband Middle Ear Muscle Reflex Magnitude: Limited Potential for Diagnosing Cochlear Deafferentation.

PURPOSE: Cochlear synaptopathy, the loss of the synapses between the inner hair cells and their auditory nerve fiber targets, is expected to be a common type of auditory deficit resulting from noise exposure or aging. Unfortunately, there is currently no means for diagnosing cochlear synaptopathy or other forms of cochlear deafferentation. Wideband middle ear muscle reflexes (MEMRs) have been proposed as a potential diagnostic indicator of cochlear deafferentation, but we lack normative ranges for MEMR magnitude. The objective of this study was to develop normative ranges for wideband MEMR mag

Sudden Unilateral Hearing Loss and Vertigo in an Adolescent: Viral Labyrinthitis Versus Idiopathic Sudden Sensorineural Hearing Loss.

Labyrinthitis and idiopathic sudden sensorineural hearing loss (SSNHL) are uncommon in children and are both considered otologic emergencies, as delayed diagnosis can lead to permanent disability. We describe a 15-year-old boy with a history of focal cortical dysplasia and a single remote seizure who presented with abrupt left-sided hearing loss, tinnitus, vertigo, nausea, and headache. Neurologic examination revealed no focal deficits; however, marching Romberg testing demonstrated instability, and otoscopy revealed only a small left middle-ear effusion. Laboratory studies, including complete

Endolymphatic Sac Surgery in Refractory Ménière's Disease: Exploratory Associations and Postoperative Clinical Outcomes in a Bicentric Cohort.

Background/Objectives: Endolymphatic sac surgery (ELSS) is a non-destructive surgical option for medically refractory Ménière's disease (MD), yet factors influencing surgical outcomes remain poorly understood. This exploratory study aimed to describe clinical outcomes following ELSS and identify potential associations between preoperative characteristics and surgical success. Methods: This retrospective, bicentric cohort study included 45 patients with definite MD who underwent ELSS (predominantly endolymphatic duct blockage) between 2019 and 2024. Vertigo control was assessed using AAO-HNS cr

Labyrinthine hemorrhage: practical lessons from a rare cause of acute vestibular syndrome.

Labyrinthine hemorrhage (LH) is a rare cause of sudden sensorineural hearing loss (SNHL) and acute vestibular syndrome (AVS), which may mimic both peripheral and central etiologies. We report a case of a 76-year-old male on edoxaban therapy presenting with sudden right-sided SNHL and vertigo. Brain magnetic resonance imaging (MRI) performed 2 weeks later showed T1 and fluid-attenuated inversion recovery (FLAIR) hyperintensity of the right inner ear, consistent with LH. Evaluation of AVS in the emergency setting remains challenging, as LH may present as a rare stroke mimic, often indistinguisha

Various Surgical Techniques for Cochlear Implantation in an Ossified Cochlea: A Systematic Review.

BACKGROUND: Cochlear ossification, otherwise known as labyrinthitis ossificans, is not an uncommon encounter in candidates for cochlear implantation (CI) that may necessitate alternative surgical approaches due to the insertion difficulties. We aimed to review the literature for the documented approaches for CI in cases with cochlear ossification. METHODS: This review was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines on articles that described their surgical approach for CI in cases with cochlear ossification. CSNK1A1 Mediates Inner Ear Inflammation and Endolymphatic Hydrops in a Lipopolysaccharide-Induced Mouse Model.

OBJECTIVE: Our previous studies demonstrated that casein kinase 1α1 (CSNK1A1) was significantly upregulated in the endolymphatic sac of patients with Meniere's disease (MD). However, its specific role in the development of MD remains unclear. This study aims to investigate the effects of CSNK1A1 inhibition on alleviating hydrops, inflammation, and preserving audiovestibular function in a mouse model of endolymphatic hydrops (EH). STUDY DESIGN: Experimental animal study. SETTING: Translational research laboratory. METHODS: An EH model was induced in mice by postauricular lipopolysaccharide (

Cochlear Implantation Following Neurobrucellosis-Related Bilateral Profound Sensorineural Hearing Loss: A Case Report and Literature Review.

Brucellosis is the most common bacterial zoonosis worldwide, and neurological involvement - neurobrucellosis - can cause bilateral profound sensorineural hearing loss (SNHL) that does not respond to antibiotic therapy. We report cochlear implantation (CI) outcomes in a 60-year-old man with confirmed neurobrucellosis who developed bilateral profound SNHL that provided no functional benefit after a six-month binaural hearing aid trial. Preoperative temporal bone computed tomography and magnetic resonance imaging confirmed intact cochlear anatomy with no labyrinthitis ossificans on either modalit

Factors Associated With Bilateral Involvement in Non-Infectious Labyrinthitis: A Case-Control Study.

BACKGROUND: Non-infectious labyrinthitis (NIL) is an inflammatory disorder of the inner ear characterized by cochlear and vestibular symptoms associated with impairment of the blood-labyrinth barrier on delayed FLAIR MRI. Although the condition may initially present unilaterally, some patients subsequently develop bilateral disease, which can lead to greater functional disability. Identifying factors associated with bilateralization (i.e., progression from unilateral to bilateral involvement) remains clinically important. METHODS: We conducted a retrospective case-control study in a tertiary

Otologic Manifestations and Peripheral Facial Nerve Palsy As the Initial Presentation of Granulomatosis With Polyangiitis: A Case Report.

Granulomatosis with polyangiitis (GPA) is a rare antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis that predominantly affects the upper and lower respiratory tracts and the kidneys. Although otolaryngological manifestations are common, isolated otologic symptoms as the initial presentation are rare and may delay diagnosis. We report the case of a 65-year-old man who initially presented with refractory bilateral otitis media associated with progressive mixed hearing loss, vestibular dysfunction, and subsequent peripheral facial nerve palsy. Initial temporal bone imaging was unrem

Benign, persistent, and invasive: mechanistic and translational approaches to middle‑ear cholesteatoma.

Acquired middle-ear cholesteatoma is a histologically benign keratinizing squamous epithelial lesion that paradoxically exhibits locally destructive, recurrent, and invasive behavior, often resulting in ossicular erosion, hearing loss, labyrinthine fistula, and, rarely, intracranial complications. Surgical excision remains the primary management strategy; however, recurrence is common due to persistent microenvironmental drivers. Recent mechanistic studies-including single-cell transcriptomics, spatial proteomics, and epigenetic profiling-reveal a multifactorial pathogenesis orchestrated by ch
