Chronic suppurative otitis media (CSOM) is a common otologic disorder that may occasionally result in intracranial complications. However, extension of infection into the posterior cranial fossa through the facial (VII) and vestibulocochlear (VIII) nerve complex is exceedingly rare. We report the case of an 18-year-old male with a 7-year history of neglected bilateral ear discharge, hearing impairment, and left-sided lower motor neuron facial nerve palsy who presented with severe headache, vomiting, and seizures. Contrast-enhanced magnetic resonance imaging (MRI) demonstrated findings consiste

Complicated infections of the middle ear and paranasal sinuses remain a relevant clinical challenge. Although acute otitis media and acute sinusitis are generally self-limiting, a small but clinically significant proportion of cases progress to severe local, orbital, or intracranial complications. These include acute mastoiditis, labyrinthitis, facial nerve palsy, orbital complications, frontal bone osteomyelitis, intracranial abscess, meningitis, and cerebral venous sinus thrombosis, all of which are associated with considerable morbidity and potential mortality. The current review provides a

Acute mastoiditis represents the most common complication of acute otitis media in childhood and, despite a declining incidence in the era of vaccine prevention, remains a clinically significant entity. Particularly in infants and immunocompromised patients, symptoms may be nonspecific, thereby complicating early diagnosis. The present review provides a structured overview of the etiology, pathophysiology, diagnostic workup, and therapeutic management of acute and chronic mastoiditis, with special emphasis on interdisciplinary aspects. The pathogen spectrum is age-dependent and subject to epide

Complications of acute otitis media (AOM) are rare but can occur intratemporally or intracranially. Clinically relevant mastoiditis usually presents with subperiosteal abscesses, whereas subclinical opacification of the mastoid cells is common but does not require specific treatment. Other potential complications include labyrinthitis, facial nerve palsy, intratemporal abscess, cerebral venous sinus thrombosis, or meningitis. Diagnosis relies on clinical examination, laboratory tests, and targeted imaging. Intratemporal complications are primarily treated with antibiotics. Surgical interventio

This case report illustrates the difficulties that can arise when diagnosing a CNS infection. Following an episode of gastroenteritis, our patient developed dizziness, a fever up to 39 °C, photophobia, double vision, multiple cranial nerve deficits, and a resting nystagmus with alternating directions, and presented to the emergency department. Blood inflammatory markers were unremarkable, and a contrast-enhanced head CT scan revealed no abnormalities. The patient was discharged home with a provisional diagnosis of Meniere's disease. Two days later, the patient developed increasing lethargy and

BACKGROUND: Olfactory training is the most widely recommended treatment for smell loss; however, there are no randomised placebo-controlled trials evaluating its effectiveness in COVID-19. We aimed to evaluate the efficacy of isolated training and factors associated to olfactory recovery.
METHODS: This is a prospective randomised double-blind controlled trial, using standard olfactory training (OT) and placebo (PB) in COVID-19 patients experiencing smell loss. They were followed up for 180 days and assessed with the Connecticut olfactory test (CCCRC) and with subjective methods on a monthly b

BACKGROUND: Intra-aural tick infestations, though uncommon, pose a serious clinical challenge owing to the risk of acute labyrinthitis-an inflammatory condition of the inner ear. This inflammation can lead to severe complications such as sensorineural hearing loss, vertigo, and facial nerve palsy. Prompt recognition and management are crucial to prevent these adverse outcomes.
CASE PRESENTATION: A 21-year-old female patient of Limbu ethnicity from Dharan presented with sudden-onset right-sided otalgia, accompanied by vertigo, emesis, and auditory impairment. Otoscopic examination revealed a t

INTRODUCTION: Penetrating middle ear trauma-induced pneumolabyrinth is an exceedingly rare finding in pediatric patients. This condition can lead to significant hearing and vestibular dysfunction. We present two pediatric cases, highlighting their clinical complexity.
CASES PRESENTATION: Case 1 presented a 9-year-old boy with hearing loss, vertigo, and vomiting following a penetrating trauma to the left ear. Surgical exploration revealed ossicular chain disruption and perilymphatic fistula, which were repaired. Despite initial recovery, he developed ossifying labyrinthitis and required cochle

Norrie disease is a rare X-linked condition characterized by early childhood blindness and later onset sensorineural hearing loss. We report two male infants with genetically confirmed Norrie disease and characteristic ocular abnormalities consisting of bilateral funnel retinal detachments, anterior segment dysgenesis, and/or buphthalmos and microphthalmia. MRI demonstrated enhancement of the cranial nerves, cochleae, and cerebellum with cerebellar restricted diffusion. Intracranial findings mimicked meningitis, labyrinthitis, and cerebellitis. Neither infant showed clinical signs of infection

OBJECTIVE: We aimed to evaluate the efficacy of routine post-op X-ray in cochlear implantation patients.
STUDY DESIGN: Retrospective chart review study.
SETTING: Primary or revision cochlear implant patients who had routine postoperative X-ray (XR) or had planned postoperative computed tomography (CT) due to clinical concerns for array malposition.
METHODS: All images were reviewed, and those were considered abnormal if there was a bent tip, kinking, incomplete insertion, or if the electrode array didn't follow the expected cochlear curvature. Postoperative CT scans were performed in patien

The inner ear is an often overlooked system in elasmobranchs with few documented reports of disease or other abnormalities in the literature. Similar to terrestrial vertebrates, it is located in the cranium, and there are multiple components to the ear of elasmobranchs including a pair of membranous labyrinths each with three semicircular canals and four chambers or end organs (the saccule, the lagena, the utricle and the macula neglecta) making up the endolymphatic system (ELS). There is species variability among the inner ear anatomy of elasmobranchs, and this may play a role in disease deve

BACKGROUND: Benign paroxysmal positional vertigo (BPPV) is the most common cause of peripheral vertigo. BPPV is usually idiopathic but can also result from trauma, viral labyrinthitis, or Meniere's disease. We present a case of BPPV caused by earbuds.Case Descripton:This is the case of a 43-year-old male, a health care management and emergency medicine specialist, with no history of inner- or middle ear disease, vertigo and dizziness, or other medical conditions. The subject began using earbuds while driving and during routine exercise. He reported suffering from nausea and temporary vertigo a

OBJECTIVE: Temporal bone fractures (TBFs) can result in long-term adverse outcomes including meningitis, facial nerve disorders (FNDs), and hearing loss (HL) that may require surgical intervention. This epidemiologic study aims to examine the sequelae of adult TBF with HL, including the utilization of cochlear implantation (CI), using data from a large, multinational database.
METHODS: Using the TriNetX database, a retrospective cohort study was performed of adults (≥ 18 years) using ICD10 codes for other fractures of the base of the skull and hearing loss, an approximation of TBF with hearin

BACKGROUND: Labyrinthitis ossificans (LO) is the end stage of labyrinthitis. Existing case reports primarily center around hearing loss and overlook a crucial aspect that patients with LO frequently experience balance problems, while our study fills this gap. We report a rare case of bilateral labyrinthitis secondary to meningitis with recurrent fluctuations in cochlear and vestibular symptoms over more than a decade, and incomplete ossification of the labyrinth.
CASE DESCRIPTION: An 8-month-old female patient was diagnosed with meningitis, and later experienced developmental delays and heari

Dizziness and vertigo are common complaints, often linked to inner ear or neurological issues. Although inner ear houses both the cochlea and vestibule, the relationship between hearing loss and vertigo is still a longstanding controversy. Here, we probed systematic audiometry and vestibular function on 1115 patients, attending consultation for dizziness and vertigo. Peripheral vestibular pathologies accounted for 49.3% of the diagnoses, while 41.1% were attributed to central pathologies. Principal component analysis identified six distinct audiometric phenotypes, which were then correlated wi
